Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.

Remedio: Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. FCA US will notify dealers and begin notifying owners on or about: 03/21/2023 for 2021 model year vehicles, 05/02/2023 for 2019-2020 model year vehicles, and 06/15/2023 for 2017-2018 model year vehicles. Chrysler (FCA US, LLC) is recalling certain 2021-2022 Pacifica and 2021 Voyager vehicles. The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."

Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.

An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."

This vehicle has experienced three separate powertrain failures stemming from the same transmission defect (DTC P0A43 — Drive Motor Position Sensor, eFlite transmission). Incident 1 — April 19, 2026: Complete sudden loss of motive power on the highway with family aboard. Vehicle towed to Castle Rock Chrysler CDJR. Failed Repair — May 8, 2026: Chrysler corporate mandated a software flash rather than a hardware repair. Vehicle failed identically within 10 miles of pickup in live highway traffic. Towed back same day. Incident 3 — June 6, 2026: A replacement transmission was installed. Within one week of pickup, a progressive transmission whine developed followed by visible smoke from the engine compartment while driving. Vehicle was immediately pulled over and towed to avoid a third highway failure. Chrysler's own STAR engineering team (TSB 21-014-26) has documented this as a known defect requiring transmission rebuild or replacement. Chrysler blocked a full transmission replacement for over 5 weeks citing backorder, then located one within hours when a repurchase review was requested. This defect causes sudden loss of motive power at highway speeds with no warning, creating an immediate safety hazard. I am requesting NHTSA investigate this defect pattern on 2022 Chrysler Pacifica Hybrid vehicles and evaluate whether a safety recall is warranted.

We got a notice that said to pull over because the vehicle will shut down. We did and the vehicle shut down and went into red turtle mode only a few seconds after the indicator light appeared. This was unsafe in our situation and would be very unsafe at highway speeds or in busy traffic. The dealer has not been able to reproduce the issue (although has confirmed the error code) but will also not confirm that the car is safe for us to drive. The car is still at the dealer. They will not fix the car unless the error code is replicated. We do not feel comfortable driving the car until it is fixed or they are willing to confirm it is safe to drive. It has been more than 2 months with no progress.

I was driving on the highway when an alarm sounded and warning message displayed: "Stop Safely, Vehicle Will Shut Off Soon" At the same time, a turtle icon came on, along with the check engine light. I lost all power in the gas pedal. I pulled over, and after shutting my van down for a few minutes, I was able to turn it back on and get to my destination. However, the same exact issue happened on my way home. That time, I had to pull over -- still on the highway -- during rush hour and I did not feel safe as other cars zoomed by me. I often travel with my show dogs, which was my main reason for purchasing a new van. Naturally, I am concerned about their safety and well-being too. After this happened, I took my van -- which has less than 36,000 miles on it -- to my local dealership. They performed a software update and sent me home. I took my van on a "test ride" and the same alarm sounded and warning message displayed. I brought it back to the dealership and they investigated the problem further. I now have to replace the transmission -- a $7,000 repair -- on a van that is less than five years old with very low milage. I had NO prior issues, odd noises, or warnings indicated by my van.

Mazda dealership which I purchased the vehicle verified by running a diagnostic.engine light comes on after the vehicle shuts off. If I drive under 65mph it goes fine, but who wants to drive behind semis all the time on the freeway? Untrustworthy vehicle.

The contact owns a 2022 Chrysler Pacifica. The contact stated that while driving at an undisclosed speed, there was a loud sound coming from the engine with the message "Service Hybrid System" displayed. The sound was noticeable while accelerating and decelerating. The vehicle was taken to the dealer, but no DTC was retrieved; however, the contact was informed that the transmission control module needed to be reprogrammed. The TCM was reprogrammed; however, the issue persisted with a high-pitched whining sound. The manufacturer was notified of the issue. The approximate failure mileage was 48,000.

This car has lost power train at highway speeds several times almost casing accidents in major interstates intersections with no left shoulder. Chrysler has more than reasonably failed to repair this car transmission 9 times. This car has been in repair for 7 months in 2023 waiting for a new transmission. It has now spent more than 30 accumulative days just this year, and it is now at the dealership having its transmission replaced for the second time. I am submitting the second transmission replacement notice as evidence and first repair order as evidence. I have been able to avoid collision but have had to many close calls to the point that I feel unsafe driving this car and my wife won't drive this car anymore.

While driving several warning lights came on and the transmission shutdown. I coasted to the side of the road. The engine kept running but the van would not move. I shut the engine off and after a few minutes (5 to 10) I restarted the van and the warning lights were on on. I drove the van 2 or 3 miles and it happened again. Once again after a few minutes all the warning lights were off except the check engine light. I returned to my motel and called a local Chrysler Dealer. I then was able to drive to the dealer with no problem. When they diagnosed the problem they said it had several codes. The following is what the service department did. “Digit-verified cel stored codes p0973 in Tim and in pcm, abs bcm u0418 u0402, u422, alll are stored erased codes, no updates, removed air box to access ground and Tim connector, inspected harness from Tcm to transmission, removed and cleaned ground on trans, reassembled and test drove could not duplicate concern. After more test drives still unable to duplicate concern.” After this I was able to drive home 1500 miles without any problems. Not sure if the problem is fixed or not. I just wanted to report this just in case others are having the same problem and to build a case for a recall if needed.

Eventually stopped at a hotel for the night and left car at a dealership. Car is currently being looked at. No contact from the dealership yet.

The contact owns a 2022 Chrysler Pacifica Hybrid. The contact stated that while driving at undisclosed speeds, the transmission unexpectedly downshifted, with the image of a turtle displayed, and the message "Vehicle Speed May Be Limited" was displayed. The contact pulled over and waited for several minutes before restarting the vehicle. The vehicle was taken to two dealers to be diagnosed; however, the contact was not provided the diagnostic results by the dealers. The vehicle was not repaired. Upon investigation, the contact discovered NHTSA Action Number: PE22008 (Power Train); for which an investigation was launched by the NHTSA to address the failure. The manufacturer was notified of the failure and the contact was provided a case number. The failure mileage was approximately 37,000.

this weekend my wife was driving my kids home from out of town and was on the highway when she got a warning that said "Stop Safely, Vehicle will shut off soon". we were able to get home and I scheduled the van to be looked at by the dealership. today the dealership was able to look at the van and told me that the code while stored as no longer showing as active and that they could not help me. they told me the best thing for me to do was to continue driving and bring it back when the warning comes back, or the vehicle does completely lose power. I tried to explain to them my understanding of the situation, that there were issues with vans stopping unexpectedly and suddenly in a way that was unsafe. the recall (03A/NHTSA 23V-010) was not a fix to the underlying issue but merely provided ample warning to safely get the van off the road. I feel like the instructions I was given are a direct disregard for the recall in place. basically ignoring the warning that was put in place and asking me to continue driving until the event that triggered the recall happens anyway. This all seems to be confirmed by the NHTSA investigation that was started February of last year (Investigation: RQ24004 ) that confirms that the solution to the recall did nothing to stop the event, but only provided a better warning.

While driving on a road, vehicle came to a sudden stop and began bucking wildly. Dash indicator said to place vehicle in park, but gear selector knob was unresponsive the first few tries, but was eventually successful. Attempted to put in drive to move from roadway and vehicle began bucking again, bucking continued regardless of gear selected. Attempted to place vehicle in neutral for tow truck, and bucking continued in neutral. Two children and two adults were placed in danger when the vehicle came to a sudden stop on a road and could not be moved. Big O Chrysler Dodge Jeep Ram at 2645 Laurens Rd Greenville, SC 29607 received the vehicle for warranty repair. Big O was advised that the issue was likely a transmission failure common to this model and trim of vehicle. Big O's service technician replaced the fuel pump control module, which is a relatively low cost repair compared to servicing or replacing the faulty transmission. This repair was unrelated to the transmission failure and unlikely to resolve the issue. Big O returned the vehicle to the owner and the transmission issues continued in their parking lot. By returning the vehicle in an unrepaired state, Big O placed the safety of the driver in danger by clearing her to drive an unrepaired and unsafe vehicle on public roads.

The contact stated that the failure occurred while driving on a busy highway while driving out of state. The contact stated that when the failure occurred, it was such a scary situation. The contact contacted the Budget Rental Cars where the vehicle was rented. The contact stated that a replacement vehicle was provided. The manufacturer was not made aware of the failure. My vehicle is in perfect shape with no body damage and fewer than 28,000 miles. Today I was at a very busy intersection and when the car attempted to restart , it suddenly decided that my hood was open and it would not restart. It left me stranded. I ended up thinking quickly and pulling the hood latch exiting my vehicle and pushing the hood down. The vehicle it has the feature that stops the engine when I stop at a stop light. I do not think that the hood was open. I had it in for service over a month ago, that would have been the last time that anyone opened the hood. I was stranded, had to exit my vehicle in a very high traffic area. Luckily, the car restarted when I tried again. This default position is very dangerous. I turned off the feature that shuts off the engine at a stop. I understand a warning that the hood is open, but I do not understand stopping the vehicle from starting and placing the driver in a very dangerous position

While driving on the highway in rush hour traffic I got a message to pull over when it was safe to do so, and the car started losing power. The breakdown lane was in use for traffic, so I had to drive 1/2 mile to find a safe spot with the car going much more slowly than the flow of traffic. We were driving our van uphill and about to go through a stoplight when the van suddenly lost all power and multiple warning lights came on. We turned the car off and on several times over maybe 5-10 minutes, 3 stoplight cycles until we were able to get power back and proceed through the light, causing a lot of traffic backup and unsafe situation because drivers were going into oncoming traffic lanes to get around us. The car would not move at all until we got power back. It then was ok until a couple days later it lost power again. that time, it was on a flat road and able to creep the 3 blocks home at about 5mph. Presumably the first time, it couldn't move at all because we were on an uphill? We had it towed to Chrysler and the transmission was replaced. we have had multiple other issues with this car but our request for a buyback was denied. I am concerned about the safety and reliability of this car.

Last week on the freeway I suffered the issue described in 23V-010 while traveling at around ~75MPH. This was after the fix applied by the recall. My car suddenly began to slow and I was forced to navigate to the side of the road, and stop on the freeway with cars whizzing by, until I could turn off the vehicle and turn it back on. I am hoping to take the car into the dealer within the next few weeks once they confirm that a loaner vehicle is available. Otherwise, I am concerned about the car randomly losing significant propulsion during critical driving events (e.g. getting onto the freeway, changing lanes, etc) or in an area where I cannot pull over safely. Ironically, during my last service appointment I was provided the same vehicle as a loaner. The loaner vehicle from the dealer shut down on me 15 minutes after receiving it with the same issue. The headrest failed. The headrest pivots on the steel supports, and there's a ratchet inside, that is actuated by a plastic piece, which is then connected to cabling. The plastic piece internally broke it's actually unbelievably small for the application, the engineers should be ashamed of themselves. I'm going to guess 1/8" diameter, maximum, snapped right at the ratchet mechanism. If the weak plastic does happen to let loose in an accident, that headrest is going to flop forward and will not provide the head support that it's supposedly designed to provide. The way the headrest angles forward I worry about decapitation or severe neck injury causing quadriplegia in the event of an accident. This piece should work and be dependable not a tiny piece of plastic that often breaks with minimal use. We should know we have dependable headrest function in the event of an automobile crash.

Part 68324912AC Latch Release Cable failed on both middle seats within one week of one another and approximately three weeks after the manufacturers warranty expired on the vehicle. The result is that both seat backs do not latch into place vertically and the entire seat freely slides forward and backward, slamming backward on acceleration and forward on braking. We have read on Internet forums that this is a very common problem. The left side seat failed while my daughter was riding in it and the seat back released. If this had happened during a collision, it could’ve been catastrophic. Upon stopping the vehicle and exiting, we found that it slid freely forward and back as well. The other side failed while the vehicle is parked, and we released the seat back lever. The seats are unusable as a result. If the failure had happened while driving or impact, it could have been devastating as the seat would have slid independent of the seatbelt and the back slammed forward driving the passenger’s head into the back of the front seat. A Chrysler dealer diagnosed the problem charging us $250 for the diagnostic fee and quoted another $750 for the repair. Parts cost about $30 each. There were no warning indicators prior to the failure.

The vehicle experienced a failure of the hybrid battery thermal management system. A diagnostic trouble code (P0E15 – Hybrid/EV Battery Pack Coolant Heater “A” Control Performance) was set and the check engine light illuminated. This component is responsible for regulating high-voltage battery temperature and is part of the hybrid cooling/heating system required for safe operation and emissions readiness. The failure occurred at approximately 100,700 miles on a 2022 Chrysler Pacifica Hybrid. The battery coolant heater is a sealed, high-voltage electronic component with no maintenance or wear interval and is not a consumable part. Loss of proper battery temperature control can affect hybrid system operation, charging, regenerative braking, and power availability, and can potentially result in reduced power or limp-mode operation as the vehicle attempts to protect the high-voltage battery and inverter. The problem was confirmed by a Chrysler dealership and an Area Service Manager using factory diagnostic equipment. The fault code is stored and permanent. The vehicle was inspected by the dealer, and the component was diagnosed as failed and requiring replacement. No accident, misuse, or maintenance neglect was involved. The manufacturer declined warranty or goodwill coverage based on internal policy, despite the failure occurring immediately beyond the hybrid warranty mileage threshold and involving a non-wear, safety-critical hybrid thermal management component. The repair estimate is approximately $1,900 out of pocket. The warning indicator (check engine light) first appeared at the time of failure. The component remains available for inspection and replacement by the dealer. Also the dealership did not provide is with a written justification for the denial and corporate (Stellantis) stated that they were not able to provide justification either other than what the automated prompt was giving them (that the part was not covered under goodwill)

The vehicle is equipped with multizone AC, and one day the REAR passenger AC system malfunctioned. This occurred during the heatwave period this summer. The AC would not turn off, after multiple attempts, neither through the infotainment screen nor the manual buttons. We tried all possible ways to turn it off and it would not. Furthermore, it was automatically set to high heat, and the temperature also could not be changed. It is available for inspection if needed. Our [XXX] child was the rear seat occupant at the time, and was on the verge of overheating. We took the vehicle to the dealership, explained the issue in detail, and also showed them the clear video evidence of the problem. Because the issue was intermittent, they did not inspect it nor repaired it because it was working fine in their possession. Manufacturer was also notified of the issue, and repeated the same dialogue as the dealer, which is, that they can not resolve it if they can not reproduce it. There were no warning codes, lights, or messages for this issue. After receiving the vehicle back from the dealer, unrepaired, the problem happened again the same day. It is now back at another dealer hoping to be repaired and the issue diagnosed/reproduced. This is making us very nervous if it happens again, during a freeway drive or a long road trip, our road safety and children's health is at risk. The dealership and manufacturer have not demonstrated urgency for this matter, and treat it as any other "intermittent" issue. They advised us to bring it back if it occurs again. I would like to submit a formal complaint regarding the inadequate rental reimbursement being provided while a 2022 Chrysler Pacifica is undergoing recall repairs (Recall No. 226 – Ejection Mitigation). I took my vehicle to the dealership for recall repairs on March 12, 2026. The dealership does not provide loaner or rental vehicles. Chrysler Customer Care representatives advised that rental reimbursement would be limited to 60 per day. However, the actual cost of obtaining transportation exceeds this amount. Even a significantly smaller compact vehicle currently costs approximately 69 per day to rent. A comparable vehicle to the Chrysler Pacifica, such as a minivan or similar family-sized vehicle, is substantially more expensive than the reimbursement amount being offered. As a result, a compact vehicle that is significantly smaller and not comparable to the Chrysler Pacifica had to be rented. This vehicle does not meet the same functional needs, comfort level, or standard of transportation normally relied upon. Additionally, the difference between the rental cost and the reimbursement limit must be paid out-of-pocket. This situation creates an unfair financial burden resulting solely from a manufacturer recall that is outside of the consumer’s control. Customer Care has been contacted multiple times regarding this matter; however, no resolution or adjustment to the reimbursement limit has been offered. The current policy places additional financial strain on the consumer during the recall repair process. Assistance in reviewing this matter and encouraging a fair resolution would be greatly appreciated.

My 2022 Chrysler Pacifica PHEV has experienced three separate complete losses of motive power at highway speed, each preceded only by a brief dashboard warning to pull over safely. First occurrence: April 18, 2026. Second: May 7, 2026 — vehicle towed to Spitzer Motor City (Cleveland, OH), an authorized Stellantis dealer. Dealer replaced the Power Inverter Module hardware under warranty (R.O. 42463CY). Third occurrence: May 17, 2026 — just five days after the repair. Notably, the battery was at 40% on the third occurrence, meaning the engine had not cycled on, distinguishing this failure from the prior two. The root cause is the internal transmission wiring harness connector short identified in NHTSA Recall 03A (23V-010). Per NHTSA's own ODI investigation (RQ24-004), the software recall remedy does not prevent loss of motive power. TSB 21-008-23 / Warranty Extension XE1 identifies full transmission replacement as the correct fix. The dealer did not perform a transmission replacement. The safety risk is severe — complete, sudden loss of drive power at interstate speeds with minimal warning creates immediate rear-end collision risk. The defect has been confirmed by the authorized dealer. The transmission assembly is available for inspection. No police or insurance involvement. This VIN currently shows zero open recalls, suggesting the incomplete repair was logged as satisfying Recall 03A.

I was traveling on the freeway at approximately 65 miles per hour. I slowed to approximately 50 miles per hour to pass a disabled vehicle and began accelerating again when the dash displayed a message that said "Stop Safely- Vehicle will Shut off soon" the check engine light, a red turtle light also lit up. I had to make a quick maneuver to the far right lane putting myself and other vehicles at risk to avoid the car shutting down in the middle of the freeway. I wanted to get closer to an exit so I continued driving slowly on the shoulder. The wouldn’t accelerate, so whatever speed I let it get down to was the fastest it would go. At the exit I was concerned I wouldn’t have enough power to make it up the hill so I shut off the car. I let it sit for a couple of minutes and then turned it back on. It turned on with the check engine light still on . I then drove it straight to the dealership using surface streets and keeping my speed around 40-45 miles per hour. This was the second time this vehicle has experience this kind of loss of power. The first time happened in July 2024 and resulted in a full transmission replacement. This incident constitutes a dangerous loss of motive power at highway speeds. The failure mode appears identical to the issues described in NHTSA Safety Recall 23V-010 (Chrysler Recall 03A), yet this vehicle has already had a full transmission replacement for this exact issue in the past.

As the light turned green and she went to proceed, the car started up and then cut back off while simultaneously placing itself in park and all of the lights on the dash began blinking and messages stating "service shifter", "service parking brake", "service brakes" etc started popping up on the dash. She tried to restart the car and nothing happened and the emergency flashers would only work intermittently before shutting off and randomly coming back on for a few seconds minutes later. Eventually a police officer pulled up behind her to block traffic while she and they tried to get the car started. When I arrived, I was able to jump the car and get it out of the road and to a nearby parts store to check the batteries. Oreillys and Advanced Auto confirmed that the alternator, batteries and starter were good based on their diagnostic scanners. She was able to drive it to San Antonio and back the next day with no issues, but that night, when we went to dinner and stopped for gas, it did it again while in the fuel line although I was able to restart the car this time but now displays "Stop/Start Unavailable, Service Stop/Start". After the second event, went back to Oreillys and had it scanned, there were no primary diagnostic trouble codes, but 8 stored additional ABS trouble codes relating to low voltage, and electronic failures across multiple systems. This all came after having the oil changed and a safety inspection performed on 23 July in which everything passed, but they recommended a coolant and brake fluid flush. Concerned about this happening in congestion on a highway or somewhere with limited options for recovery.

Outside temperature was over 80 degrees, we were driving long distance on a highway. First the air conditioning failed. Then the "Remaining miles on battery" started to go up ( even though it had been a long time since we charged). When it got to about 12, we got a dashboard message that said "Engine shutting down, get to a safe place". We pulled over just in time as the engine did shut off, the shutdown happened in about 15 seconds. The car did not restart for at least 30 minutes. Message said something about "Check charging system". This happened repeatedly, leaving us in dangerous conditions on the side of the very busy highway. "Check Engine" light was on. At least once we had a message that said something about "Check Powertrain". One dealer checked the code on his computer, but could not help any further as they were too busy. He would not give us the paper with the code. We took the car to a dealer when we got home. The "Check Engine" light had turned off, but the dealer cannot find any problems with the car. It is a huge safety problem when your car shuts down with very little notice. We are afraid to drive the car now.

With several 18 wheelers having to brake and swerve behind us, we had to navigate onto the shoulder of the interstate. We had to wait on the side of the interstate for an hour for a tow truck to take us to the nearest dealership. We took the car into the dealership and they kept it for several days and then reported that the car just needed a new battery (12v, not hybrid). I expressed concern with the dealership and later after contacting Chrysler that a dead battery shouldn't cause the car to suddenly lose acceleration while driving, because that is an obvious safety risk, but was told again it was just a dead battery. I also received no invoice from the dealership and was told that it would be emailed to me as soon as they could since a cyberattack had made them unable to issue invoices.

Driving on the freeway going roughly 70 MPH, on electrical power, and several instances (XXX, XXX, XXX, XXX, XXX) the dashboard displayed 'Stop Safely, Vehicle will Shut Off Soon' with a red turtle icon, also known as Limp Mode. In all instances we were able to safely maneuver over to the shoulder, turn off the engine for 1-2 minutes, and turn the vehicle back on. In most instances, the Check Engine light remained for a period of time (sometimes just a few hours, others a day). I have taken the vehicle in to 2 separate Chrysler dealerships. The first at CDJR of Los Angeles, who sent a STAR team and was unable to diagnose/replicate the issue, so they claim the issue had been solved. The second at CDJR of LAX where they replaced the Power Inverter Module (and also tightened a loose nut, which was another safety concern). Since both of those visits, the issue has persisted, including the loose nut which made a clunk sound every time when accelerating. This is a serious safety concern. I have spoken with Stelantis, and they refuse to take responsibility to resolve the issue. They only provide the robotic response that 'since the dealer cannot replicate the issue, they cannot do anything.' Complete [XXX]. The dealer has acknowledge error code P0843 in my vehicle, but currently it is not 'active', whatever that means. This is a serious problem that needs to be fixed! This is a good vehicle when it works. I love the PHEV that after 2 years, I only have 16k miles, and most of my miles, as my daily driver, are on electric power. But the issue with this error are severe. Chrysler was aware that there were cars stopping without prior notification, so their solution was updating the software so the system would have these notifications to alert the driver. While driving 75MPH, the vehicle shut down with a message to pull over. All acceleration was lost. We had to navigate, without warning, across 3 lanes of traffic to pull over on the right side of the highway. This was EXTREMELY unsafe as we were slowing down and essentially cutting off other vehicles who were honking at us as this was a traffic hazard. All 6 members of my family were in the vehicle. We had this happen another 3 times before arriving home. I took it to my local dealer, who could not replicate it. They performed a software update, which I have since learned would NEVER fix this problem. We had it happen to us 5 more times, so I took it to another dealership, who initially could not replicate it. In all cases, we had the following check engine light codes be triggered. I have screen shots of them from my personal OBDII reader: P0A43 "Drive Motor 'A' Position Sensor Circuit Intermittent", C2225 "Status: MIL - PCM Disabled EDU: No Sub Type Information". After doing my own reading, it seemed many others have experienced the exact same symptoms and it has required a transmission repair/replace. I also learned most people had a battery charge on the high voltage battery (as this is a plug-in hybrid) when the vehicle shuts down. I charged my van, took it for a ride at 75MPH and it occurred within the first mile. I took it back to the dealership, and at this point, I refuse to drive it until the hardware fix happens. It seems like this should be a recall for all vehicles that can have this problem. It is EXTREMELY unsafe as it occurs at 70MPH+ speeds.

As I was driving on the highway, the warning "Stop Safely Vehicle Will Shut Off Soon" with a red turtle icon appeared. The warning indicator light also appeared. The car began to decelerate on its own as I tried to merge across several lanes of highway traffic to get to the shoulder and stop. This first happened on Thursday December 12 (also in a highway scenario). The vehicle was inspected by my dealer from December 16-18. During that time the dealer updated software and replaced drivers side PHEV engine mount. The dealer cites "Bulletin 21-042-24" as guidance for their troubleshooting. About 30 minutes later, after exiting the highway, another warning message was displayed: “Stop Safely Vehicle will shut off soon”. After about 30s later the motive power was stopped and the car continued rolling. There was no propulsion power anymore, and I was only able to roll into the entrance of a parking lot. Luckily it was a relatively safe location where the vehicle came to a stop. I was alone in the car and was not in direct danger. The vehicle could not be restarted. I called a towing service, and the car was transported to a Chrysler dealership. The fault codes were read out. Chrysler Star recommended to replace a 350 A fuse. The dealership service team drove the car for more than 70 miles without being able to reproduce the fault after this repair. One day after collecting the car from the dealership I drove about 300 miles on the highway. After about 250 miles the warning message “Service hybrid electric vehicle system“ appeared. The high voltage battery charging level was less than 1%. When driving on the highway with about 55 mph, about 150 miles after the warning message appeared, the motive power stopped inadvertently without any warning. I felt a short impulse on the gas pedal, then while rolling at below 50 mph the message “Switch P to start engine“ was displayed. I managed to roll out from the 3rd lane from the right to the emergency stopping lane. It was a dangerous maneuver and I feared for the safety of my five family members traveling with me in the car as the oncoming traffic behind my car didn’t slow down even after switching on the hazard lights. The vehicle could not be restarted and I had to call a towing service to transport the vehicle to the nearest Chrysler dealership. Now I am waiting for the service team to start reading out the failure codes.

My 2022 Chrysler Pacifica Hybrid has developed a dangerous rear brake system malfunction. During a dealer inspection on 4/27/26, the certified technician confirmed that the rear brake pads are only contacting approximately 50% of the rotor surface, leaving the remaining half completely unused and heavily corroded. This is not normal corrosion or wear. It is a mechanical failure in which the pads are not engaging the rotor correctly. The technician also stated that the pads were sticking in the caliper slides, which prevented full engagement. This is a functional defect in the brake hardware, not environmental corrosion. Corrosion does not selectively prevent half of a brake pad from engaging the rotor unless the caliper mechanism is not operating properly. This defect results in: Reduced rear braking force Increased stopping distance Excessive load on the front brakes Potential loss of stability during emergency braking Possible degradation of the parking brake The vehicle was serviced on 9/23/25 (tire rotation + NYS inspection) and 12/10/24 (four new tires + NYS inspection). No rear brake issues were identified at either visit. If this were normal corrosion, it would have been visible during those inspections. The sudden onset of severe partial engagement by 4/27/26 indicates a latent mechanical defect, not gradual wear. Chrysler/Stellantis has refused to classify this as a defect and instead labels it “corrosion,” despite the technician’s findings of pad binding in the slides and improper brake engagement. The manufacturer’s internal system blocks any warranty or engineering review because it incorrectly categorizes the failure as routine maintenance. This is a premature and unsafe brake system failure at only 33,000 miles. The braking system is not operating as designed, and the defect could lead to reduced braking performance or loss of control. Brakes have a skip feeling horribly. We took to garage after changing brakes and rotors 2x to be sure it wasnt the issue. But doesnt even feel safe to drive the garage took the whole front end apart down to the calipers and didnt find any issues so they questioned if maybe it wasnt a similar recall to the hybrid braking issue to do with the caliper? I had the power steering or from their repair journal (68525334-AB GEAR RACK 19075120) replaced at 7062 miles from new as it was making knocking noise from the front of the driver-side. now at 29,650 miles the exact same noise and sound is coming from the same side. Dealer stated this exact VIN had already received a rebuilt trans around 50k miles from another dealer. Took dealer 2+ months to repair, and had to call many many times to encourage them to push to get it completed. Manufacturer seemed to delay sending parts because they can - no good reason provided. First Power Inverter Module (PIM) was replaced, but that did not resolve the issue. Then dealer rebuilt entire transmission. Dealer lead tech stated that a roller bearing near the bottom of the trans appeared to have start failing, which then causes a short and or some kind of damage with the electrical "phasers". Multiple conversations with lead technician and Chrysler cares indicate clearly that Chrysler is NOT investigating the issue deeply (they did not want any of the failed components, nor did they ask for details from dealer - are they even trying to resolve root cause.... this has been occurring since the earliest models). Chrysler should have a financial incentive to fix to remediate warranty repairs. Why can't chat GPT find literally anything on the web about the root cause, why is it nothing other than reports of the issue can be found??? Nobody has done a root cause analysis. Is Chrysler paying to minimize bad press??? If Chrysler replaced entire trans with rebuilt one at 50k and now it failed again at 80k - when can I expect it to fail a third time?

6/21/25 , a Saturday, I stopped at a traffic light when gas saver function kicked in and shut car off. The car went into park mode by itself and parking brake engaged. The car would not start and had to be towed. It sat until Monday when I was able to start it and drive it to my mechanic's business a mile from my location and shut it off. Immediately after shutting it off it would not start again. My mechanic was able to start it a few days later and when he drove it about 5 miles it just shut off and wouldn't start again. Today he is replacing the starter and believes the gas saver function may have contributed to the increased wear on the starter leading to failure, as well as the heat from the catalytic converter located near the starter. I am very nervous about driving this car again and worry it may happen again when I am in traffic. This is a major safety problem if not resolved.

We purchased a 2022 Pacifica Hybrid in Feb of 2024 that is still under full factory warranty. We had the vehicle serviced once for A/c issues and it's regularly scheduled 30k mile service in April and May. [XXX] our van had a. Catastrophic failure in Texas, where it has remained since June 1st. The deep cycle hybrid battery overheated, making it inoperable. They have replaced the deep cycle hybrid battery twice-those batteries run 15-16k$, plus the additional parts and labor. Stellantis customer service promised rental reimbursement for the 2 vehicles we had to rent to continue our trip to Arizona. Then they declined to reimburse for both. They also stated they would pay for rentals ongoing while our van was in Texas and after we rented a van for two additional weeks, refused to pay for that. We requested a buyback at week 7 and that was denied. Two weeks ago, they stated the van was repaired and it sat in Texas while the dealership argued with Stellantis customer service about who was responsible for returning our van. Finally chrysler agreed to ship it to us and while the area manager was driving the van to the loading point for the shipping company, it had the same catastrophic failure. Chrysler had it towed to a different dealership in Texas and that dealership says they are going to start over and hopefully they can diagnose the issue fully, but they will have to replace the battery for a. 3rd time, totaling 4 batteries in 3 months or 64k$..The vehicle purchase price was 42k$. We purchased extensive extended warranties. We put down 24,500$ in cash and trade. We have since found out that a dealer in New Jersey put this car back on the market after they knew there was something wrong with it and made it someone else's problem. The selling dealership had no way of knowing the vehicle battery would overheat in the summer, as they purchased it from auction in November and sold it to us in Feb. In May 2025 shortly after purchasing this vehicle I had complete Brake failure while driving down the road. I was driving approximately 65 mph when the forward collision warning, ABS warning, Brake Warning, Auto Emergency Brake warning lights came on and when I pushed on the brake pedal it was hard as a rock and wouldn't stop the vehicle. It was returned to the purchasing dealership, and they replaced the Brake Booster. On November 27th of 2025 I was driving down the road and the exact same problem occurred with a complete loss of brake function. It went to the local Chrysler Dealership; they replaced the Master Cylinder. I picked it up 12/10 drove it 5 miles home and parked it. I got in the vehicle the next morning to go to work and on my way to work all the same dash lights came on and I had a complete loss of Brake function. It was towed back to the dealership again; this time they replaced the ABS module. I picked it up on 12/30 drove 5 miles home, parked it, tried to leave 3 hours later and couldn't push the brake pedal far enough to get the van started. When I did finally get it started all the same dash lights were on and the brakes were not functional. It was towed back to the dealership again and this time they replaced the wiring Harness. I picked it up 1/16. Drove it minimally over the weekend, on 1/19 the front-end collision warning lights came on and the check engine light came on while driving 70 mph. The brakes didn't lose function this time, however. The following day I was sitting in line at an elementary school waiting to pick up my daughter when all the same dash lights came on again and I had a complete loss of brakes, it was towed back to the dealership, where it still is. There was contact made with Chrysler engineering during the 2nd repair. The dealership has been able to recreate the problem more than once since they have had it. This put myself and my children a significant risk of injury, and other cars if there was an accident.

Shortly into my trip home from work, I entered onto the interstate and accelerated up to speed. Within a minute the vehicle seemed to slow down and I had no power using the accelerator Pedal. The vehicle continued to slow and the gas engine failed to turn on.(this is a PHEV hybrid model). Eventually the vehicle lost all power and I was able to park it along the side of the interstate. The vehicle was still on, AC and electoral systems worked. After about 5 minutes, gas engine spontaneously started back up. I was able to gain speed start bad down the road. Approximately 3 min later, gas engine shuts down and same process occurred. This cycle took place about 6 times before vehicle would not start up anymore and ultimately had to be towed. After a week, dealership is not sure what’s wrong as the transmission has been replaced which did not fix the problem. Dealership is now planning to replace the PIM module although there is an over 45day wait and they are unsure if that will fix the problem.

I was driving the vehicle in the middle lane of a 3-lane (one direction) interstate highway when a message appeared on the dashboard "stop safely vehicle will shut off soon," the check engine light turned on and the vehicle lost all ability to accelerate so it was extremely challenging to navigate to the side of the road safely through busy rush hour traffic. As the driver I had zero ability to accelerate the vehicle whatsoever, the gas pedal did absolutely nothing, which can lead to an accident on the highway due to other vehicles moving very quickly and not expecting a vehicle that is not at the right of the road to come to a stop. I brought the vehicle to the Chrysler Dealership where they initially stated this is a known issue and they would be replacing the transmission and that the transmission is under 10-year/unliminted miles warranty for this specific error (POA43 Driver Motor), however once the dealership spoke with Chrysler corporate, they indicated that they would simply be clearing the code and they returned the car to us. We were advised to bring the vehicle back when the error recurs. We opted to sell the vehicle immediately instead, because it it not safe to drive now, but unfortunately this means we are passing the problem on to another consumer as Chrysler refuses to fix the problem. This vehicle shuts down in the middle of the highway causing us to come to a complete stop while driving on the highway, also on back roads one lane roads, the vehicle completely shuts down causing us to block the road and becomes a hazard. This has been a continuous problem. This will now be the fifth time it has been to the repair shop for the same exact problem. It is covered under the warranty, however, when they fix it, it happens right away within two business days, again without warning causing us To be in danger and becomes a hazard anytime we drive the vehicle. The vehicle has been inspected by the dealership service center many times. They also sent out an engineer from their company to inspect it. We now have no warning like before the problem occurs only after the vehicle loses power. This will be the third complaint I've filed for this same issue. after it's been "fixed and inspected" this last time.

1. Most emergency lights come on while the vehicle's brake engages automatically. The brake then pumps up and down on its own if any pressure is applied. Shortly after, the vehicle goes into "limp mode". The vehicle is currently at a Chrysler Dealership service department and available for inspection. 2. This has occurred 8 times since the vehicle was purchased in October 2024. All but one time, it has happened while driving. Once it happened while on a major highway. I had to quickly veer into a turn lane so our vehicle was not rearended by another car. Another time, I was driving my children to school and it happened about 10 seconds before crossing the worst intersection in our city. That could have resulted in a major crash based on the history of that intersection. A third time, I was traveling at about 40 miles an hour when warnings came on and the brake started to engage. Luckily, I had just gotten off a major highway and I was able to get to the side of the road. 3. The problem has been confirmed by a dealer, since it happened while I was in their service parking lot. However, they have been unable to recreate, find the source of, or fix the issue after 7 times of servicing it. 4. I am unsure whether the manufacturer has sent anyone outside of the dealer's service department to inspect the vehicle. We have been in contact with the manufacturer directly since December 26, 2025 in relation to this issue. Our extended warranty company may have had an inspector out on the first visit to the dealership for the issue, but no one since that I am aware of. 5. Emergency messages have included: Service Electronic Stability Control, Service Charging System, Service Hybrid Electronic Vehicle System, Auto Emergency Braking Unavailable Service Required. Multiple emergency lights come on including: Check Engine, Electronic Throttle Control, Electronic Stability Control Active, Service Forward Collision Warning. I was in an accident. Where the van rolled 17 times. On or about the 12th roll. The retaining lock on the shoulder strap released. This allowed me to slide out of the seatbelt. We purposely got a seven passenger van to transport our entire family, and at this point we have no way to do. So. The dealership says they are unable to give us a loaner for that, and we are getting no assistance with this with no timetable. As far as when the vehicle will be fixed. Therefore, I’m requesting a buyback of the vehicle so that we can get something to transport our family as the vehicle is down. I am still having to pay for the car note without having anything to Show for it. I cannot drive it as I don’t know when it will shut off. It is currently sitting at the dealer.

The contact stated that the accelerator pedal failed to respond while being depressed. The contact stated that the failure had occurred three times and occurred while driving at various speeds. The contact stated the failure also occurred while driving at 70 MPH. The vehicle was taken to an independent mechanic who was unable to retrieve any fault codes but informed the contact that the 12-Volt battery was not at full charge. The vehicle was taken to a local dealer, where it was diagnosed, and determined that the accelerator pedal assembly and the main computer had produced fault codes, but the dealer later stated that there were no codes. The vehicle was not repaired. The manufacturer was informed of the failure and a case was opened but was later closed. The failure mileage was 41,607.

The van was going about 10 mph and the side wall just burst and went to flat right away and tire pressure warning came on. Yokohama had a recall for this same issue in 2022 but this DOT was not included. Nothing came in contact with tire or van when the issue occurred. This would have been a major issue if going full speed. Discount tire in Southaven MS stated the tire just gave out with no indication of anything hitting the tire. Yokohama refused to do anything as this is a BraunAbility handicap van which shows them as owner 1 to do the conversion through Chrysler. These Vans cannot be purchased through Chrysler directly. Yokohama did now even offer to report this issue internally as a complaint on the tire. I kept the tire for further inspection and investigation. I feel very scared and confused when I am driving on [XXX] and the car stops like that. I feel like our lives are in danger while driving. Then "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol appear more frequently. My car has appeared "Stop Safely Vehicle Will Shut Off Soon" and the turtle symbol 6 times in 3 days. The vehicle experiences sudden power loss due to the following error codes: P079A-00 (transmission issue), P0A43-00 (hybrid battery issue), and P0A7D-00 (battery voltage issue). These problems have nearly caused accidents, including: On the highway: Power loss while overtaking, leaving me in a dangerous situation. In city traffic: Repeated power loss, almost causing collisions. The issues are well-documented among Pacifica Hybrid owners in the USA. NHTSA has received numerous complaints, and there are active class-action lawsuits due to unresolved problems. My vehicle was involved in a minor collision in the USA, classified as "clear title" (cosmetic damage only, no impact on the drivetrain or battery). Repairs were done professionally, and there is no connection to the current issues. I have contacted Chrysler and the authorized service center in Berlin, but received no response. During 3 separate drives (October11, 2024; December 30, 2024, and January 2, 2025) the vehicle shut off while going uphill on an interstate while the engine was at 3,000 RPM. When I pulled over to the side of the highway and stopped the car, the emergency parking break automatically came on. I had to restart the vehicle 2 or more times in each instance. On January second, the vehicle shut down 5 separate times, some on Interstate 70 and some on US Route 40, all while going uphill between 2000 and 3000rpm. I took it to a local Chrysler dealer January 8th, but they could not recreate the problem or explain why the vehicle was shutting off with no warning.

Driving on highway and car shut off with a "Stop safely soon / vehicle turning off" warning message, a red turtle warning, and the Malfunction Indicator Light on. Pulled off on shoulder. Turned car off. Turned car back on. Malfunction Indicator Light remained on but red turtle was gone. I was able to get to my destination. Malfunction Indicator Light remained on for next few driving cycles, but then went off after fourth drive. My safety was definitely put at risk as I was driving on the highway at 60-70MPH when the car just shut down and slowly decelerated on its own. Because the MIL ultimately went off after my fourth drive, I thought I was in the clear--as the Chrysler manual says this light can come on if you've had a recent servicing because the on-board diagnostics may just be recalibrating. I had my car serviced 9 days previous to the incident. The manual says if the light doesn't go off on its own, then bring it in. It went off on its own, so I thought I was in the clear. However I see in online forums this has happened to others multiple times and I'm worried it will happen to me again. I am calling my dealer tomorrow. Thank you for taking this report!

I was renting this vehicle. On my way home on the highway, going about 70 mph, the vehicle suddenly illuminated the check engine light. Soon after, an message appeared on the dash warning me that I needed to stop and that the vehicle would shut off soon. I immediately pulled over and turned at the next county road. However, I did not realize until this point that my engine power was now being limited to almost nothing. As I crossed a railroad crossing and realized I didn't have engine power, I just hoped that I'd be able to cross and not end up dead on the tracks. Thankfully there was still enough engine power being made available to me to cross the tracks safely and park further down the road. I then turned off the vehicle, called the rental emergency hotline, and was actually able to start the vehicle again at that point without the error. However, the check engine light was still illuminated. I drove the rest of the way home with the check engine light on, but had no other encounter of the message warning me that the vehicle would shut off soon.

Vehicle check engine light came on preventing state inspection, at same time message on dash states "service charging system" . Using the oem charger 120v. Many complaints online say clear the code and power cycle the circuit and charger. Did both and code persists. Battery not charging affecting fuel economy and raising emissions above the stated usage average due to no battery assist. Try clearing codes no luck. OBD code P0d27 is permanent. Try using OEM level Opus access, factory tech says its a known issue, Tried clearing code no luck. This defect could cause vehicle to have no electric propulsion causing stall if gas engine died and lead to a collision. This defect should be recalled and corrected with a module or software update so generic scan tool or automatic monitor resets upon removal and replug of charging cable. Dealer tech says the IDCM is the cause. 30625 miles. This is causing higher than expected emissions, gas costs, and possible accident risk.
