Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2021-2025 Sienna Hybrid vehicles. The bolts for the seatbacks on the third-row seat assemblies may not be tightened properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. While applying the brake at the stop light. (25 to30 mile speed limit zone) The Toyota hybrid sienna start to accelerate for split 5 or more seconds. I have to apply the brake harder even though I was already applying the brake to slow down because the light was red and the are several cars in front of me. For the first time I was scared thinking about rear ending some one in front of me or the Toyota Lexus incident where it accelerated and everyone died flashing through my mind. On rare occasions (about once every 500 miles), when uniformly applying force to the brake pedal, the deceleration suddenly ceases and the vehicle seems to jerk forward. To a passenger, it feels like the driver's foot slipped off the brake pedal, but the driver continued to provide the same uniform force to the brake pedal and lost all deceleration. Typically, the driver is startled and presses significantly harder on the brake pedal jerking all occupants forward. The brakes re-engage and continue the deceleration at a much higher rate. Toyota addressed this problem with TSB 4724 which involved reprogramming the brake actuator assembly. This corrected the problem for about 3 months, but it is now occurring again. When the car was taken to the Toyota dealer again, the technician stated that TSB 4724 was cancelled, he could not duplicate the action (even though he was told it only happened on rare occasions), and claimed that the "brakes function properly as they should." Fortunately these loss of braking incidents have only occurred on dry pavement so that the driver did not lose control of the car when slamming on the brakes to compensate for the loss of deceleration. Coming down a hill on a snow covered road could have caused the vehicle to slide into an oncoming lane. It is my speculation that there is a null spot that exists during certain circumstances which causes there to be a "gap" between when this hybrid vehicle transitions between the dynamic hybrid brake and the friction wheel brake. Both braking systems independently work fine, but the transition between them (on rare occasions) does not occur continuously. This causes the driver to experience an unexpected loss of braking which, under the right conditions, could cause the driver to loose control of the forward motion of the car. As an engineer, I feel it is unacceptable for a safety system (braking) to not properly operate 100% of the time! On a Toyota owner's site, others have seen the same.

In general the braking system on this vehicle is poor. The van does not seem to want to stop unless you apply a lot of pressure on the brake pedal. I have taken the vehicle to the dealership who explained to me that the braking operation is normal. I replied “so the poor brake design was intentional”. The dealer said the braking system takes a little while to get used to. I have contacted Toyota corporate 5 times to express my concern about the brakes. The customer service representatives say that they will pass my concerns along. I personally own 6 vehicles. The brakes on this Toyota are by far the worst. It is not even close. I do not feel like this vehicle is safe. This is a minivan that is used to move families around. I do not feel safe in this vehicle and most importantly I am worried every time my wife drives this vehicle that someone will slam on the brakes in front of her the van will not stop in time. Toyota is the world’s largest auto maker, they can and should produce a better braking system than what is available with this van. The contact owned a 2024 Toyota Sienna Hybrid. The contact stated while driving at an undisclosed speed, the brake pedal was depressed, however, the vehicle failed to stop as intended. No warning lights were illuminated. The contact stated the vehicle was unstoppable. The contact stated the option was to crash into the three vehicles which were coming from the opposite direction or to crash into the pole to bring the vehicle to a stop and avoid injuring pedestrians. The paramedics and the Police department arrived at the scene of the crash. The contact's wife, who was the occupant of the front passenger's seat, was transported to the local hospital. The contact sustained bruises and an open wound which was continuing to heal. The contact's wife sustained bruises and bumps. The vehicle was towed to a local tow yard. The vehicle was condemned as a total loss. A police report was filed. The manufacturer was made aware of the failure. The manufacturer ordered an investigation of the black box. The investigation was completed, and the contact was provided a copy. The failure mileage was approximately 3,000.

When going down hill while braking, the brakes will release causing the car to lurch forward and then they grab again. It’s intermittent and Toyota has a TSB on it but the dealership with not fix it because they can’t duplicate the problem. I notice the driver seat had a clip that popped off unto my carpet, when inspected it came off the seat rail. While moving my seat back using the electronic seat lever a white clip with two metal bearings was sticking out, I notice the ball bearings were not in place and had came off the white clip. Yes, it is available for inspection. This pose a safety concern as the seat rails will not function correctly when moving seat or using the memory seat buttons for each driver. I have not brought the vehicle to a dealer to inspect as there is a estimate fee involved. I am currently out of my 2 year toyota care warranty. I am not seeing any warning lamps or messages besides the seat making a clicking noise when I move the seat back and forth.

When a lever is pulled, the second row seat can move freely back and forth on a track until the back of the seat is returned to the upright position. I was driving at highway speeds and had to brake suddenly. The seat was at the back of the track with the lever pulled, and it slammed into the back of the driver seat, hurting my back. It stuck there putting pressure on the seat (which could be felt) until I stopped and had to use force to make it slide back.

The third-row seat does not lock in the stowed position. As one goes over speed bumps etc. it bounces up and down. The is no positive lock to ensure the third-row seat is secured in the stowed position. In the event of a head on collision the seat may fly out and forward either killing or injuring the passengers.

Depending upon the speed of the vehicle, the seats might jump up several inches and almost return to the fully stowed position with a loud thud. Rear foldable seats do not lock down into compartment. They bounce up when you stop just a little hard. Cable has already broken when this occurs. Incident Description (Firestone Destination LE3 – P235/65R17) I purchased four new Firestone Destination LE3 tires (size P235/65R17, DOT IW2 8ILE31) and within the first week three of them developed noticeable bulges in the sidewalls. I returned to the tire service center, and they replaced the three tires due to the bulging. The following week, new bulges appeared again in the sidewalls on multiple replacement tires, for a total of six affected tires. Sidewall bulging creates a risk of sudden tire failure and loss of vehicle control, and I do not feel the tires are safe to drive on. One service center stated this was “normal from driving on roads,” but I disagree due to the number of tires affected in such a short time. The condition has been repeated and confirmed by multiple service centers. The tires remain available for inspection upon request. There were no warning lamps or messages before the bulges appeared. The issue began immediately after installation of the first set, and the problem persisted with the replacement set. In my assessment, the repeated sidewall bulging indicates a potential structural or manufacturing defect.

The lift gate strut bent when I tried to close the rear door, then when I tried to open the door to see what the issue was, the strut exploded. The parts went flying and almost hit me in the face, I could feel the air on my cheek as it blew past me. The spring and metal parts inside the strut shot all the way across my cul de sac, and the noise it created sounded like a gunshot. My neighbors came out of their houses to see what happened because it was so loud. We have an appointment at the dealer tomorrow to get it looked at. If I was standing any closer the spring would have hit me in the face; my children luckily were in the car already, if it happened a minute earlier it could have hit them. I noticed after the fact that the gaps in the back door are uneven when it’s closed, and I wonder if the door is installed unevenly and that put pressure on one side more than the other causing the strut to break. I can’t take a picture of the broken strut because I am too afraid to open the rear door in case the other one explodes.

Wobbling vibration on exterior mirror, driver side. It looks like incoming vehicles shake sideways. This happens on any bumpy road and it is easier to observe at highway speeds. Purchased on [XXX]. Vehicle is available for inspection. The distorted image resulting from mirror wobbling affects driver's awareness of surroundings, visibility gets worse at nights/rain due to blurred lights. This can lead to an accident. My regular dealerships in Milford, CT have confirmed the issue but have not been able to provide a fix. They say the vibration is normal. I do not believe so as the passenger side mirror never has issue like that. I have first noticed this issue a couple of weeks when driving on highway. Posting my case online has gotten at least 4 people reporting the same issue with the Sienna since 2021 in multiple trims. Driver side outside rear view mirror shakes and makes driver nauseous. Mirror was replaced in December and replacement mirror shakes as well. Right side (passenger) doesn't shake. Vehicle now has 6500 miles on it. Original equipment windshield has a wavy appearance for the right 1/3 of it (in front of the front passenger seat). Both issues have been brought up with Mitchell Toyota in San Angelo, TX. The dealer will not replace the driver side mirror again, saying "some movement is allowable", even though the passenger mirror doesn't do it at all. They also said that the windshield waviness is "normal". These issues are in addition to other issues I've had with this Sienna, from moldly HVAC smell that caused my eyes to sting and tear up last week.

When crossing a one-way road with oncoming traffic from the left, a vehicle that has recently passed might be triggering the forward collision avoidance system which cuts off throttle control to the Sienna, even when the passed vehicle is well out of the way. It is unknown if the collision avoidance feature is being triggered as there are no warning lights or messages in the display. This results in the Sienna having no accelerator response and sitting “dead in traffic” sideways in front of oncoming cars. This this is the second time this issue has occurred and today I was nearly t-boned by on-coming vehicles as there was no throttle response to move the car forward. This issue has been reported in 4th generation Sienna (‘21-‘24) forums on Facebook. This issue puts at risk vehicle occupants as well as other vehicles on the road.

Vehicle was driving on a slow traffic local street at roughly 10MPH. Spacing between car in front of me was around 30 ft. I was gliding towards front and ready to brake. Suddenly the vehicle start to accelerate hard by itself without driver’s operation. I tried to engage brake / gas pedal and was not helping. I am the original owner of a 2024 Toyota Sienna Hybrid (VIN: [XXX]). The vehicle has required two significant engine repairs within the first 57,000 miles of ownership. Both repairs were performed at authorized Toyota dealerships under warranty. INCIDENT 1 — ENGINE REPLACEMENT (10,000 MILES): At approximately 10,000 miles, the engine developed severe knocking. The dealership determined the engine was defective and performed a complete engine replacement under warranty. INCIDENT 2 — OIL LEAK (57,000 MILES): At approximately 57,000 miles, the engine developed an oil leak between the oil pan and the timing belt cover. The dealership repaired the leak under warranty. The dealership informed me that this repair carries only a one-year warranty on the work performed. The fact that the engine required complete replacement at only 10,000 miles indicates a manufacturing defect. The second engine issue at 57,000 miles suggests a pattern of powertrain reliability problems. I contacted the Toyota Brand Engagement Center (Case #XXX) and spoke with both an advocate and a supervisor. Toyota declined to offer any extended powertrain coverage and would only agree to review future issues on a "case-by-case basis." I am concerned about the safety and long-term reliability of this vehicle's powertrain given the recurring nature of these engine failures. A complete engine failure at 10,000 miles on a brand-new vehicle is not normal wear and tear — it is a defect. I am reporting this so that NHTSA is aware of potential systemic engine issues with the 2024 Toyota Sienna Hybrid. While driving on highway, on 3/11/26, car powered down. I had to coast to side of highway. I was able to turn vehicle on after several minutes, but SEVERAL system malfunction error messages (hybrid, accelerator, pre-collision, tpms, airbag, engine…) appeared. The vehicle would not accelerate beyond 10mph. Once I moved it to a safe location, I had the vehicle towed to the dealership. The dealership technician was able to replicate the issue during test drive. The technician apparently had the vehicle towed back to the dealership last night. The car has had other related issues since June 2024 when I bought it. The TPMS system, the accelerator system, and the airbag system have all malfunctioned independently. The accelerator system malfunctioned in Jan. I had to pay for a replacement fuse despite the warranty. The vehicle is modified with Braunability equipment for wheelchair access. These modifications are approved by Toyota and were completed by MobilityWorks. Aside from the vehicle itself being unsafe to drive, the passenger sliding door has had numerous problems. On 3/07/26, the door mysteriously slammed shut after opening and would not open again. These are all dangerous situations that could have resulted in fatal accidents.

The contact owns a 2024 Toyota Sienna. While the contact’s wife was driving 5 MPH out of a parking lot and attempting to make a left turn, the vehicle suddenly lost motive power and stalled. After approximately 3- 4 seconds while depressing the accelerator pedal several times, the vehicle eventually accelerated as needed. The vehicle was driven to the residence. There were no warning lights illuminated. The vehicle was not taken to the dealer to be diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was 68,000.

Keeps bottoming out suspension is specification is stated at 5.6 inches and I measured 3.3/4 unloaded never get a response on my complaints from my last repair and resolution on what they admitted that suspension was too low and wouldn’t put it in writing only saying that vantage mobility saying that it’s height requirements are as stated by vantage but no mention of ground clearance that I wanted them to verify that it’s specifications are stated at 6.5 inches and i measured 3 3/4 empty then they tell me not to go by specs and I’m a retired from wright Patterson Air Force base in the engineering department with logistics command and I’m like really guys you have to to have a specifications for a engineered system and they can’t even call back after they admit you have to go by specs! And then the authorized repair specialist tells me i had a bad brace on the front tire that was welded wrong and didn’t even mention it on the invoice or reported it to vantage mobility to call me about anything I had concerns about ?

There are no messages/warnings. The vehicle will surge at very low speeds such as when I am coming to a stop, trying to avoid a person in a parking lot etc. It was SO scary this evening as I accelerated into a very very busy intersection...I pressed on my brakes harder and it was instant - but about 1 second (which is a LOT in this scenario) later it then slowed instead of accelerated. I have owned the vehicle almost a year. I will be bringing in for inspection this week and let the dealer know. I have put on about 11K in miles and would say this has happened a dozen times thus far with this evening being the scariest one yet. I figured it was just something normal for a hybrid but as I researched online - many people have this issue. TOO SCARY for me and dont want to hurt anyone or myself.

Van purchased new October 2024. On two separate occasions, one in late 2024 and next today, [XXX], when pulling into street from a stop, there is no throttle response. We had a complete tire blowout of the rear driver tire in the desert, the temperature was 108° ambient. Toyota did not mention to us nor did the dealership that the car does not have a spare tire. We were 50 miles from any service Including cell service, I had to walk a mile to find cell service meanwhile two of our kids experience heat exhaustion. It took four hours to get the Vehicle towed. In that time our children experience heat exhaustion and had to be taken to the hospital. This is a huge oversight part to not make the customer aware that the vehicle especially family vehicle does not have safety equipment like a spare tire. In the desert this is a life and death issue that was not addressed by the dealership nor Toyota. All that is given is an inflation kit. We were not told by the dealership that a card did not come with What turns out to be essential equipment in the middle of a desert.

My child [XXX] ) engaged the automatic locking retractor (ALR) and as it wound back in it wrapped around his neck. By the time we got to him it was tight enough that it was difficult to move or rotate or slide the seatbelt over his head, and we ultimately cut the seatbelt to free him. In addition to the actual safety incident, I was very surprised to learn that this was even possible. I had never heard of this safety issue, and had absolutely no game plan or awareness about this issue. I saw the SBIR program addressing innovation in ALR systems which is exciting! However I would also strongly encourage some sort of awareness campaign or warning messaging to raise awareness and ensure parents keep belt cutters in their cars. Dear Sir/Madam, I am writing to formally raise a complaint regarding a serious and ongoing issue with my 2024 Toyota Sienna XLE AWD, purchased in April 2024 in Illinois. From the very first day, the trunk door has not functioned properly — often failing to close, randomly opening, or requiring manual force to shut. Despite repeated service visits across multiple states (IL, SC, NC) and investing considerable time, fuel, and lost income as a contractor, this issue persisted for over 18 months, creating safety concerns and inconvenience for me and my family. Key Issues: Trunk door opens on its own – even while reversing, creating serious safety risks. Malfunctioning power close mechanism, requiring manual closure frequently. Unreliable performance: door would close, then immediately re-open. Visited Toyota service centers at least 6 times across 3 states. Each visit took 2–3 hours, costing me work hours as a contractor. Customer service experience has been poor, especially with manager Patte, who: Failed to schedule proper service appointments. Directed me without coordination. Refused to escalate my request or connect me to her manager. Dismissed my request for compensation by saying, “It’s your issue.” Please help me on this issue. Thanks in advance.
