OnStar Module failure. This did not trigger any type of notification. I had to make several phone calls to OnStar to determine I would need to schedule a service appointment for a technician to look at the vehicle. After inspection it was noted that the OnStar Module is not working and will need to be replaced. Upon ordering the part I was informed it is on backorder with no delivery time noted. If I were to get into a crash OnStar would not have triggered a call to first responders for my safety - which is a service that I pay for. No warning lights appeared and the the OnStar button still shows "green" as in active service.

After hitting a deer, the body shop fixed the damage to the front part of the vehicle. They realized the battery kept being drained even after purchasing a new one. It’s now at a dealership and they are staying it needs a new ONSTAR module, not sure when it will come in either. I’m paying for a rent car that I can’t afford to pay for anymore. I am submitting this letter to formally notify the NHTSA of a safety critical defect in my 2025 Chevrolet Equinox Activ starting on 12/9/25 involving the Telematics Control Module (TCM), GM part number 85669062. This defect results in intermittent or complete failure of essential OnStar safety functions, including Automatic Crash Response, emergency communication, and accurate GPS location reporting. During normal driving, the vehicle experiences the following failures: •Loss of telematics connectivity •Inability of OnStar services to initialize or maintain a connection •GPS location drift or inaccurate positioning •Failure to reliably transmit vehicle location or contact emergency services These failures occur without user action and under normal operating conditions. The defect directly impairs systems intended to provide crash detection and emergency communication. If a collision occurs, the vehicle may not contact emergency services or may transmit an incorrect location. This represents a loss of a safety system that General Motors advertises as part of the vehicle’s crash response capability. On December 9, 2025, my Chevrolet dealer confirmed that the Telematics Control Module had failed and requires replacement. General Motors has placed this part on national backorder with no estimated availability, leaving the vehicle in an unsafe condition with no available remedy. General Motors has not issued a recall, service bulletin, or customer satisfaction program addressing this defect. The absence of available replacement parts suggests a potential supplier level issue affecting multiple vehicles. Given the safety implications, part unavailability, and potential for a broader pattern of failures, I respectfully request that NHTSA: •Review this defect as a potential systemic issue •Evaluate whether similar failures are occurring in other GM vehicles •Consider opening a Preliminary Evaluation (PE) if warranted. Telematics unit in my Chevrolet equinox EV 2025 has failed. GPS/navigation/wifi does not work anymore. Calls to onStar for collision data / support in emergencies also do not work anymore. Also, basic features on the myChevrolet app to unlock/lock the car do not work. This happened gradually over time. Around 6 months after owning the vehicle, the GPS unit would sometimes randomly turn on/off and show my current location 1-2 miles away from my actual location. Then around 9 months after owning the vehicle, the unit as a whole failed and the whole tablet doesn’t work anymore. The dealer initially claimed that it was an issue with my onStar plan, but it is actually an issue with the telematics unit that needs to be replaced. This is a very well known problem is searched online across all newer GM vehicles that depend on Onstar.

I am the owner of a 2025 Chevrolet Equinox EV and have experienced ongoing and unresolved issues with the vehicle’s connectivity systems — including OnStar, internet access, GPS functionality, and over-the-air (OTA) updates. The problem first began in late July 2025, when I suddenly lost both internet connectivity and GPS navigation. During that episode, I was unable to use the OnStar system or navigation tools. Although the issue self-corrected after a few days, the instability raised concerns. The issue returned later in the year, and I visited my local Chevrolet dealer on November 24th, 2025, where a reset was attempted. While temporarily effective, the problem recurred. A follow-up service appointment on December 8th, 2025, confirmed that the Telematics Communication Control Module had failed — the same hardware component that controls OnStar access, vehicle geo-location, navigation, and software updates. At that time, I was informed by the service advisor that several other EVs were experiencing the same issue and that the replacement part was on national backorder, with some customers waiting more than two months. I was not given a clear estimate of when the part would arrive. This issue has effectively disabled: All OnStar connectivity (a paid subscription service), The vehicle’s ability to receive manufacturer-recommended OTA software updates, GPS navigation and location services, including vehicle tracking — a feature relied upon by my insurance company. Given the widespread nature of this issue, the lack of available parts, and the critical role of these systems in vehicle safety and functionality, I believe this constitutes a serious safety and consumer-protection concern.

This car is equipped with a telematics module that feeds information to navigation system, Blue Cruise hands-off driving, and provides speed limit information in a heads-up display based on map data. The same module also controls connectivity to On-Star, a safety monitoring service. There appears to be a bug in the software whereby the module doesn't appropriately initialized properly. This has several effects. The OnStar system ceases to function (and the attendant feature of calling emergency services in a collision). The bug also causes false GPS coordinate data to be fed to the car; since the reporting of the speed limit and other alerts are based on this data, along with the Blue Cruise hands-off driving, the driver is presented with false information about the speed limit on the current road (instead using data about the road at the false GPS coordinates). If using guidance, the guidance presents the driver with false and confusing directions (which change frequently as the system believes the driver is in a different place). This bug has happened about 6 times in the past 6 months. There's no mechanism to reset the unit other than disconnecting the battery or fuse. The attached file shows a typical event. The guidance clearly shows the car in the middle of the Charles River in Boston, and the navigation system is instructing the driver to get on Memorial Drive. The HUD (not shown) indicates that the current speed limit is 20 MPH. This picture was taken while on Interstate 93 in Medford MA (speed limit 65 MPH).

Our 2025 Equinox EV was leased new in March of 2025. Unknowingly, it had a water intrusion in the front passenger floor due to a manufacturing defect that GM is aware of. This is related to TSB 25-NA-149. The water intrusion was covered up and went unnoticed for some time due to the factory GM All-Weather floor liners. Once discovered due to the musky smell in the vehicle, black mold was found after pulling up the floor liner. The vehicle was inspected by a Chevrolet dealer, who confirmed it was due to a poor body weld that has now been resealed. Replacement carpet is on backorder due to many experiencing this defect. Our safety and the safety of others is put at risk since GM is not recalling the vehicle for this defect. We have been unknowingly driving around breathing in black mold. As someone who is immunocompromised with an autoimmune disease, this is especially troubling to discover. My autoimmune disease was sucessfully managed with medication since 2017, however, shortly after leasing the vehicle in March I began to experience flare ups. I have been unable to determine why, but this discovery makes me believe it was due to the mold in the vehicle. From discussing with my doctor, mold exposure is a potential trigger of my condition.

On November 30, 2025, while driving my 2025 Chevy Equinox EV LS, the vehicle displayed a “Service High Voltage System” warning message on the center screen indicating an issue with the vehicle’s high‑voltage electrical system and that some driving and/or charging functionality may not be available. The high‑voltage battery and related components are still in the vehicle and available for inspection upon request. This fault could cause sudden loss or limitation of driving or charging capability, which may increase the risk of being stranded in traffic or in unsafe locations. Beginning in the first week of November 2025, the myChevrolet app has continuously shown a “Lithium‑Ion Battery – Schedule Service Now” alert under Diagnostics, even though no corresponding warning appeared in the vehicle until November 30. I presented the in‑app lithium‑ion battery warning to the Chevrolet dealer earlier in November; they declined to act on it, stated they do not rely on the app, and asked me to reschedule and return later, so the issue has not yet been reproduced or confirmed by a dealer or independent shop. The vehicle has not been inspected by the manufacturer, police, insurance, or others specifically for this high‑voltage/battery issue. The only prior symptoms were the persistent lithium‑ion battery service notification in the myChevrolet app since early November and, as of November 30, the in‑vehicle “Service High Voltage System” warning message.

The telematics module has failed on approx. [XXX] and due to a large scale amount of GM vehicles having this same failure the replacement parts are unavailable. My research has found that this module failure has been a problem since at least February 2025 and now in December the parts are still on backorder for each failure. I purchased this car for my [XXX] daughter and now that this module has failed I am unable to track the vehicle and the vehicle's mapping system is useless, along with the ability to locate nearby charging stations. Additionally, the OnStar system is disabled and that is a safety issue. When we took the car into the dealership, they were very accommodating and diagnosed the issue quickly. This was in part due to the fact that my daughter is only [XXX] and cannot be provided a loaner vehicle or rental for age related requirements. I am unable to swap cars with my daughter since I drive a very large truck for work. So, basically we are stuck with a vehicle that has a faulty telematics module for un uncertain amount of time (Dealership expected 6 weeks) all while GM continues to produce new vehicles with the parts it is receiving and not honoring those who have already purchased a vehicle and should have priority to parts under both the warranty itself and the fact that this is a safety related issue. Also, the fact that this has not become a recall or at least a part that needs to be inspected for potential premature failure is unacceptable. GM appears to have zero ability to rectify this in a reasonable amount of time and instead is putting it on the backs of their customers while they continue benefitting from new cars sales with the parts stock that are available. One time (so far) the screen behind the steering wheel that shows the gear and speedometer did not turn on when we started the car. We were able to back the car out of the garage and park it in our driveway. This is a safety concern because we could have driven the car in that condition - without a working speedometer. Our OnStar Control Module on our 2025 Equinox started having connectivity issues. It failed to access apps and showed our location 100 miles from where we actually were, and drains the battery. My vehicle has been at the dealership for over a month for an issue with the HVAC system. The technician and GM engineering support believe the problem is related to a faulty acuator, which requires the entire dash to be removed for repair. (See pictures) This is extremely concerning for a new vehicle with very low mileage. An HVAC acuator failure in a brand new car should not occur, and the repair is invasive- involving disassembly of the major interior components. I am worried this will lead to creaks, rattles, or compromised fit and finish afterward, as well as potential damage to electronics, airbags, and trim panels. In addition, this problem appears to be widespread among other 2025 Equinox owners, based on multiple reports in owner forums describing identical HVAC noise or clicking problems linked to acuators. GM has not yet issued a recall or service bulletin addressing it. Given the scope of disassembly required and the frequency of similar reports, this should be reviewed as potential design or component defect affecting vehicle reliability and safety systems (defrosting performance and interior air management).

On numerous occasions, the multiple automatic collision braking systems have engaged in what I would describe as phantom breaking. For me personally, the automatic breaking has on multiple occasions caused a serious crash, both while backing out of parking, at intersections and most recently on Interstate 880. The AEB system engaged and slammed the breaks while changing a lane in traffic and the car came to a complete standstill. Two cars almost crashed into me. I feel less safe with AEB engaged than I feel without it and I this incident was the final straw. I will be disabling AEB. The system is severely flawed and will be the cause for serious accidents.

I was driving in heavy traffic. It was a traffic jam. The driver in front of me was stop and go. I was keeping a good distance as we were moving along. Suddenly the engine braked so violently I was thrown off my seat and the seatbelt had to catch me. Inadvertently my hand must have hit the the shifting toggle. The car was then in reverse. I was unaware and shocked, so I went to get out of the brake and then started to reverse for it to then brake on me again in reverse, only slower this time. I contact gm, they put me to a dealer, that treated this like it was routine maintenance and had no urgency. They offered me a rental program, but I had to process a lot of paperwork, I asked for another someone just to take care of it, and they just sent me phone numbers. They never advised me not to drive the car.

Vehicle will randomly apply brakes detecting something with the automatic brake system that is not there. I was driving at 30 mph with nothing in front of me and the vehicle applied full brakes and beeped. While driving on the highway during morning rush hour the automatic braking fully engaged during slow traffic almost causing me to be rear-ended. The braking engaged after a split second collision warning after a car had merged in front of me. I was already slowing down via one pedal driving and about to begin applying my brakes when this occurred and there was plenty of space before any type of collision. I was going roughly 5-10 mpg when this occurred having slowed down to allow the car to merge. Afterwards it took a few seconds for the car to allow me to re-engage the gas to start moving again. This type of phantom braking has occurred in other instances in my driveway and in store parking lots but this is the first time it's put me at risk. It is available for inspection as needed. Yesterday the car was moving forward at 2-3 mph, when the brakes suddenly engaged without warning, bringing the car to a sudden stop, violently enough that I thought I had been in a collision. However, there were no cars behind me, and the car in front was about 20 feet away, also moving forward slowly. No pedestrians were in view. A light mist was falling, but otherwise the weather and visibility were fine. If there had been a car close behind me, or if this had happened on a highway, a serious accident could have resulted. A quick internet search shows that this problem has been reported by other Equinox owners, and the only apparent fix is to disengage the automatic braking system. This is a safety issue (both the unexpected braking problem itself and the unsafe fix of disengaging a major anti collision feature), and warrants a recall.

on 10/9/2025 I pulled out of my driveway and less than 300 feet from my house I struck a deer. Weather was approx 50 degree and clear. There was no dew on my windows as I park my car in the garage. There was approximately $6200 worth of damage done to my vehicle and what concerned me the most was, the car never engaged the emergency stop. I mentioned this to the insurance company and they really had nothing to say. I have had bicyclists get too close to me on the road and my vehicle engaged the emergency stop. After repairs were done on the vehicle, they had a third party come in to do a calibration of all the sensors. He could not perform the calibration due to #PIT6274: Error Message Referring to Vision Camera system with DTC B1C48 - (Nov 4, 2024). I just dropped my vehicle off at the dealer and mentioned this to them and they are claiming there is nothing they can do about it. I started looking around online and have seen several other people complain about this and I knew it affected my Adaptive Cruise Control as I have gotten this error a couple times before and I just thought my window was dirty. What I did not know is this also affected my emergency stop as it does not tell me that in the warning. If this is a known issue, this is a serious safety concern and to have the dealer tell me there is nothing they can do is not acceptable. If the camera was functioning properly that morning, there is a good chance that I would not have gotten into an accident.

There is EXTREME glare from the black dashboard reflecting back up onto the windshield in partly sunny to sunny conditions. Windshield glare is horrible. It can cause distortion of objects. It can cause dizziness. Especially on bright sunlight and when many lights from oncoming traffic reflect off the windshield. I have had to pull over a few times. The dashboard shininess is a real problem. The dealerships don’t want to order the free dash mat unless they “diagnose” it even though it is a known problem and they ignored my concerns when I went in to the dealership even though a free dash mat is offered by GM The fix for this in reality is a new dashboard that will end this problem but that is too expensive for GM. They all say to wear polarized glasses to help. Wrong. That’s like putting a bandaid over a gunshot wound. If they aren’t willing to fix this error I want them to buy me out cuz it’s not a small problem that could result in injury or death if not addressed. The 2026 models have a new dashboard that corrects the problem. This problem has caused me not feel safe and avoid driving sometimes and this is not acceptable to me.

Visibility under certain conditions is very poor. Reflection on the windshield from the dash creates unsafe driving conditions. It is especially bad when driving into a dark area like a parking garage or car wash. Almost impossible to see other vehicles or people. The dash grill pattern totally blinds the driver because the sun casts a reflection or glare of this dash pattern on the entire windshield. Glare on the windshield from the reflected dash can be quite profound on sunny days. At times, when the sun is at a particular angle during late afternoon, the glare makes driving in congested city traffic very difficult due to a nearly opaque view through the glass. This is very dangerous. Here's what happened: My 8 month old 2025 Chevrolet Equinox EV FRONT WHEEL DRIVE w/ACC(no Super Cruise) has had 2 instances where a very large "thunk" sound and vibration emanating from the rear of the car while stopped at an intersection, was immediately followed with sudden brake failure and sudden forward acceleration. The ACC was on in both instances. The first time this happened was alarming but since the was nothing in front of me there was no damage and the problem seemed to resolve itself thereafter. The second time was while waiting at a red light about 10 feet behind another vehicle. My car shot forward and into the other car, which was then pushed into the car ahead of it. Both my car and the car ahead of me had very expensive damages, but fortunately, no injuries. What I experienced sounds exactly like the recall problem described in NHTSA Safety Recall Report 25V-12 which has thus far only been applied to the AWD version of the 2025 Equinox EV, not the FWD version that I have. I believe this needs rapid consideration due to the danger of of sudden acceleration resulting in serious property damage, injury, or death. I was driving my 2025 Chevrolet Equinox EV on the highway with Adaptive Cruise Control (ACC) engaged. The vehicle was also set to "One-Pedal Driving" (High regenerative braking). The vehicle in front of me slowed down abruptly. My vehicle's ACC began to slow the car, but it was not decelerating quickly enough to avoid a collision. I immediately intervened by pressing the brake pedal as hard as I could, pushing it completely to the floor. Critical Safety Failure: The vehicle's physical brakes failed to engage. Despite my foot being pressed hard to the floor, the vehicle did not slow down. It felt as though only the regenerative braking was (slowly) working and that my manual brake application had zero effect. The vehicle continued to glide forward with no significant braking power, and I was unable to stop the car in time, resulting in a collision with the rear of the vehicle ahead. Summary of Defect: The primary safety failure was the complete lack of response from the manual brake pedal. My attempt to override the vehicle's automated systems in an emergency was unsuccessful, as the brake pedal was unresponsive. Reporting History: I have already reported this incident to my auto insurance, my local Chevrolet dealership, and the Chevrolet Customer Assistance Center. A representative from Chevrolet recommended that I file this report with the NHTSA to help identify and track similar safety-critical incidents.

I have driven the vehicle 13,000 miles without incident. However, twice this week, once moving forward and once backward, the emergency parking brake engaged without warning, bringing the vehicle to an abrupt stop. The vehicle became completely disabled due to a sudden battery drain at approximately 5482 miles. I purchased the car with 2553 on it in May. The vehicle would not start a few times and had to be towed to the dealership on Friday Dec. 26, 2025. The dealer confirmed that the OnStar/telematics module is malfunctioning and continuously draining the battery. The defective module is on backorder with no estimated arrival date, leaving the vehicle unusable. They also do not know if they have a loaner for me. There are two issues with the headlights as it relates to the instrument panel and controls. First, the symbol on the instrument panel is the same for both headlights and running lights. This means at dusk, I cannot know whether my auto-headlights have turned or if I need to override the auto-feature to turn them on manually. Second, the headlight controls are buried in a menu, hidden behind the steering wheel. The "quick menu" is inconvenient, but the fact that it is hidden bedding the steering wheel means that I have to lean forward and around to fiddle with it, which has led to me drifting into another lane as I am distracted. From the driver's perspective, the "quick menus" are all hidden behind the wheel, and the control panel is just far enough out of reach that the driver must adjust and lean forward to reach for any controls not on the steering column.

The system that failed is the headlight design. I was driving home this evening in a moderate snowstorm, and the snow accumulated and completely blocked the headlights. Vehicle rolls back very often when in drive. Even holding down the brake at a hill then hitting the accelerator it still rolls back. When in cruise control and I take over it also tends to make the vehicle jump when the vehicle takes back over. Overall harsh riding. Feels like it’s all coming from that CVT. The vehicle owner took the vehicle to the dealer Greco Chevrolet of Fort Lauderdale The dealer reported no vehicle failures. The braking system melted due to right temperatures. The dealer reported DTC C0072 Brake System Temperature too high. Report stated front rotor temperature reached greater than 1112 degrees Fahrenheit, and that the rear rotors reached greater than 932 Fahrenheit. The report stated that the high temperature was caused by: -Antilock Braking System -Traction Control System -Vehicle Stability Control No warning lights shown in the vehicle. The dealer gave the owner the number 833-948-4631 to speak with GM tech support. The case was referred to GM Concierge 833-382-4389 Owner alleges that this could only be caused by faulty materials in the batch of this vehicle construction. Is concerning that the manufacturer did not come to the dealer to determine the causes of failure. Is very possible that the same materials were used in building other vehicles in the same batch. Concerning also that these high temperatures could cause fires in the wheels during operation. Is quite urgent that NHTSA visits this dealer and performs a non-bias inspection of the vehicle and the alleged bad materials.

While in a parking lot going about 10mph or less, a car pulled out from a parking space on the left. I did a full brake and despite being dry, warm pavement, the car activated abs and released some brake pressure and allowed the car to continue to move. This ment that my braking distance increased by about 5-10 feet more than you'd expect. I have had this happen 3 times, each time a car was pulling out perpendicular on the left. I tried to replicate this in an open parking lot and it never repeated therefore I think it has something to do with the car approaching on the left and the active braking system.

I called OnStar and was going to have it towed and turned the car off. Once the car came back on all of the warning lights were gone, and then the car drove fine.. I got lucky today, but this is an issue that still needs to be addressed.. The issue was supposed to be taken care of in 2024. I just purchased my 2005 in September and I have 3100 miles on the car.. I was driving in a rural area with many hills and was scared to death. I had to keep my foot all the way on the brakes the entire time to keep myself at a slow speed..

Just after Thanksgiving, my vehicle displayed a “Service Emergency Calling” message. I brought it to the dealership and they documented the concern, referencing GM bulletin PIP6077B, but no repairs were performed. On December 30th, while parking, I shifted into reverse and the vehicle would not reverse and instead moved forward. A “Service Transmission” warning appeared and the vehicle was towed. The dealer cleared the codes and released the vehicle without performing any diagnostics. About two weeks later, when starting the vehicle after work, several warnings appeared, including “Service Safety Restraint System,” “Forward Collision System Unavailable,” and a rear-seat alert despite no passengers. While driving, lane keep assist, lane departure warning, and blind zone steering assist were not functioning. The next morning, the vehicle suffered a complete electrical failure. It would not start, the screens stayed off, and the doors would not lock or unlock. The vehicle was towed again. The dealer replaced the battery, claimed it had simply died, and attempted to return the vehicle to me. I refused because none of the prior warnings were diagnosed, and I was told the codes “erase when the battery is changed.” After I demanded full electrical and transmission diagnostics, including a parasitic draw test, the dealership informed me the vehicle was unsafe to drive. They stated the OnStar/telematics module was draining the battery and the replacement part was backordered with no estimated arrival date. I was told multiple customers are waiting for the same part, which means this is NOT an isolated event and needs way more attention than it's being given. This defect results in loss of essential safety systems and complete vehicle shutdown, creating a significant safety risk.

AND FINALLY AT THE BEGGINING OF NOVEMBER HAD TO TAKE IT TO SERVICE DUE TO A SUDDEN BATTERY DRAIN. SHE WOULDNT TURN ON FOR NO REASON.THE DEFECTIVE MODULE IS ON BACKORDER WITH NO ESTIMATED ARRIVAL DATE, LEAVING THE VEHICLE UNUSABLE. ITS BEEN THERE FOR 2 MONTHS NOW... THEY PROVIDED A LOANER. My vehicle has had recurring interior water intrusion and related safety concerns. The most serious symptom was interior window fogging on the inside of the vehicle for multiple days during or following rainy, snowy, and cold conditions, which significantly reduced visibility while driving and made it difficult to see out of the windshield and side windows. To clear the fogging quickly, I have had to turn off the heated seats and defrosters and open the windows; otherwise, I have needed to pull over and wait until I could safely see through the windshield again. A strong mildew odor would also develop and could last for days after rain. Sometimes the seats felt damp and the odor became worse when the heated seats were turned on. The first repair visit for this issue was January 13 to 19, 2026 and during that visit, the dealer was unable to recreate the issue. The problem continued, and after my navigation/infotainment screen began having issues, I returned the vehicle to the dealer again on March 7, 2026. I was then informed there was water intrusion and water-damaged carpeting, and I was told there was a known issue involving a faulty or broken seal on this model and there was a recall, but I have not been able to confirm that recall by VIN online. It remains unclear whether the leak is the full cause of the repeated interior fogging condition. In addition to the water intrusion issue, the vehicle has also had other electrical and safety-related concerns, including the infotainment screen turning white for weeks, abnormal turn-signal behavior after completing turns, lane-assist steering irregularities that pulled the vehicle toward the lane line, and braking irregularities, including the brakes locking up while backing into a parking space or during slow stop-and-go traffic. It is currently unknown whether moisture intrusion contributed to any of these other issues. My vehicle is still with the dealer for repairs until around April 15.

21" Equinox EV RS wheel cracked at the lip even though we do not recall hitting anything. No known potholes or other impacts. This caused air loss from the tire and had to refill the air at least once a day. When the tire shop removed the tire to inspect it further we found no damage to the tire, just the wheel was cracked. Called dealership and they claimed it would not be a warranty issue. We ended up putting different wheels on the car as we didn't want to have this issue again.

The vehicle has experienced repeated failures of the integrated telematics system, including loss of GPS location accuracy, loss of data connectivity, navigation malfunction, and intermittent failure of voice command and steering wheel controls. On one occasion, the vehicle experienced a complete display blackout while driving, during which both the infotainment screen and digital instrument cluster went entirely black. The condition has occurred repeatedly and persisted despite multiple repair attempts. The vehicle has been out of service for approximately 82 cumulative days for diagnosis and repair of these issues. An authorized Chevrolet dealership ultimately diagnosed a failure of the telematics control module, which was replaced under warranty. Following this repair, the same data connectivity failure recurred and was documented with photo and video evidence. The vehicle was returned to the dealership for further diagnosis. The failure is intermittent and may not be present during every inspection. The telematics system supports navigation, driver information displays, and emergency communication features, including transmission of vehicle location and system data. Loss of GPS and connectivity may impair the vehicle’s ability to provide accurate location information during an emergency. The prior display blackout resulted in loss of driver information while the vehicle was in motion. The condition has been reported multiple times to authorized Chevrolet dealerships. Diagnostic procedures and programming were performed, and the telematics module replacement confirms a system-level failure. Recurrence after component replacement indicates the condition has not been permanently resolved. Prior symptoms included loss of GPS signal, incorrect location, loss of connectivity, navigation failure, and intermittent driver assistance alerts.

The onstar telematics module has stopped functioning. That means I am not receiving any systems updates, cannot use onstar, and cannot use the my Chevrolet app for remote start or remote unlock. I took it to the dealer 2 weeks ago and they said the telematics module is backordered. They did do a number of system updates. After those updates my adaptive cruise control has completely stopped working. When I called about that, the service department said that one module could have caused the other module to fail and that they won’t try to look at my cruise control until after they can replace the telematics. This is a 2025 vehicle with 25,000 miles and the Check Engine Soon light stays illuminated due to a faulty PCV Valve or possibly a poorly designed crankcase. The engine code is P04FB and is commonly found as an issue when you do a search. As a safety precaution, the vehicle is programmed to not allow the remote start to work as designed when the engine light is on so this issue affects the overall functionality of the car. Having spoken with the dealership, they report seeing that engine code often and that the part is on backorder. The dealership seems to think it's the crankcase causing the issue and not the PCV Valve as the fix they have been ordering for them is the case. It will potentially go away as the weather is hotter, but if a chilly night or morning comes up, then the light will come back on because there is moisture buildup on the PCV Valve when the temperature drops that causes the light to come on. With all of the complaints I have read online for this very issue, I would like to see NHTSA step up and make Chevy fix their problem. Thank you.

While operating the vehicle on both interstate highways and local roadways, an intermittent malfunction occurs involving the All-Wheel Drive (AWD) system. The Chevrolet Equinox EV has a large blind spot because of the size of the front window pillars and the adjacent side view mirror. To quote from an Internet search, "This creates a significant blind spot when preparing to make left at intersections, making it difficult to spot pedestrians cyclists, or oncoming traffic." My vehicle was involved in an accident during a left turn in which I believe the blind spot was a significant factor. Brake failure at any speed presents a serious safety hazard.I am aware of similar complaints reported for this issue. My vehicle was towed to and inspected by a dealership where it sat overnight before being diagnosed; however, they were unable to reproduce the issue at the time of service. The service advisor told me to "bring it back if it happens again" and that it "lost communication with the EBCM" which sometimes happens. There is currently no estimated time of arrival for the replacement parts. A known issue with this OnStar failure is that the vehicle’s battery can drain as the system continuously attempts to reconnect. Twice, while driving in falling snow away from home, the front wipers did not work properly when switched to LO or HI. They behaved as if set to intermittent wiping. The intermittent wiper setting worked properly to adjust the wipe interval. But setting to LO or HI did not cause the wipers to go constantly. As far as I could tell, the arm pivot area was clear enough for them to wipe. The problem went away on its own without restarting the car. I took it to the dealer in marlton NJ with the foward collision fault light on and they cant find anything wrong with the system as it resets itself after thr vehicle is turned off for two minutes. On our Facebook page I am not the only one who is experiencing this I will attach some documents of similar issues apparently GM has been aware of this for a while and no fix yet.

My rear traffic alert or any of my other safety features when backing up are not working. On several occasions the entire instrument display including my speedometer and everything has completely blacked out.

The contact then pressed the OnStar button, but the system failed to connect, with the message "Cannot Connect" displayed. The vehicle was taken to a dealer, where it was diagnosed, and determined that the Telematic Control Module needed to be replaced. The manufacturer was notified of the failure. The failure mileage was 10,484.

OnStar Module malfunctioned, has been diagnosed and awaits identification of specific type of module to determine if one is available or if it is one that will be on back order. The safety of myself and my family was put at risk, along with others, when vehicle braking system randomly activated and slammed on the brakes despite clear highways all around, almost causing us to be rear-ended more than once. This has happened prior to 12/01 several times randomly but we assumed the sensor was sensitive and was picking up something we couldn't see, however this was not the case in this situation. The battery keeps going dead and needing jumped and there are numerous warning messages/lamps. Dealer service department confirmed OnStar module failure and need for replacement, has not yet reproduced braking issues. Vehicle is currently at dealership service department for inspection and was deemed unsafe to drive at this time. Dealer stated they had no loaner vehicles to supply and instructed us GM would reimburse up to $45 daily for rental at Enterprise, so a rental vehicle was obtained and placed on waiting list at two different dealerships for loaner vehicle. WiFi and Navigation System originally quit working, then car battery started dying and requires being jumped multiple times if not driven within 8 hours, Received alerts off and on of "Lane Change Alert System Off", "Service Parking Break", "Adaptive Cruise Control Temp Unavailable", "Service Power Steering", "Service Electronic Stability Control", "Service Transmission Now, Unable to shift soon", and cruise control/Lane assist etc quit working. We became stranded in another town and had to purchase a portable jumper. Dealer currently has no anticipated repair date.

I drive 20 miles each way to work every day. Much of this drive is on highways, and currently the bridge connecting Ohio and Kentucky is down to three lanes with no berm. On a windy road with low visibility and a steep hill near my home, the car completely lost power and acceleration without any warning. I wasn't able to drive it completely off the road before it came to a stop and due to the loss of power was unable to move into neutral to push it and the hazards would not work. Thank God that this didn't take place on the bridge or with my family in the car. I made the decision to exit the car while it was still on the road and find a safe area in the grass where I could call for help. When I exited the vehicle, it locked me out and the only way to get in was through the driver's side door, which would put me back on the road in traffic and at risk. The electrical failure prevented me from opening the other doors. The car's complete system failure put my safety/life at risk and could jeopardize the lives of others if it wasn't investigated. To make matters worse when the tow truck came via OnStar they weren't equipped to move a car stuck in park or drive due to the electronic gear shift so they drug it with the tires locked onto the bed. After several days of testing the electrical system, the dealership found a loose wire and informed me that it had been fixed. I picked up the car and after a mile or so, the check engine light came on. I contacted the dealership to come pick it up and that's where it stands at the moment. My intention was to pass this on to my boys when they start driving, but this experience will alter that plan.

On Th, Nov 6 ~1700, the 25 Equinox EV being driven encountered an electric drive failure and brake failure, including friction brakes. This Equinox EV is essentially still brand new with 3900 miles and no accident history. The vehicle was driven off the EB SR 528 exit ramp to Narcoossee Rd in Orlando and driven to the stop light at the WB on ramp. After the light changed and began driving, the vehicle console reported an exception with drive assist. At this time the driver attempted to slow down to take stock of the situation and the vehicle accelerated, at which time the driver took their foot off the brake pedal and attempted to identify a place to take the vehicle off the roadways. Thankfully a stoplight at McCoy Rd was green and the driver continued through that light and while attempting to brake, the vehicle accelerated instead. The driver then coasted, maneuvering the vehicle was into the Vista Palms commercial center. The vehicle, unresponsive to braking, propelled through a stop sign into the Kohl's parking lot at which time the driver began looping around the lot until the weight of the vehicle brought the motion down. While the vehicle was stopped, it rotated through warning messages indicating..forward collision, brake assist is reduced, steering assist is reduced, then finally Service Transmission Now Unable to Shift Soon. The vehicle was towed by AAA to AutoNation Chevrolet Airport in Orlando. Technicians found codes U1610 and U1639, and found K160 connecter unseated. It was reseated and greased. The concern is that this should have been locked and unable to be unseated. Is this an indication of manufacturing problem or operating problem? All braking was dysfunctional. How can we be certain that this was not a one off event?

Returning home on I-40 on Sunday, the engine just quit working. Fortunately it WAS Sunday and the traffic was light. Had there been more traffic, someone probably would have hit me from the rear. We managed to get it almost all of the way off the road before it stopped. According to the dealership that worked on it, it was the fuel supply service module, $46.09 part. The service technician said they had been having a lot of problems like this and knew what to do. All of the code were lost because to tow truck driving had to take the leads off of the battery. That was the only way he could get the car out of park in order to tow the vehicle.

When attempting to engage the Left turn signal, the lever has to be pushed down several times to "catch or engage" and stay engaged. Frequently it won't stay engaged and turns off before the turn is completed. (The Right turn signal works properly). This happens 8 out of 10 times on a consistent basis. The dealer shop supervisor and tech test drove it as well as another brand new lot vehicle and both did the same thing. They said that that's what they all do, and I should push down more firmly, which doesn't help. I'm in my late [XXX] , have driven new GM vehicles for decades and know how to use a turn signal. I have been in the middle of LH turns and the turn signal was not flashing to advise other drivers of my intent to turn. I believe that this is a significant and dangerous safety issue. I bought the vehicle new a little over a year ago and it currently has about 2500 miles on it. I am not sure when this started happening, but I started to notice it at about 1700 miles, 6 months ago when I started driving it regularly in place of my 2024 Chevrolet 1500 pickup. Vehicle was being parked in garage when it accelerated in reverse, on it's own, very quickly. Fortunately I was able to avoid colliding with any thing behind me by braking in time. I then shifted into drive when vehicle accelerated forward, on it's own again, very quickly. I was not able to brake before vehicle crashed into concrete garage wall, severely damaging front end. Vehicle is still under warranty, and I intend to have dealership haul it in for repairs. I have found other drivers complain on the Internet of experiencing the exact same issue as I described above. This model, Chevrolet, Equinox, 2025 is not safe to drive!

On star Telematics is not working the issue is effecting GPS and most importantly the adaptive driving system including cruise control system. I have stopped using these services to due an intermittent failure while driving. The adaptive cruise will disengage itself. Also the collision avoidance system will initiate braking randomly. I was told that the replacement parts are not available and or “extremely long wait time”. I’m not comfortable driving this vehicle and have no options but to continue until the lease is up.
