Chrysler (FCA US, LLC) is recalling certain 2020-2023 Pacifica and 2020-2021, 2023 Voyager vehicles. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."

Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.

The second-row seat-to-floor mounting latch may bind and stick open, preventing the seat from properly securing to the floor. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 225, "Child Restraint Anchorage Systems."

The back up camera does not work intermittently. The screen is just black. This was supposed to be resolved as part of recall 66B/NHTSA 24V-436. I had that recall work completed in January 2026 but it did not resolve the issue. The transmission is defective, and according to the warranty, under the lemon law, it has a lifetime warranty, which in my understanding means the entire defective transmission should be replaced. However, I received a call where I was told they would only repair the transmission with a 3-year warranty, and if it breaks down again, I would have to bear the future costs. In my understanding, if I have a warranty on the transmission, they should replace it with a new one. Therefore, I want to make it clear in writing that I do not wish to or authorize any repair of the transmission, but rather the replacement of the transmission as is my right. We were driving on the interstate at 70mph and suddenly the dash displayed a "Stop Safely Vehicle Will Shut Off Soon" and slowed the vehicle suddenly. The check engine light also lit up. There were no indications prior to this that anything was wrong. Dramatic slowing like this is dangerous at highway speeds. We were in the left middle lane and couldn't get to the right because of oncoming traffic, so we went to the far left lane and pulled on a narrow shoulder the left, with four lanes of traffic to our right. It was not a safe situation, and I felt in immediate danger of being hit by oncoming traffic. We tried shutting the vehicle off and powering it back up but the message was still there and the car would only proceed slowly. Feeling like we were in immediate danger, we drove it on the shoulder with hazards on to get it to the next exit so we could pull off. It would not let us get enough speed to safely exit the interstate. We had to cross 4 lanes of 70+mph traffic at a slow pace with the hazards on. Once off the highway, we pulled into the nearest parking lot and tried to find answers. It was Sunday so dealers were closed. After about a half hour the car re-started without the warning on the display. We drove it home on back roads rather than the interstate and took it to a Chrysler dealership the next day, July 7, where they couldn't identify anything wrong with it, and there were no active check engine codes. Reddit and message boards are full of similar situations for Chrysler Pacificas and advised reporting the issue here. I resumed driving it after the dealer and it happened again yesterday (July 14). This time I was close to my home so I drove the vehicle directly there. I'm not using it now and I'm trying to get it in to the dealer again. I checked it this morning before I left for work. It did not have the stop warning but it did have a check engine light on. I don't feel safe driving my children in this vehicle.

On [XXX], I was driving the vehicle on a busy highway with a posted speed limit of 70mph. While operating the vehicle at ~70mph, the front display showed a message that the vehicle was going to stop and to pull off safely. Unknown at what point the vehicle will stop, in a panic, I pulled off on the highway and immediately called road assistance for a tow. After calling the dealership, they told me to start the vehicle again, which I did and it showed the check engine light illuminated on the dash. He advised that the vehicle should be able to be driven to the dealership rather than needing a tow, so I cancelled the roadside assistance. On May 12, 2023, the dealership informed me that they did not find anything wrong and said I could pick the vehicle back up since they couldn't replicate the issue. On my drive home, the exact same issue happened and in a panic I pulled off on the same highway with posted speeds of 70mph. The car was at the dealership from May 12 2023 - June 26 2023. The dealership replaced the valve body and transmission. On [XXX], while driving with two young children [XXX] and [XXX] ) on the same highway with posted speeds of 70mph, I received the same dash message. The highway was very busy that day and the concern was escalated with my young children in the vehicle. I was able to pull off. I turned off my vehicle, turned it back on, which put it in limp mode so I could putter to a safer spot than the middle of an off-ramp. I turned it off once more and restarted it, and it allowed me to drive with the check engine light. 5-6 times, our car has gone into "turtle mode", where a warning comes on telling you to pull over ASAP, and that your maximum speed will be limited. We've been in and out of Gwinnett Chrysler in Stone Mountain, GA, and they've never solved the problem. Since then, it has happened twice more and we made it to an Auto Zone type store each time, and they read the error code. Both times, the code was POA43. On [XXX] while driving my vehicle on the fast lane of the freeway the warning came on that the vehicle was going to shut down soon. I was able to pull over to the slow lane and immediately exit the freeway and as I was exiting the freeway the vehicle completely shut down. I was able to move the car to park in a safe area on the street with what little power it had. Shut the vehicle down and contemplated what to do. I waited for a few minutes and then turned the engine on and it drove but the orange engine light was still on. I slowly drove it to the Carl Burger Dodge dealership in La Mesa, CA using side streets, avoiding the freeway, approximately 10 miles distance. They would not take my car in for service due to their back log and asked for I to make an appointment which would likely be around two to three weeks away or take it to another dealer that may look at my car sooner. The engine stopped responding in a safe manner and we were not able to maintain safe speeds along a highway. While traveling on the highway in a busy, high-speed merge area at night, I received a warning "Stop Safely Vehicle Will Shut Off Soon." I was far from a safe pull off location along a highway with sound barriers on all sides and no way to walk away from the highway. I made my way to the shoulder. Seeing that I was not in a safe location for me and the other car passengers, I drove very slowly with my hazards on until I could exit the highway. There was not a safe shoulder available for much of the distance and the car was unable to go faster than about 30 miles per hour at its peak. This max speed dropped gradually to close to 10 mph. The engine barely responded and I struggled to make a turn onto a residential street without a collision with oncoming traffic. Once were in a safe location, I turned the car off and back on again and the same warning light appeared and the engine was similarly limited to moving the car at about 10 mph max. I have not had the car towed to a dealer yet for inspection.

When driving our van from Nashville to Memphis, we were 2 hours from home when we received an on screen alert stating “Stop Safely - vehicle will shut off soon”. This was in a construction area with little to no shoulders to pull the vehicle over as power loss began to occur. We contacted Chrysler for help but the nearest dealership told us the earliest they could see the vehicle was 3 weeks from Tuesday. I had 3 children in the car ages [XXX}. On 12/20/2024 my wife was driving about 20mph on a road and with no warning lights or messages the vehicle completely shut down. Nothing would work on the vehicle. She could not even put my hazard lights on or put the vehicle in neutral to move out of the roadway. The vehicle blocked traffic for an hour. It was fortunate the car behind did not hit our vehicle and also that my children were not in the car with her. The police assisted in redirecting traffic around the vehicle but they also were unable to get anything working. A tow truck towed to the dealership to assess what's going on. There was absolutely no warnings or previous instances or issues which makes this extremely dangerous. At only 20mph the car still significantly jerked when it came to an abrupt halt.

The next issue is electrical. Our van rear door opens if we are too close to it with the keys and now the sliding door will not open. They just fixed a recall regarding the entertainment system and back up camera. The entertainment system is still having issues. When we start the car up the system will reboot itself.

The ESS battery fails constantly and manufacturer will not find problem and just replaces battery. When the ESS battery fails it drains the main battery which then leaves the driver and its occupants at risk of being stranded. The transmission is an electronic transmission and if the battery fails while driving that could compromise the occupants also. We have had to have the battery replace twice and a third time tomorrow 9/21 if the dealership will "fit" us in. The ESS system will tell you it is unavailable but only after the battery has failed.

On two separate occasions (on April 30 and May 1, 2024), my Chrysler Pacifica's engine suddenly lost power while driving on highways. I was served with an error "Stop Safely. Vehicle Will Shut Off Soon." After a few minutes on the side of the highway, the vehicle was able to start again and drive. After the first incident, I drove to a Chrysler dealership, who plugged in a portable computer to check for error codes. Despite the severity of the issue, the vehicle's onboard computer recorded no errors. Since then, I found that the NHTSA is aware of the problem (NHTSA Action Number RQ24004) related to a wiring harness short circuit. Chrysler's supposed "recall" only entails a feature to supposedly allow time to pull over safely. I enjoyed no such time. I lost power immediately and had to immediately traverse multiple lanes in busy traffic to pull off the road.

After deploying the second-row Stow 'n Go captain's chair seat from its stowed position in the floor, the integrated headrest fails to lock in the upright, passenger-safety position. The headrest remains folded forward or flops forward under normal conditions and cannot be secured in the upright position required for safe occupancy of the seat. The root cause, as identified by dealership technicians and documented by numerous owners across multiple model years (2017–2022), is failure of a small plastic component within the headrest actuator cable assembly. This cable mechanism is designed to automatically fold the headrest during stowing and return it to the locked upright position when the seat is deployed. When the plastic cable end or the locking mechanism fails, the headrest cannot return to or maintain the upright, locked position. The cable itself is not sold separately by Chrysler/Stellantis, with dealers quoting $500–$1,000+ for repair requiring full seat frame or headrest replacement. This constitutes a safety hazard because: (1) a headrest in the forward/folded position provides no rear-impact protection to an occupant, increasing the risk of whiplash or cervical spine injury in a collision; (2) the condition prevents safe installation of child booster seats in the affected seating position; and (3) the defect is a known, widespread, and persistent design flaw affecting multiple model years with no manufacturer recall or remediation issued. This issue has been reported by a large number of owners on owner forums and acknowledged by Chrysler customer care representatives, yet no recall, technical service bulletin fix, or goodwill repair program has been established. I am filing this complaint to support an investigation and potential recall.

2nd Row Seat fails to latch and lock in position. The seat reclines back and forth without being able to be latched in place. My [XXX] and [XXX] 's safety is greatly impacted by this, as the seat is unable to be properly secured in the event of a crash. I have had the other 2nd row seat repaired nearly 2 months ago at a Chrysler dealership, but am dealing with the same problem on the other 2nd row seat. When I took the vehicle to the dealership 2 months ago, the 2nd row seat that has now failed to latch was not experiencing those problems. There were no warning lamps or symptoms of this failure to occur other than knowing that the other 2nd row seat had experienced the same failure 2 months prior to this. The rear headrest will not stay in its proper upright position. After putting it into its Stow N Go position in the floor, the headrest is broken and down and no one can sit in the seat safely. This is a safety issue and Chrysler needs to repair it for free. It is a design flaw. A very thin piece of plastic breaks and results in an unusable rear seat! So many complaints from people who have had this happen. It is not a fluke. It is a regular problem with this otherwise great car! And if someone has an accident with someone trying to sit in that broken seat and gets hurt because there is no headrest………..

Vehicle Information: Make/Model: 2021 Chrysler Pacifica Hybrid Incident Dates: Thursday June 12, 2025 & Saturday June 14, 2025 Description of the Incident: On two separate occasions, the second-row passenger seat of my 2021 Chrysler Pacifica Hybrid collapsed and slid forward without any manual intervention. The first incident occurred when my wife was buckling in our son. The seat unexpectedly folded over him, and it would not latch back into place. To ensure safety, she moved him to the third-row seat. When she stopped at a stop sign, the unlatched seat slid forward and collided with the front passenger seat. If my son had remained in that seat, he could have been seriously injured. A similar incident happened a couple of days later with the second-row seat on the driver’s side. This seat also folded over and slid freely on the floor. Both incidents present a major safety concern, especially since the vehicle is relatively new and was purchased as a certified pre-owned vehicle in excellent condition. The dealership diagnosed the issue as broken seat cables, which prevented the seats from latching properly. While there is no open recall for this specific issue, it closely resembles the conditions in recall Campaign Code Z22, which also addressed seat latch problems in the same make/model and model year. Despite the safety risk, Chrysler has declined to cover the diagnostic costs or the repair under because the basic warranty had expired (although two of their phone reps assured me that there was still some warranty available to help with the expense). I am filing this complaint so that NHTSA can investigate this issue further to prevent potential injuries to other passengers.

For the 3rd time after driving approximately 200 miles, the vehicle has shut down on the highway. I receive a warning instructing me to pull over safely because the vehicle is shutting down. All three of these times happened on interstate highways. Two times this occured on the NJ Turnpike. My husband is disabled and the process of getting off the highway - involving exiting our vehicle and getting into a tow truck is frightening and daunting. I had 3 dogs with me in addition to my handicapped husband. I could not imagine getting out into the lanes of traffic with the three dogs and the disabled husband. I waited 45 minutes and the car started and we were able to proceed 10 miles. At which point we were several miles from the exit off of the turnpike. We were now within 7.5 miles of a dealership. We were instru cted by the vehicle again to pull over safely because the car was shutting down. We waited another 30-45 minutes. Managed to get off the highway. Now within 1.5 miles of the dealership we were told again to pull over. We waited again and made it to the dealership. The codes were the same codes we had on the April 7 breakdown -- the February breakdown also had our car not being able to start. I bought a used Chrysler 2021 Pacifica Hybrid that has broken down for the third time with the same codes P1A9A, P1A9AE, P1A21, and P1AA2, and the low-temperature active pump is suspected to be the issue with battery cooling and heating.

We turned the car off for a few minutes and proceeded to start the engine. The engine warning light was still on, but the other message was gone. We attempted to drive towards our destination and approximately 30 minutes later the same thing happened again. We tried one more time since we were several hours from home and within 30 minutes the vehicle had the same issue. At that point we decided to try to get home. We drove the vehicle 15-20 mins at a time and then exited the highway over and over giving the vehicle time turned off. After returning home with no additional events the engine warning light went off the next warning. This was on a Sunday so we were unable to get it to a dealer. The next day we dropped off the vehicle at Nappletom Chrysler dealership. The called us the next day and told us they could not cause the event to happen again. They could see a set of error codes showing the problem occurred, but told us there was nothing they could do and it was up to us to cause the problem and then return the vehicle. Now I have a minivan with 70k miles on it that my wife primarily uses my with to get my daughter from place to place and I have no idea when this vehicle will shut the engine down at any time potentially causing a dangerous accident. I believe Chrysler has the responsibility to determine what the issue is. Researching on the internet show this is a common problem for my vehicle and typically requires a new transmission or and engine wiring short.

On [XXX], I was driving uphill on the interstate at around 70 mph on gas power (hybrid battery was discharged) and warning came up "Stop Safely Vehicle Will Shut Down Soon" and lost power (Limp mode). I was able to move safely to the shoulder, but with 5 passengers (3 young kids) and a dog and a full load of camping gear it was a bit nervewracking. Could not get the car started again (stayed in limp mode) and used an OBDII scanner I had which popped up error code (P0A43 - "intermittent issue with the drive motor "A" position sensor"). The vehicle was towed to the dealer ( 4 hours from home) and after a few days they said the vehicle was fixed and I pushed for them to test drive it on the interstate. Was returned with this comment: " FOUND CODES FOR DRIVE MOTOR "A" POSITION SENSOR CIRCUIT INTERMITTENT AND HYBRID BATTERY PACK STATE OF CHARGE LOW. CHECKED TRANS PLUG AND WIRING VERIFIED OPERATING CORRECTLY. FOUND THAT RESOLVER NEEDS TO HAVE RELEARN PREFORMED. PREFORMED RELEARN ON RESOLVER AND CHARGED HYBRID BATTERY. TEST DROVE VEHICLE ON INTERSTATE AND VERIFIED THAT ISSUE DID NOT RETURN. VEHICLE OPERATING AS DESIGNED." I received a notice of an unlimited mile/year warranty on the transmission for this car, and from what I read on the internet, this is a recurrent issue with a known cause. We were able to get off the road safely in this case, but it is nerve-wracking to drive, especially with my family, in the current state. My wife was driving driving the vehicle on October 21st, 2024. She was shifting from Park to Drive just after our children got out of the vehicle in the school parking lot when the whole vehicle froze up and the dashboard lite up with all error lights including the engine light. The tow truck had to drag our Pacifica onto the flatbed while it continued to turn on and off and cycle through the engine and hybrid system. This vehicle has 36,000 miles and just had the transmission replaced under the "Recall for hybrid transmission systems" This is the second time this has happened, it originally occurred at 7000 miles and after the dealer had it for a week we were told there was no problem. The dealer had to pull a bunch of fuses and the 12v battery to get the vehicle to stop turning off and on, the same thing happened this time when it was dragged off the flatbed at the dealer.

I began having trouble with the electric/gas drivetrain around January 30 of 2023. The vehicle lost all power, as well as throttle and proper braking response. I brought my vehicle back to Helfman Dodge Chrysler Jeep RAM FIAT at 7720 Katy Fwy A, Houston, TX 77024 for repairs on February 6, 2023. The repairs performed seemed to have worked until [XXX]. The most recent event on [XXX] my Pacifica Hybrid lost all power on the highway. While attempting to pull the vehicle off of the highway it abruptly entered its parking gear at 5mph, almost causing a rear end collision from the semi tractor trailer following behind us. Without any prior warning or predisposing symptoms, my vehicle lost all power when I was driving in the carpool lane at 70 miles per hour. I had to "coast" across 5 lanes of freeway and come to stop in the emergency lane. When I applied the brakes I had no power to brakes which made the car skid violently. I could have lost control because I was not expecting that secondary issue. The traction conrtol may have been triggered causing the car to overreact. The dashboard lights and "d" on my gear shifter were all flashing. I turned the car off and allowed it to sit for a few moments. I turned the car on again and nothing happened. I turned it off a second time and it turned on, went into drive and I was able to drive the remaining 5 miles home without incicent. I have already completed the recall notice for this very issue back in Jan 2023. However, even though the dealer states they completed my recall work the upbove described incident happened on 1/14/24 nearly a year after their recall completion.

High pressure fuel leak with large fan spray pattern in fuel line part number 68492134AA where it sits and rubs against a sharp corner of another component, (appears to be ignition coil) above the rear passenger side of the engine. The cause appears to be how this line is routed by the factory. it is resting against the coil and not secured in a way to prevent chafing. Result is extremely dangerous with fuel spraying down towards the exhaust. The part is available for inspection. Safety of my family was put at risk by fuel spraying towards hot exhaust while my wife was driving. She was unaware of problem until fuel smell just prior to arriving at home. Problem is easily reproduced. I spoke with Olympia Chrysler Jeep and the service advisor Corey, said he had seen this issue before as well. He also said because car is not under warranty and I don't have roadside assistance, towing would be at my expense. I have ordered replacement part and will install and secure it to prevent chafing. I will keep part to be availalbe for examination. There were no warning lamps, messages or other symptoms prior to the fuel smell.

While driving the vehicle, the accelerator will become inactive and result in the vehicle slowing to a stop. I will shut off the ignition and let vehicle rest for 10-20 mins before restarting. This does not resolve the issue and I end up towing the vehicle into the manufacturer’s service department. This often results in 100s of dollars in “diagnostic” fees. The service dept claims it’s bad battery, changes and issue arises again. Next they say it needed a software update, and a few days later it fails again. Next they said it was PCM module and replaced it, and finally they replaced the transmission. The 4th visit, change of transmission, lasted a little over a year and then the issue arose once again! After paying $259 in diagnostic fees, they said I used a bad “charger”

While driving my children home from an evening activity, there was a strong odor of gas inside the cabin. I had the vehicle towed to the dealership the next morning for diagnostics and repairs. The dealership 's technician was able to reproduce the issue and upon further inspection discovered a small leak in the fuel line. It was determined that the fuel line had been rubbing against the coil pack, which created a small hole. The fuel was leaking onto the coil pack. This puts both the driver and the passengers at risk because the coil pack is in close proximity to the spark plugs. When there is a spark and gasoline, fires and/or explosions tend to be the result. The technician stated that there had been no tampering with the components and that the gap between the fuel line and the coil pack is very small. I believe that the gap is too small because normal operation of a vehicle should never cause a hole to be rubbed into a fuel line due to it's proximity to the coil pack. Van had a smell of gasoline inside the cabin and out. Upon inspection, the fuel line going from the hard line to the nylon line in the engine bay was leaking. The nylon line between the fuel pressure sensor and the hard line had rubbed against a coil pack and was leaking fuel. Any fuel leak, especially in the engine bay, poses a safety risk as it can combust and cause a fire inside the engine bay. Other owners have posted about this exact problem on forums, hence why I knew where to look for the issue. I have ordered the new fuel line through the dealership, however, if the fuel line has the same bend in it as the original, the same thing will happen in the future.

Van has 70K miles. After a long road trip. last stop noticed a gas smell from the front of van. When opening the hood noticed gas had been leaking over top of the motor by spraying fuel. Moving the fuel line running over the top of the motor, showed it was rubbing against top of the coil pack on engine. The rubbing over time caused a small cut in the fuel line allowing for fuel leak. The fuel line seam to be malformed or is unsported causing the rubbing.

High pressure fuel line leaking due to tube rubbing against hard plastic clip. Smelled gasoline fumes from outside the vehicle. Narrowed it down to the passenger side of the engine compartment but couldn't isolate it. Read some forums from similar cases and identified a high pressure fuel line that could wear a hole due to a design flaw. Sure enough I wigged it just slightly and upon starting the vehicle it started spraying gasoline all over the engine at an alarming rate. I drove my children to daycare the previous day, this could have started a fire or caused the engine area to explode. Crazy unsafe and glad I caught it quickly.

When driving at speeds over 45 mph and making turns (eg on high ways) the van will sway and rock. I have installed new shocks, had an alignment, had ball joints and sway bars inspected, and had tires restated. I’m wondering if there is some sort of an issue with the electronic assist power steering that senses g forces and compensates inappropriately. It is extremely dangerous and we have nearly lost control of the car while doing a routine turn. It happens when turning left, but not when turning right.

More often than not, the steering wheel column gets stuck at 10-11 o’clock positions and 1-2 o’clock positions. So it typically happens when making slight bends on my local roads and highways. This requires a hard jolt on the steering wheel to get it un-stuck. This is extremely unsafe to do at high rates of speed and/or with traffic on either side of me and it feels only a matter of time before it results in an accident. When I brought the car into the dealer for a different repair, they noted in their follow-up inspection a, “rep clunk from steering gear the wheel both ways” and quoted me $4800 for repair. I’m assuming that’s to replace the entire steering column, which is pretty outrageous considering the part is less than $900 OEM and would be about 2-3 hours of labor. I purchased the car 6 months ago as a certified pre-owned car from a dealership and the CarFax report came back completely clear, recall repairs weren’t even mentioned on it. I’ve seen some discussions on online forums describing a similar issue.

My 2021 Chrysler Pacifica Hybrid has a recurring hybrid/electrical defect that causes the accelerator to lose all power while driving, leaving the vehicle unable to accelerate or respond to throttle input. The vehicle remains “on,” but it cannot move under its own power and must be coasted or pulled over immediately. This has happened in traffic, including with children in the vehicle. The dealer confirmed fault code P1A21‑00 and documented that the high‑voltage system can lose its loop connection, which results in a no‑propulsion condition. They also found the HVIL high‑voltage connector loose and damaged. The vehicle was in the shop for 18 days for this issue. I picked it up on June 18, 2026, and within 24 hours the same loss‑of‑propulsion failure happened again. The vehicle is now back at the dealer. The dealer performed recall 52D and updated the BPCM software, but the failure still occurred immediately after pickup. This defect has been reproduced and confirmed by the dealership multiple times and remains unresolved. This is a serious safety hazard because the vehicle loses propulsion without warning and cannot accelerate in traffic.

While driving 70MPH on the expressway the dash randomly sent a message saying “Pull over vehicle shutting off” and then it put itself into limp mode. Thankfully we were able to coast to the small shoulder and put on our hazards. This happened at 2am with 4 kids in the car. Nothing in the car would work, it wouldn’t turn on, shift, nothing. We had to sit on the side of the expressway for hours waiting for a tow. While waiting it had the check engine light on, limp mode light, and messages to service hybrid system & plug in battery for reconditioning. It is waiting to be serviced, but there is a very limited amount of techs that will service it due to being a hybrid.

I was traveling at highway speed. The hybrid was utilizing mostly the battery. I needed to accelerate to the speed of those around me. The van switched from just battery to utilizing battery and gas power. About a minute after it switched, the engine completely shut off. A warning flashed up on the dash display to “Stop safely Vehicle will shut off soon”. The engine shut off. I could no longer use the gas, the only thing I could do was coast to a stop. This was very terrifying because I was traveling at such a high speed on the interstate. I immediately put on my hazards and coasted to a stop. This was harmful to others on the interstate because they had to slow down behind me or get around me. Eventually when I was slow enough, I managed to coast onto the shoulder and come to a complete stop. I was able to reproduce the problem upon restarting the vehicle. I have taken the van to a dealership but they have not been able to look into it yet. The engine light is now on in the vehicle. This happens consistently at high speeds

Reversing onto roadway and shifted from reverse to drive via rotary dial and vehicle went into park and would not move causing me to block the roadway. Traveling down [XXX] in Escondido at approximately 73mph. In the fast lane and moving left into the car pool lane the vehicle lost all motive power and the error "Service Charging System" came up on the dash then flashing back to "SHIFT TO P TO START". We were able to make it to a very small emergency lane on the left side of the interstate. Had to power vehicle off and restart twice before be regained motive power. The passenger sliding door on my 2021 Chrysler Pacifica is stuck half way open and will not move. The vehicle was parked and we opened the automatic door and it opened fine. I pushed the close button and it closed half way and stopped. The cables that connect the door to the open/close motor are hanging out. I called the Chrysler dealership and they tried to schedule me an appointment next week. I asked how to get it to the dealership and they said to drive it in. I said the door is stuck half way open and its not safe. They said to either drive it in or tow it in. I asked if there was a recall or an extended warranty. They said there was not and that I would have to pay for all repairs out of pocket. The motor actuates but since the cables don't move it faults out. I have pushed and pulled as hard as I can and the cables will not move. This puts all passengers in danger as the door is part of the structural integrity especially in a side impact collision.

There is no safety feature on the automatic sliding doors that prevent them from fully closing on something or someone. Once they begin closing they will not reverse if they encounter an obstacle. Given the nature of the vehicle being primarily for family use with children as well as children being the passengers that would use this entry, not having a safety measure to prevent them from being crushed is unacceptable. My 3 year old had accidently hit the close button and the door kept pushing them forward toward the latch and even with considerable force pulling it the other way when I noticed and realized it also wasn't stopping or reversing to open, I was lucky to be there. Then again recently, my wife was in the car cleaning out some items where she had reached in and she accidently bumped the same button, however, the door kept closing on and pressing into her. I thankfully was on the other side and ran over and threw all my weight backwards for it to stop, again, not reversing. It was something we noticed early on, but figured with the electronic failure they continue failing to resolve it was related and would be either solved or the fault become a recall. However, after it happening again recently and without any safety recall, I'm now reporting.

I encountered two incidents of the same type. One was on Friday, March 27, 2026, the same day I picked up the vehicle from a local Chrysler dealership, where software updates and issues related to recalls were supposedly completed. The vehicle was at 38% battery level at the time of pick up from the dealership parking lot. After driving it for about 14 miles, the battery level depleted to <1% and that's when the "Stop Safety. Vehicle will shut off soon" displayed on the dashboard. I slowly drove for less than a minute and parked the vehicle and turned it off. Then restarted and drove for less than two minutes, but again the same warning ("Stop Safety. Vehicle will shut off soon") displayed; I turned it off again. After taking a few minutes break, I restarted it and it was showing 1% battery; it gave me a little distance to travel until unfortunately the same warning displays. Fortunately, this time I was close to a place where I can charge the battery. The battery got charged to 100%, partly on the night of March 27th and the remaining in the morning on March 28th. The second incident was on March 29, 2026, on Sunday afternoon. As soon as the battery level reached <1%, the exact the same warning ("Stop Safety. Vehicle will shut off soon") was displayed. I slowly drove the vehicle to the side of the road and called my insurance company; I reported the issue and requested them to tow the vehicle to a nearby Chrysler dealership. As of now, the vehicle is at the dealership parking waiting for diagnosis. Based on my initial conversation with the customer service person, they don't know if this is considered part of any recall or not. Not only was I stranded on the road because of the above incidents, not having the vehicle at the moment is straining me a lot - trying to handle personal and work schedules.

The contact owned a 2021 Chrysler Pacifica. The contact stated that occasionally, while driving at various speeds, upon depressing the brake pedal, the vehicle would surge forward 3 to 4 feet before proceeding to slow down or come to a complete stop. No warning lights were illuminated. The vehicle was taken to a local dealer, where it was diagnosed, and the contact was advised that the failure was not duplicatable and nothing was found to be wrong with the vehicle. The local dealer proceeded to replace the spark plugs, the brake fluid, field spark plug weld, and gasket cylinder head cover, however the contact stated that after the repair the failure increasingly persisted leading them to be involved in a crash where the contact stated that while driving on a highway at 55 MPH, there were two vehicles stopped ahead, the contact depressed the brake pedal and the vehicle proceeded to surge forward and crash into the vehicle Infront of them with the entire front end of their vehicle. The impact caused the vehicle in front of the contact to crash into the rear end of the vehicle in front. That vehicle sustained damage to the entire rear and front end of the vehicle as the impact caused that vehicle to crash into the rear end of the vehicle in front of it. No property damage was reported. The vehicles came to a stop upon rebound after the impact. The air bags were deployed. The vehicle was towed to an auto body center, and no warning lights were illuminated. A police report was filed. The vehicle was destroyed. The contact was injured while seated in the driver's seat; the passenger in their vehicle was not injured. An individual in the vehicle in front of the contact was injured. The contact sustained injuries to their ribs and sternum. Medical treatment was needed for the injuries, and the other individual also required medical treatment for unknown injuries. The individual at the front of the line in this collision was not injured. The local dealer was contacted however, the vehicle was not diagnosed as it was declared a total loss. The approximate failure mileage was 82,500.

The vehicle was towed to a tow yard and deemed a total loss by the contact's insurance provider. A police report was filed, and injuries were reported. The contact's daughter was transported to an area hospital by EMT and treated for nondescript injuries to her neck and shoulders. The manufacturer was notified of the failure, and a case was opened. Since week 1 of owning my brand new 2021 Chrysler Pacifica Hybrid, I have had issues of loss of motive power. I have done all of the recall remedies yet my car with a 35day old 12v battery not only died on me, but drained every battery in my vehicle so that I couldnt open my doors. The vehicle was unable to be jumped so that it could be driven, so I ended up paying o or $1000 in towing to get the vehicle to my dealership to be diagnosed,AGAIN. That happened on July 12th, I finally had it towed on July 14th to my dealership & it has been there since. Although the PIM & BATTERY are under WARRANTY, Chrysler is refusing to reimburse me any of the tow fees & only 20 days of a rental I have now had for a month. The dealership installed the PIM which cleared a couple of codes, but not all. They installed ANOTHER new 12v battery, which was immediately drained. They have ordered another coolant something or other for the battery & here we are. I attempted in 2022 to retain a lemon law lawyer but he gave up very easily once Chrysler just stopped answering him.he said because I had my car updated under the class action lawsuit Moran is Chrysler FCA et Al, I was not able to continue under Lemon Law. I have not been able to find a lawyer, even one from the Class Action Lawsuit, to assist me in fighting Chrysler to do something to make me whole. My vehicle needs to be inspected in less than 40 days but at this point my 5 yr old vehicle can NOT pass the Pa inspection, so what am I supposed to do? I live in the mountains, have an 8 yrold grandchild that im raising & certainly cant afford to rent cars forever. I still haven't gotten my pension from the Coty of Philadelphia after 3 years of retiring after getting hurt on the job. I am having a problem with my under-warranty (32,000 miles) 2021 Chrysler Pacifica which the service department at my dealership appears unable to help. I was driving on an interstate in stop-and-go traffic for a couple of hours. It was a very hot day, temperatures well above 90 degrees. Six times over 15 minutes the following occurred: While coasting/braking, so at low RPM the engine turned itself off I braked to stop the vehicle The vehicle put itself into Park A message on the dash said to place my foot on the brake pedal and press the button to start the engine I did so, and the engine cranked for an unusually long time, becoming longer each of the 6 times, before eventually starting By the last time we were doubtful it would start at all After the 6th time I cut my long-distance trip short, turned around, and came home in normal (not stop-and-go) highway traffic conditions, with no more engine failure. The check engine light did not turn on, and no service codes were generated which could have been related to these power failures My local dealer's service department (Pohanka in Salisbury, MD) tested the alternator, starter, and battery and found nothing wrong. They did charge the battery up a bit, saying it took only 20 minutes. They have been unable to re-create the problem, which is not surprising, due to the unique conditions under which it happened. They say there is nothing they can do about my problem under warranty because there is no code and they can't reproduce the problem, so they want to give me back a vehicle that we know behaved unsafely six times under dangerous conditions. Engines failing without warning on interstates are likely to result in being rear-ended. I see online that the hybrid model of the same model year has a recall for exactly this same issue, so it is not a new issue for Chrysler, although maybe new-ish for my non-hybrid model of the same vehicle.

There are clearly many design flaws on my 2021 Chrysler Pacifica Hybrid. One issue I am having with this car is the Daytime Running Lights. These are expensive LED lights that should function for the life of the vehicle. Unfortunately, I believe there is a problem with the control module on the LED lights, and part of the driver's side light does not function. This is a clear safety issue, and seems to be very common problem people are having with the 2021 Chrysler Pacifica Hybrid. I notified Chrysler about the issue, and because the car was just over 36 months old, they would not repair the issue. They told me I would have to pay for a complete replacement of the headlight which would cost over $1000. Just today, I noticed Ford posted a recall for the same issue they are having with their Daytime Running Lights. I believe it is only fair for Chrylser to provide a voluntary recall on this same issue because of the clear safety hazard it can creates. What is the purpose of Daytime Running Lights if they don't function properly. This is a minivan I rely on to safely transport my family, and I do not want to risk a hazardous collision because Chrysler refuses to repair a faulty part in their vehicle.

This is a follow up to an earlier submission (NHTSA Tracking Number 11591550). After the initial incident, the dealership replaced the hybrid battery on the vehicle. A month later, yesterday, while driving the vehicle on the interstate, I again received the dreaded, "Stop Safely, Vehicle Will Shut Off Soon" warning indicator and the car lost all power about 30 seconds later. Thankfully, I was on an offramp and was able to stop safely. Had I been traveling at full speed in the carpool lane, it could have been very dangerous. The van is now at the dealership for repair, and I hope they fix it for real this time. This is an extremely dangerous situation, with vehicles dying while going full speed.

While driving at full speed on an interstate, I received the message, "Stop Safely, Vehicle Will Shut Off Soon." The van then lost power, and I was able to pull over safely (thankfully!). After about 15 minutes, the van restarted, and I made it about 10 miles until it happened again. After two more shut-downs, I was able to get the van to a dealership, where it has been sitting undiagnosed for over a week now.

On June 23, 2024 a NHTSA recall #24V-436 was posted indicating the rear visibility system defaults to the rearview image being visible. To date multiple dealerships and the Chrysler Corporation contacted trying to get this issue remedied. No one seems to have an answer ass to when the update will occur. I contract with a car rental service, TURO, to rent my personal vehicles. Because of this recall TURO will not allow my vehicle to be rented until this issue is resolved. I have to continue making car payments and pay insurance on this vehicle without any income coming in to offset my costs.

The windshield wipers are not synced together causing them to contact each other to the point that the stuck together. As a result, the wipers are no longer functional, and the drivers vision is impaired which increases the likelihood of an accident.

While driving at normal road speed, the vehicle suddenly lost propulsion and entered limp mode. A turtle warning icon appeared along with multiple system fault messages. The vehicle began slowing down on it's own and was unable to accelerate. I pulled to the side of the road. The vehicle displayed an error "Stop Safely Vehicle Will Shut Off Soon". After sitting for a period of time, the vehicle restarted and was able to be driven again, but the failure occurred without warning and created a dangerous situation in traffic. Diagnostic trouble codes were present related to the hybrid system and high-voltage battery, including P1D43, P1A21, U029A, and U0412. These codes indicate faults in the hybrid control system and battery communication. A Chrysler service department inspected the vehicle, confirmed the trouble codes, and returned it to me without repair because the issue was not immediately reproducible at the dealership. The vehicle has a history of similar intermittent issues, including prior loss-of-power events and repeated service visits for hybrid system faults. The condition is not consistently reproducible, but when it occurs it results in a sudden loss of motive power while driving. This creates a significant safety risk due to unexpected loss of propulsion in active traffic.

This is an intermittent issue that has been happening for a while now and the car was sent to the dealership twice but the issue still persists. The engine would shuts down or partially loses torque all of a sudden and the steering becomes very stiff. At the same time you would see all kinds of alarms, error messages, and check lights on the dashboard. I would then have to pull over to the side of the road, turn off the car, then try to start it back up. Usually after several attempts (one time that took nearly 30 minutes), the car works properly again with no error messages but sometimes I'd still have some persistent check lights on the dashboard. Error messages and check lights that I see include the following: -Service Electronic Stability Control -Autopark Engaged -Shifter Autocorrect Active -Park Brake Temporarily Unavailable -AWD System Temporarily Unavailable -Automatic Emergency Braking off check light -Forward Collision Warning off check light -Check Engine light The dealership has been unable to properly diagnose and fix this issue so far. On the first visit, they replaced the main battery, auxiliary battery, some wire harnesses that I was told had some corrosion, and the adaptive cruise control module. On the second visit, they replaced the CAN Bus star connector. The same issue still persists after replacing all above components.

The contact stated that while driving approximately 30 - 35 MPH, there was an abnormal rattling sound coming from the wheel well or near the front underbody near the air dam, as if there was a piece of ice stuck in the air dam. The vehicle was taken to the dealer, who diagnosed that the front wheel hub hardware was loose, affecting the wheel bearings, and causing the failure. The contact was informed that the wheel hub hardware and front wheel bearings needed to be replaced, and the front wheels needed to be rotated. The vehicle was not repaired. The manufacturer was informed of the failure, and a case was opened. The failure mileage was approximately 20,000.

When adaptive cruise control is enabled and I’m at cruising speed, new vehicles entering the lane in front of my vehicle are not being acknowledged leading to my vehicle getting too close and collision becoming likely. This leads to me having to takeover control frequently to prevent accidents. This happens often, not always, but it is becoming more and more frequent. This happens at all speeds from 30 mph to 70mph

The side back doors do not work all of a sudden when you push the button on the vehicle frame inside of the van. This is on both sides of the van. They will only shut by pushing buttons on the outside door handles. Also, the windshield wipers will only work on high. There is no intermittent, low, or medium speed working.

1. This vehicle was recalled for a software update due to the rear camera failing to operate at times. The software was updated by the dealer. However, this did not completely fix the problem. While the camera fails less frequently, it still does not energize at times. I took the vehicle back to the dealer. I was then informed that there is water intrusion into the rear lift gate, which causes the electrical to short out. I was told that I would have to replace the wiring harness which energizes the camera as well as the rear vehicle lights. I have not noticed any problem with the lighting as of yet. However, I have been told that this is a problem with the Pacifica in general, as well as the Durango. I was told that replacing the harness would not guarantee that the problem would be solved as there would still be water intrusion, which could cause the problem again, even with replacing the harness. This is obviously a safety issue, as the camera fails to operate at times, and there is a potential of losing lighting on the back of the vehicle. This is also a known defect by Chrysler. 2. When using the Adaptive Cruise Control on streets and highways with multiple lanes traveling the same direction, the vehicle will brake suddenly and hard when the vehicle in front of me changes lanes. This occurs after the vehicle in front of me has completely changed lanes and is not impeding my forward progress whatsoever. My vehicle speeds up to the set speed and then violently brakes as if it is about to crash into the vehicle ahead of me, even thought the lane is completely clear. The vehicle has been inspected for this scenario. I was told that it didn't perform as described above when the technician drove it. This is a safety issue as the vehicle suddenly and violently brakes at highway speeds, with traffic behind me.

The contact's sister-in-law owns a 2021 Chrysler Pacifica. While the contact was stopped at a mailbox to retrieve mail, the contact opened the front, driver-side door. When the contact opened the door, the vehicle's parking brake engaged independently. As the contact attempted to drive forward, the vehicle's rear wheels had locked upon the depression of the accelerator pedal. Due to the failure, the vehicle was stopped in the middle of an intersection as the parking brake failed to disengage. The parking brake warning light appeared on the instrument panel. The contact made multiple attempts to restart the vehicle but to no avail. The contact then stated that a "Service Parking Brake" warning message appeared on the instrument. All occupants exited the vehicle and directed traffic to avoid hitting the vehicle. The vehicle was then towed to a dealer where they managed to restart the vehicle without incident. The manufacturer had yet to be notified of the failure. The vehicle was not repaired. The failure mileage was unknown.

Dear Sir, I am submitting this formal complaint regarding severe misconduct, fraud, and reckless endangerment committed by Hyundai Escondido, Hyundai Motor America, and Ally Financial in connection with the sale and financing of a dangerously defective 2021 Chrysler Pacifica Hybrid Touring AWD (VIN: [XXX] ). 1. Background of the Case On [XXX] I purchased the above-referenced vehicle from Hyundai Escondido under the representation that it was a Hyundai Certified Pre-Owned (CPO) vehicle that had passed rigorous manufacturer standards. The dealership provided me with a Carfax report showing no prior accidents or major repairs and falsely assured me that the car was in excellent condition. However, immediately after purchase, I discovered that the vehicle had severe hidden defects, including: •Extensive flood damage, which was concealed at the time of sale and significantly compromised the car’s electrical and safety systems. •Prior frame and structural damage from an accident that was never disclosed. •Critical electrical malfunctions causing the vehicle to swerve uncontrollably on its own, without any driver input. •Severe safety risks, which nearly resulted in an accident with another vehicle carrying a baby on board. •Substantial mechanical and structural issues requiring over $20,000 in repairs, making the car unsafe and unusable. Despite reporting these issues on the first day of ownership, Hyundai Escondido refused to acknowledge or rectify the matter. 2. Hyundai Escondido’s Role in Fraud and Misrepresentation •Knowingly sold a flood-damaged and structurally compromised vehicle without disclosure. •Misrepresented the vehicle as a Certified Pre owned. WE'VE SEEN MANY GROUP MESSAGES ABOUT THIS ISSUE AND THE COMMON THREAD IS THAT CHRYSLER DOES NOT HAVE A FIX OR KNOW WHAT TO FIX AND THAT THIS ISSUE WILL GENERALLY OCCUR AGAIN. My car has the 03A recall, and had the software update in March 2024. In Summer 2024, I got the red turtle, "Stop, pull over. your car is going to stop" warning, my car stopped moving, etc. I was able to get it restarted and CEL came on and has been on ever since. I took it in for the open recalls (66B), and state inspection, as well as the liftgate recall that was performed by the dealership but the liftgate has never worked since then. They stated that all the recalls except 66B were completed and when I asked why I had gotten the STOP NOW message after the software "fix" was done, they had no answer. I provided them with research stating that Chrysler had issued an unlimited lifetime warranty for these transmissions, since that is where the short is located that is causing the cars to stop. They have insisted since then that the transmission isn't the problem; that my hybrid battery is the problem. I have had no issues that would indicate the battery is the problem, but plenty to indicate the transmission is the problem. My car's been in the shop since March 17, and it is April 27. The new battery was supposed to be in on April 10, but there is still no ETA. I have a STAR case and a case open with Customer Cares, who has been absolutely no help in getting this problem actually fixed. Despite my providing the dealership and Customer Cares the necessary information stating that the new transmission is the only repair that will stop the car from stopping randomly, endangering myself and my family and other people on the roads, they continue to encourage me to "trust the engineers" and let them replace the battery. They are REFUSING to fix the issue, and I will not drive that car until it's properly fixed. This transmission issue is creating death traps and there is a known fix that even Chrysler has acknowledged by issuing lifetime warranties on the transmissions affected by this recall, yet the dealership and Chrysler Cares refuses to perform the fix for it.

I was backing out of the driveway. I had all the safety features on, and I was checking my surroundings. Typically, the blind spot and front sensors would alert me to dangers. However, on this and many other occasions the warnings didn't alert me to danger on my passenger side that I couldn't see. this resulted in damage to the front quarter panel of my van as well as damage to the bumper. The body shop said it would cost at least $5000 to repair the damage. I can't afford that; in turn I am driving around with the damage.

Second incident of loss of motive power at 37352 miles on the odometer. Vehicle was traveling on freeway and suddenly displayed message, "Stop Safely Vehicle Will Shut Off Soon." Accelerator pedal had no effect and vehicle began coasting. It was dangerous to coast across multiple lanes of freeway to reach an off-ramp with shoulder wide enough to stop the vehicle. We also had to make it over an incline while coasting and it may not have been possible to reach the shoulder or off-ramp. Prior to this incidence, the vehicle had been brought to a Chrysler dealership due to a previous occurrence at 36997 miles on the odometer. It was inspected by the Chrysler dealership and deemed to be operating normally.

Ghost braking. While driving on the road using the adaptive cruise control, the van initiated its braking system as if someone was in front of me. This happened with nothing in front of me for miles in sunny cool weather and while at night passing a car with fog. I assumed the fog case had some bearing as the car I was passing their tail lights appeared to illuminate the fog to a degree you would think was a car in front of me. Glove box needed to be replaced. Kept opening and slamming on my bride's chins. The trunk is soaking up water from an unknown source after heavy rains and car washes. We noticed the issue when we vacuumed the rear trunk space and took out the mat. We dried it at the local car wash and didn't think much of it until we went through a car wash again. And there was the leakage but unknown from where.

The forward collision alarm was triggered when there was no collision. Additionally the ABS system and cruise control went offline. This occurred numerous times during winter while at a full stop as well as when driving between 45-55mph. Once triggered the collision alarm remained on for hours to days at a time. During that time cruise control was not able to be used and ABS was offline. Additionally the assisted parking was not available. ABS and stability control is important for safe driving and braking in icy/slippery situations putting both my own, my passengers, and other drivers at risk. The vehicle was inspected 4 times by the Bath NY Chrysler dealership. A sensor was replaced the first time but this did not correct the issue. The 2nd time another component was replaced, but the technician stated the issue was still present and I would need to bring it back in for further testing. The 3rd and 4th time the dealership was not able to recreate the issue and did not perform any repairs. The issue has recurred, but the dealership was not able to get the car in at that time. This vehicle has not been inspected by manufacturer, police, insurance etc. There was no prior warning to the issue. When it occurs the warning lamps that come on are: BRAKE, Front Collision, Front Collision OFF, ABS, Seatbelt, and the Traction Control. I have a photo of the dash from one of these incidents.

As I was leaving my son's school parking lot - moments away from an on-ramp to a major freeway - an error message flashed on the dash saying "Service Charging System" followed by a message about the vehicle's speed being limited and a red turtle icon. The van instantly was driving strangely. Since I was in a parking lot, I turned everything off and waited for the van to shut down completely. I restarted the van and the warning messages were gone, but the check engine light remained. The van showed more than 15 miles of battery mileage remaining. The week prior, we had taken it into the dealership for the open recalls to be repaired, so we took it back for this issue. The dealership just messaged me saying the van is ready to be picked up and that the charging system was just low. But it still had nearly 1/2 the battery left when this happened! The dealership went on to say, "So here is the deal, we could not verify the concern at this time. There are no codes in the system, nothing to really test for. I can have someone drive it and see if anything happens, but without being able to duplicate your concern, we cannot diagnose it." My sister-in-law had the exact same issue on her own Chrysler pacific Hybrid just a few months ago, while she was on the freeway and her speed was instantly throttled. They replaced the entire electrical hybrid transmission. This is a known issue with this vehicle and it is going to put my life and my kid's life at risk since the dealership cannot reproduce the error. I even sent a picture of the error message, and they say there's nothing they can do.

The vehicle stalled on the freeway while traveling 75 mph in the HOV lane. After the engine shut off, I recalled an error message on the dash that stated, "Motive power will stop and pull over quickly.” I needed to take evasive action to bring the vehicle to the side of the 5 freeway. I could not restart the engine in Neutral while the car was in motion. The car showed a charging error on the first and second attempted restart on the side of the road with the transmission in Park. After letting the vehicle sit for a few minutes, I successfully restarted the engine, but the dash continued showing an illuminated malfunction indicator lamp (MIL). We were traveling at 75 mph in the HOV lane (lane #1) when the vehicle engine cut off without warning. Once the engine was off, I saw a warning on the instrument cluster that the vehicle would lose all motive power (it's a hybrid) and that I should immediately stop it. I needed to take emergency action, cross over 5 lanes of freeway traffic, and then stop on the side of the road. The vehicle did not allow an engine to restart while in Neutral, with the car moving. The vehicle was brought in for the recall of 03A/NHTSA 23V-010. From what I understand, this recall did not fix the problem (reported to be a faulty wiring harness in the transmission). I installed a software update to display the error message that instructed me that all motive power would be lost and that I should stop the vehicle.

On empty roads at low speeds, vehicle flashed emergency braking signs and stopped abruptly. Happened multiple times in January both in reverse and when driving forward normally. Safety of driver and passengers was impacted by the sensor issues and abrupt braking. If there were other cars behind the vehicle, it could cause accidents and injury. Vehicle was brought into dealership and could not identify or address the issue. Suggestion provided was to disable the sensors while driving.
