Icon Vehicle Dynamics, LLC (Icon) is recalling all 05-UP FSD 2.5 Centerline Stabilizer Kits with part number 65050. The kits were sold as an accessory for certain 2005-2021 Ford F-250 4X4 and F-350 4X4 pickup trucks. The foam ring inside the stabilizer "through shaft" may block the piston ports, possibly causing the steering to lock up.

During manufacturing, the windshield may not have been properly bonded to the vehicle, allowing it to detach in a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 212, "Windshield Mounting."

Consecuencia reportada: In the event of a crash, if the windshield separates from the vehicle, it can increase the risk of injury.

The owner's manual does not include instructions for adjusting or removing certain head restraints. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 202, "Head Restraints."

Ford Motor Company (Ford) is recalling certain 2017-2022 F-250 and F-350 Super Duty vehicles equipped with gasoline engines and aluminum driveshafts. Underbody heat and noise insulators may loosen and contact the aluminum driveshaft, which could damage the driveshaft and cause it to fracture.

Consecuencia reportada: A fractured driveshaft can cause a loss of drive power, or a loss of vehicle control if the driveshaft contacts the ground. Additionally, unintended movement could occur while parked if the parking brake is not engaged. Any of these scenarios can increase the risk of a crash.

A small void may be present in the top-center of the vehicle's underhood secondary fuel filter cap, which may progress to a pinhole over time, allowing fuel to leak from the top of the secondary fuel filter.

The failure created a potentially dangerous situation, as the loss of power limited my ability to maintain safe speeds and respond to surrounding traffic conditions. After the vehicle was delivered to the dealership, it took approximately one week for inspection and an additional two weeks to complete repairs. During the repair process, I was informed that newly installed components—including the fuel rail and two fuel injectors—were defective and required replacement again. This raises concerns about whether the root cause of the failure was properly identified and whether contamination or systemic failure within the fuel system was present. I am also aware of widely reported issues involving high-pressure fuel pump (HPFP) failures in Ford diesel engines, which can result in catastrophic damage to the entire fuel system, including injectors, fuel rails, and lines. Given the nature of my vehicle’s failure and the components involved, I am concerned that this incident may be related to a broader, known defect. This type of failure—sudden loss of power followed by complete shutdown—poses a serious safety risk, particularly if it occurs at highway speeds or in high-traffic conditions. I am requesting that this complaint be reviewed as part of a broader investigation into potential fuel system defects in Ford diesel trucks. While operating my vehicle on a busy road the power failed and vehicle shut down. Saftey of myself and others were at risk as this occurred while actively driving. Upon restart attempt vehicle failed to start. Vehicle was towed to and inspected by an independent service center. Mechanic advised, based on codes from check engine, the high pressure fuel pump failed and metal shavings were in my fuel system. Mechanic replaced pump, flushed system. Upon return of fixed vehicle power dropped out, check engine light appeard again. Returned to independent service center with codes of metal shavings in fuel sytem again. Pistons replaced, still being worked on for further issues. No prior warnings, check engine lights, recall notices etc. Contacted ford dealership, advised there was no fix and nothing they can do, advised to call corporate. Called corporate who advised I'd be notified via mail when a fix was available and no further help can be given despite the condition of the truck. Truck is unable to be driven due to constant power failures. Advised to tow truck and get a rental, both at my cost, until a fix was discovered which could be upwards of 1 year, keep receipts as I MAY be able to submit for reimbursement but it was too soon to know if those reimbursement services would be covered under a fix for the recall 24S78.

When my F-350 had only 2,265 miles on it, I was getting in my truck and closed the door. When it closed, the rear window shattered. There was no object that had been thrown at or through the window(no lawn service around that day, etc.). I immediately called the local Ford dealership(Landers Ford in Collierville, TN) and was told that the window would not be covered under warranty. I then called the Ford customer service # on the ford.com website and they stated that this was not a quality issue and that it would not be covered under warranty. It is clear by looking at the picture that the window shattered out as though the pressure in the truck when the door closed caused the break. This occurred on 9/10/21 and I took the truck to Safelite on 9/13/21 to have the window replaced at my own expense.

An unknown electrical issue possibly sensor, module or wiring related causes several faults in the system and they appear on the dash. These faults disable many safety features built in to the vehicle and a few convenience features as well. The faults displayed are: "pre-collision assist not available", "hill start assist warning", "service advance trac" and "drive mode not available" When this happens randomly the vehicle will stop producing forward acceleration and slow down very quickly as if I took my foot off the accelerator. When this occurs it is a dangerous situation made worse at higher speeds and I must quickly move off the road without causing an accident. Cold side intercooler boost tube cracked due to constant vibration impact from coolant overflow hose routed directly above and on top of it. Part and vehicle is available for inspection. The split in the boost hose caused me to loose power while attempting to merge onto a road, almost being hit by another vehicle. After research there are multiple other instances of complaints of the same thing happening to others and multiple aftermarket manufacturers producing replacement and upgraded parts with references and warnings of the same thing happening for the last 10 years. No warning lamps or symptoms prior to failure. After failure no warning lamps but could hear an audible hiss. The vehicle has not yet been inspected by the dealer or police. I was told by their parts people it is a regular failure and the part costs typically around 200 and they only sold it as a full part and not just the rubber replacement hose althought they are separate parts connected with hose clamps.

The Diesel Emission Fluid Injectors are becoming clogged due to crystalized DEF. This causes a DEF error to be generated which de-rates the vehicles speed over time from 50mph to 5 mph over a range of ~50 miles. In remote areas with desert, like where I live this can cause you to be limited to 5 mph on an 80 mph fwy which is incredibly dangerous, especially when some of the more remote fwys are 1-2 lane with limited emergency lanes for nearly 100 miles (especially over bridges). My understanding is this speed limitation is not due to the mechanical operation but is a manufacturer software introduced fault condition, which cannot be bypassed. This also appears to be a wide spread issue as my truck has been sitting in the Ford dealership lot for 2 months waiting for a new injector as these are nationally out of stock. Ford has changed the design on 2023-2024 trucks, the new parts are not forward and backward compatible. Was the design change due to reliability issues with their previous design, should there be a recall for existing customers. P.S. I have a 2021 F350 with less than 15000 miles.

I was recently informed by Ford that my vehicle is subject to a recall (21S31) because the rear axle may deform and detach from the vehicle. I believe the Ford Safety Recall Report (and NHTSA Recall Number) is 21V-536. My concern is related to the proposed remedy. Ford States that additional welds will be applied to the axle. Based on the information provided, I believe this will only mask the symptoms of a failure (detachment of the axle), and not the problem itself (crushing of the axle tubes due to insufficient tube wall thickness). I am a licensed professional engineer, which is to say that I have a basic understanding of material behaviour. With this in mind, I would respectfully ask you to consider the following: 1) Primary: Outright replacement of the defect axle housing with a unit that is manufactured with the "old design" axle tubes. If item 1 above is not chosen, I propose the following alternatives (in combination): A) Requiring replacement of the axle housing if paint loss is observed at the locations where the u-bolts are used to strap the axle to the spring perches. Many people have observed paint loss at this location, and it would indicate deformation of the tubing. B) Warrantying the axle housing for an extended period of time - perhaps 8 years or 100000 miles as was done by Toyota due to the frame defects which affected their pickups. C) Certification by a licensed professional engineer that the proposed remedy (welding the axle) will prevent deformation under rated conditions. I believe this is important because it will require a "person" to certify the adequacy of the repair. My intent in suggesting this is to ensure that the defect is not left in place. Again, the proposed remedy seems only to "mask" the problem. I fear it could result in another "Firestone Situation", where the public is put at unnecessary risk. I greatly appreciate you considering this request.

Ford motor company, and the local dealership have told me workarounds could be performed, but they would be done at my expense. A rearview camera on a pick up truck is used often and is a safety hazard when backing up Blind to low objects.

Ford knew there was an issue back in 2021 as they so filed with your agency. Yet waited. Then warranty claims continued. Now, four years later they file as a safety issue, BUT have told me twice in phone calls that I can drive the truck, despite it could shut down on it's own while driving. I tow donated RV trailers to the North Carolina disaster, now I cannot as my dealership advised I not go on long trips. Ford refuses a like loaner, or any loaner at all. This is a Ford issue, not mine nor the dealership and Ford needs to either replace all suspected parts, have a resolution now or proved no charge loaners of like kind to all owners. This approach by Ford is totally unacceptable as they in two phone calls to corporate never once said to not drive the truck and the letter I received said they are not advising we park the truck

Immediately truck lost power, a few other alerts flashed on the screen that couldn't read in time then alert that truck was being put in Neutral which it did. Smoke coming out of engine compartment. Barely coasted to stop on side of freeway. Quickly turned from white smoke to brown. Barely enough time to get me, wife and dog out of the cab before flames coming out of engine. Engine and cab completely burned out.

The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The contact stated that while depressing the accelerator pedal after stopping, the vehicle was slow to respond. The contact stated that there was several minutes before the vehicle responded as needed. The contact stated that the failure occurred after coming to a full stop. The contact stated there was no warning light illuminated. Additionally, the contact stated that while towing a loaded trailer, the vehicle was losing traction, and the rear end was swaying to the right and to the left. The contact had not taken the vehicle to a dealer to be diagnosed or repaired. The contact stated that during the failure there were several unknown warning lights illuminated. The contact stated that 3 months after purchasing the vehicle, the vehicle experienced brakes failure. The contact had taken the vehicle to an independent mechanic who diagnosed the vehicle and determined that all four brake pads and calipers needed to be replaced. The vehicle was repaired; however, the contact stated that the failure recurred while depressing the brake pedal, with an abnormal metal-on-metal sound coming from the brakes. The contact stated that the brakes were independently slowing the vehicle. The failure mileage was approximately 37,000. VIN tool confirms parts not available.

While driving my 21 F350 my truck would go into limp mode. The climate control stopped working, passenger window stopped, blind spot monitors, along with the mirror. After running forscan it showed every code possible. After an exhaustive search I finally found the main plug under the bed just above the spare tire. The wires inside of the insulation had corroded away to the point it had to be stripped back at least 1/2 inch before getting decent wires that wouldn't crumble when touched. This is a terrible place to put a plug that controls so much.

Transmission shifts erratically and sometimes completely doesn’t work. Has left me in traffic with no ability to negotiate vehicle and was almost hit on multiple occasions. There is a recall on vehicles built before 8-20 Kentucky but my vehicle build date is 11-20. The local dealer said nothing they can do until my 11-20 Kentucky build date truck is included. No offer of loaner and truck is NOT SAFE TO DRIVE !!!!! This has left me without.... transportation..... faith in Ford trucks Also. ..... same vehicle has the 7.3 ( Godzilla ). Dead miss in 1 cylinder. Internet information says not common but it has happened on multiple occasions. Stuck lifter which causes loss of oil pressure and scoring of cylinder walls. Local Ford dealership said replace 1 head with remanned parts and new pistons. My F350 has 58000 miles one would think it should last longer than that My thoughts. The contact owns a 2021 Ford F-350 equipped with AMP Tires, Tire Line: Terrain Pro A/TP, Tire Size: P35/12/R24, DOT Number: 04N9MATTW. The contact stated that while driving 70 MPH, the vehicle began to shudder. There was no warning light illuminated. As the vibration continued, the contact began to lose control of the vehicle and the vehicle nearly swayed off the road. The contact was able to pull to shoulder of the highway. Upon inspection of the vehicle, the contact's wife discovered a bulge on the center tread of the front driver's side tire. The contact replaced the tire with a spare tire and drove to the residence. However, the same failure occurred on the front passenger's side tire two weeks later. The contact notified the tire shop who had sold him the tires, and a mechanic was sent to his residence to inspect the tires. The mechanic diagnosed that the failure occurred as both tires' belts popped up. The driver's side tire was replaced but the passenger's tire was not replaced. The tire failure mileage was 9,000. The vehicle failure mileage was 34,000.

On 7 Aug 2022, I lost cruise control, Navigation function (GPS0 and had problems with other elecrical functions on instrument cluster. I took truck to my local ford dealer; they rand Ford diagnostics on truck and it had 3 module errors listed. They cleared the codes and tried to get me to accept the vehicle. Road tested it and there efforts of clearing the codes did not fix any thing. Brought back in shop on 10 Aug 2022. Diagnostics ran again now narrowed it down to APIM failing 3 times in 2 days. Now deallership says they have to order part from California if they have them in stock. This truck only has 22,000 miles on it. Has had multiple failures of equipment for fuel system listing codes P0191 2 and threee times per day. Dealer will NOT attempt to fix that problem. Just says deal with it.

Check engine light comes on almost everyday. After at least 5 minutes driving, this trucks engine will shut down with out notice. Usually when making a turn or coming to a stop light. When this happens you loose all steering control. You have to shift to neutral and try and restart. This has caused me to almost rearend a highway patrol vehicle. Very dangerous. I have had this vehicle in to the local Ford dealership on numerous occasions; however they will NOT run diagnostics to see what is happening. Have ad independant mechanic and my scan tool run tests and it always shows code P0191 fuel pressure rail faault. Maintenance is always uptodate ad fuel filters changed by dealer every 5,000 miles. Dealer and MF not honoring warrenty.

The contact owns a 2021 Ford F-350 towing a boat. The contact stated while driving 60 mph, applied the brakes however crashed head-on with a 2015 Toyota Scion. The contact stated the rear seats failed to lockdown. The contact had the tools under the seat and was released into the vehicle during the crash. The air bags deployed. The passenger sustained broken ribs, right ankle sprain or fractured, and the seatbelt bruised the chest. The driver struck the steering wheel and sustained chest pains, lower, mid, and upper back issues, and neck injuries. Medical attention was provided for both occupants. The driver of the Toyota succumbed to his/her injuries. A police report was filed. There was no reported fire. The vehicle was towed to an independent lot. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and referred the contact to NHTSA. The failure mile was approximately 6,600.

Transmission lunges forward at times even when putting into park position and starting vehicle. Also, when driving down the road vehicle jerks or bucks and throws you forward. Periodically, there is a complete loss of transmission function then you hear large bang and truck jerks forward. This has been reported to Fenton Ford Camden, NY several times and they cannot seem to find issue and stated if it doesnt duplicate happening when they have vehicle there is nothing they can do about it. This has become a serious safety concern when truck jerks forward when starting and park position and lunges forward in traffic. My setup: truck is level, trailer is 3/8" drop in front, and hitch ball up leaving one hole open from top. After installing product, ensuring proper setup, there is concern for towing safety, as the bottom of the drawbar drop is ~8-1/2" from ground, with reports from others that this setup has on numerous occasions, caused the drop portion of bar to contact pavement under normal towing conditions across the U.S. This has caused concern for possible catastrophic failure of drawbar drop when towing, believing that the welds on drawbar could become weakened and result in a disconnect of tow vehicle and trailer. I contacted the company, asking if they had plans to offer a 0" (would be an actual 3" length drop per their specs on the 6" and 8" drops) drop, or if they could say how much of the excess could be removed/cut from the drop safely. Their response is that there are no current plans to offer other drops and modifying the drawbar would void warranty. Others on various social media have warned of the drop making contact with pavement during the likes of National and State park entries/exits, as well as intersection transitions, etc.. I continued with my order/install, believing they had possibly installed incorrectly, but now see the concern, and do not want to void warranty, and in turn do not want to have a catastrophic failure that could kill someone..
