Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.

Consecuencia reportada: An unsecured wheelchair can move during transit, increasing the risk of injury.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Sienna Hybrid vehicles. The second-row seat rails may be welded improperly.

Consecuencia reportada: Improperly welded seat rails can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consecuencia reportada: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Toyota issued a recall in fourth quarter 2025 for this batch of Toyota Siennas that have bad welds on the second row seats and as a result of these, no one should be sitting in the second row. Toyota still doesn't have a fix in place for the 50k vehicles with this problem. Publicly Toyota says a fix is on the way, but at the dealership the service department has indicated that they've heard nothing on this. I am in a loaner from Toyota for the foreseeable future and it doesn't seem like I have much recouse through lemon laws based on the way Toyota issued the recall.

When we asked our dealership about it they acknowledged it, but said that there was no fix. Because we regularly take 2-4 passengers besides ourselves in our van, we asked for a replacement vehicle. They offered a "rental" which is just a loaner vehicle. The Highlander that they offered does not fill our needs. We've been without our van for nine weeks now, and there is no end date in sight. Over time, the trunk became increasingly difficult to open, progressing from what appeared to be a hands-free sensor issue to requiring a little manual force to open. June 3, 2026, approximately 5:30 PM EDT, I entered my 2025 Toyota Sienna to move it from the street beside my home, to my driveway. Upon closing the driver's door, the rear liftgate window exploded into the vehicle. This occurred without any warning or adverse action by myself or anyone else. The vehicle has been in no collisions, and has only 6429 miles on the odometer. I phoned Toyota's "Customer Experience Center" to report the incident and to seek warranty service for this problem. I was told to drive the vehicle to my Toyota Dealer, which is located 115 miles away, for inspection. I explained that it is unsafe to drive a vehicle filled with broken glass and with no rear window 115 miles. If I vacuum up the glass, there is nothing for the dealership technician to inspect. If I tape plastic over the opening, it could come loose at highway speed, and cause an accident, and the tape adhesive could damage the vehicle finish. The Toyota agent advised me that the vehicle must be inspected at a Toyota Dealership, and if the vehicle is unsafe to drive to the dealer, we could have it towed. But in a subsequent call to the "Customer Experience Center", my wife was informed that Toyota would not cover a tow of that distance in their warranty, and it was our fault for buying a car and taking it to an area that far from a dealer. The phone number we called is located inside the vehicle owner's manual cover under a heading, "If there is not a Toyota dealer near you, please call the following number:" In the morning, once they are open, I will call the nearest dealership, 115 miles distant, to seek further guidance. Based on my background in both mechanical engineering and automotive failure analysis, it is my opinion that the window and its support system was not designed to withstand the prolonged sun and heat exposure experienced. The outside temperature reached at least 82 degrees F where the vehicle was parked.

After very minor rainfall, my rear backup camera was not usable as water was dripping off of the edge of the door handle trim and it was getting on the lens. This is a complete design problem as there should be a drip edge. You made it very difficult to properly pull up at a gas station pump and I was unable to get close to the pump because of how the lens was obscured by the water drops.

Seat Belts2 quejas

Difficulty in pulling the seats belt on the second row seats when the car is turned off. Child has no wiggle room once it’s fastened and cannot undo seat belt as there’s no room to reach to unbuckle belt. I have to force the seat belt out of booster seat so belt can retract completely. The third row seats had a similar issue but didn’t have to force to out of booster seat for it to work properly.

Relatively new Toyota Sienna, bought 6 weeks ago and currently at 1300 miles. Passenger side 2nd row seat belt inexplicably became stuck and would not retract, making that seat unusable as the seat belt was no longer able to safely restrain a passenger. No accidents or unusual incidents were noted. This was completely unexpected and random following a routine short-distance errand.

My husband and I purchased a Sienna, 7-passenger van, April 2025 and have received two safety recalls, the first being the lack of second-row seats being installed correctly presenting a safety problem, and later a possible problem with the backup camera. We purchased a vehicle of that size especially for the convenience of transporting family when they visit. Also, we have a granddaughter, husband and one-year old great-grandson who live nearby and thought having a van would be helpful for trips together. Unfortunately, all our hopes were diminished at being able to enjoy the reason we purchased such an expensive vehicle, $65,000+. We are an elderly, retired couple, considered the vehicle an investment, because our two previous Toyota vehicles had lasted us for 15 years with no problems. Therefore hoping our daughter and grandchildren would be able to use the van for many years. The van has been nothing but an Albatross, not being able to drive it with only two passengers, in the front, since learning of the first recall on the second row seats. The local dealer told me if I’d get an appointment and be at their door at 7:00 a.m. they could repair the camera. BUT, dealers have no fix from Toyota for the second row seats. However, I went on line to the Toyota web site and they have a “FIX” process listed. I cannot imagine how a vehicle maker can be allowed to sell a vehicle when they find out it has such safety hazards? We consumers can beg our Legislators to pass laws to that effect, but, hope your agency will take the lead on protecting us consumers since we know this Congress will do nothing. I ask your agency to please take action on this issue and prod Toyota to do something concerning the major safety hazards with their 2025 model Sienna vans.

"We were notified by Toyota that it was unsafe to transport our children in the second row seats in November 2025 and the vehicle was under a recall. This puts our children at risk. A letter from NHTSA stated Toyota would have a remedy by 12/6. The manufacturer (Toyota) confirmed this recall. Given that Toyota has not indicated a solution date for this and the problem was discovered in October 2025 I believe the manufacturer has failed or is unable to remedy this safety recall for our vehicle in a timely manner and would like to officially log a complaint with NHTSA.

The affected component appears to be the rear seat / seat track recall system. I took my Toyota vehicle to the dealership / repair center in approximately October 2025 for recall-related repair, and the vehicle was then kept there for months. During that time, I was deprived of the use of the vehicle while continuing to make lease payments. I repeatedly contacted the repair center for updates but was not provided a clear repair completion date. Toyota and the dealership later claimed that the recall had been repaired only after the stated 60-day buyback review period had expired, yet NHTSA’s VIN lookup still showed the same recall as unrepaired for my vehicle. No credible current documentation was provided showing the exact recall repair completion date or the work performed. Under these circumstances, I am not comfortable accepting return of the vehicle. My children ride in the rear seat, and because this recall involves the seat / seat track area, I cannot reasonably be expected to trust the vehicle without clear proof that the recall repair was properly completed. I have also experienced a serious accident in the past, which makes this issue even more concerning to me. At this point, I have lost confidence in Toyota’s handling of this matter. I am seeking repurchase of the vehicle and fair final resolution.

See recall above. We just spent $55,000 on this vehicle in order to safely transport and travel with our two year old and newborn. The fact that Toyota does not have a remedy available for this major issue is completely unacceptable. The recall notice states that our only current "remedy" is to not use the second row seats, which is not feasible since child safety seats only safely install in the second row. This so called "remedy" is absolutely unreasonable, illogical and irrational. It essentially renders our $55,000 vehicle unusable, which constitutes a significant financial loss for my family. I am making a loss of use claim - Toyota's response that it does not have a remedy available now for such a significant safety problem is outrageous.

UNKNOWN Dear Toyota Motor Sales, U.S.A., Inc. Consumer Affairs Department: I am writing to formally demand a full buyback and refund for the above-referenced 2025 Toyota Sienna (the "Vehicle") under the Arkansas New Motor Vehicle Quality Assurance Act (the "Arkansas Lemon Law"), Ark. Code Ann. §§ 4-90-401 et seq. The Vehicle is subject to a critical safety recall (25TA12, with interim notice 25TB12) involving improperly welded second-row seat rails, which poses a substantial risk to the safety of myself, my passengers, and others on the road. As of the date of this letter, Toyota has issued no remedy for this defect and provided no estimated timeframe for when a repair will become available. This renders the Vehicle unusable for its intended business purpose and out of service indefinitely, in violation of the implied warranties of merchantability and fitness for a particular purpose, as well as the express warranties provided with the Vehicle. Background and Defect: I purchased the vehicle on [XXX], new from Landers Toyota NWA, Rogers AR for $49,058. The Vehicle has been registered and primarily used in Arkansas since acquisition. On or about October 7, 2025, Toyota issued Recall 25TA12, notifying owners that the second-row seats in certain 2025 Sienna models, including my VIN , are mounted on seat rails that may have been improperly welded. This defect could cause the seats to detach during a crash, increasing the risk of injury or death. An interim notice (25TB12) followed, confirming the issue but explicitly stating that no repair remedy is currently available and no loaner vehicles or temporary fixes are being provided. Since receiving notice of the recall, the Vehicle has been effectively out of service for safe operation. I cannot, in good conscience, use this vehicle with a known safety defect that compromises occupant protection in the event of an accident. Toyota has failed to provide any repair, temporary solution, or estimated completion date, leaving me without a functional vehicle for commercial purposes. As of today, the Vehicle has been out of service for approximately 21 days due to this unresolved nonconformity, and with no remedy in sight, it will soon exceed the 30-day threshold under Arkansas law. Violation of Arkansas Lemon Law: Under the Arkansas Lemon Law, a "nonconformity" includes any defect that substantially impairs the use, value, or safety of the Vehicle (Ark. Code Ann. § 4-90-403(6)). The seat rail defect clearly meets this definition, as it directly endangers passenger safety and renders the Vehicle unfit for ordinary use. Ark. Code Ann. § 4-90-410(a) establishes a rebuttable presumption that Toyota has had a reasonable number of attempts to repair the nonconformity—and thus must provide a refund or replacement—under the following conditions, among others: (2) The vehicle is out of service by reason of repair, or attempt to repair, any nonconformity for a cumulative total of thirty (30) calendar days. In this case, no repair is possible because Toyota has not developed or authorized a remedy. However, the Vehicle is undeniably "out of service" due to the inability to address this safety-critical defect, and the indefinite delay ensures it will remain so well beyond 30 calendar days. Courts interpreting similar lemon laws have held that irremediable safety recalls qualify vehicles as "out of service" from the date the owner is notified, as the owner reasonably withholds use until the hazard is resolved (see, e.g., analogous interpretations under federal Magnuson-Moss Warranty Act, 15 U.S.C. § 2301 et seq., which supplements state lemon laws). Toyota's failure to provide a repair within a reasonable time triggers the presumption of a "lemon" under § 4-90-410(a)(2), entitling me to the remedies outlined in Ark. Code Ann. § 4-90-412. Additionally, pursuant to Ark. Code Ann. § 4-90-405, INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
