Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2020-2021 Avalon, Avalon Hybrid, Corolla, Highlander, Highlander Hybrid, RAV4, RAV4 Hybrid, Lexus ES350, Lexus RX350, Lexus RX450H, 2021 Sienna Hybrid, Lexus ES250, 2020-2022 Camry, Camry Hybrid, and ES300H vehicles. A short circuit may develop in the Occupant Classification System (OCS) sensor, preventing the front passenger air bag from deploying.
Reported consequence: An air bag that does not deploy during a crash increases the risk of injury.
Remedy: Dealers will inspect and replace the sensor as necessary, free of charge. Interim letters notifying owners of the safety risk were mailed in February 2024. Some final remedy owner letters were mailed October 31, 2024. Letters will continue to be mailed in phases through January 2026. Owners may contact Toyota customer service at 1-800-331-4331. I am writing to formally file a complaint regarding the ongoing issues related to safety recall 23TA15 for my 2021 Toyota Corolla LE. The recall, issued in December 2023, involves a defect in the Occupant Classification System (OCS) sensors in the front passenger seat, which may have been improperly manufactured. Unfortunately, Toyota has failed to address this recall in a timely manner, significantly impacting my ability to use my vehicle safely and as intended. My local dealership has confirmed that my car is affected by this recall and that the OCS sensor is not functioning. However, despite the recall being issued nearly a year ago, the dealership has informed me that they are unable to obtain the necessary parts to repair the issue, and they do not expect to receive them until at least the end of December 2024—if not later. As a result, I have been unable to use my car's passenger seat in a safe and fully functional manner, effectively rendering the car unsuitable for my needs. This prolonged delay in resolving a critical safety issue is unacceptable, and I am concerned for my safety and the safety of others until this issue is resolved. I kindly request that the NHTSA investigate Toyota’s handling of this recall and take appropriate action to ensure that affected vehicle owners, including myself, are provided with timely repairs and necessary parts. The safety of any passenger in the front seat is potentially affected as the defect can either cause the front passenger airbag to fail to deploy in an accident or cause it to deploy when it shouldn't, either way causing harm to the passenger in the front. This probem was widely reported in the press in 2023 and 2024. One article I read said that Toyota claimed it would start issuing the recalls for repair in Feb 2024 but that still had not happened by the time I bought my 2021 Corolla in April 2024. I bought it as a used car from a local dealer who shared the info about the recall which was on the CARFAX report too. I am reluctant to have family or friends in the front passenger seat because of the risk of injury in the case of an accident of accidental deployment of the airbag. This reduces the value of my car significantly, as it is required by law to be safe for my passengers as well as myself. Very disappointing behavior from a company that used to be reliable.

I was notified that my vehicle has had a recall on it due to the OCS Occupant Classification System sensors being improperly manufactured which could lead in appropriately identifying the front occupant's weight and failing to deploy airbags. This recall was issued on December 20, 2023, and as of today, Toyota has not provided a remedy or timeline for repair. I have contacted Toyota dealerships in Norwalk and Anaheim, both of which advised me not to bring the car in, stating that no fix is currently available. I was recently offered a rental car, but only if I surrender my vehicle to the dealership — something I am not comfortable doing, as I wish to retain possession of my legally owned and registered vehicle. I keep the vehicle stored safely at my home and do not believe it is fair or necessary to forfeit it for an indefinite period. This situation has caused significant anxiety and mental distress, as I fear for the safety of my passengers every time I drive. It has now been over a year since I was made aware of this critical safety issue, and Toyota has not provided a viable solution. I respectfully request: That NHTSA investigate this delay in remedy. That I be allowed to receive a rental vehicle without surrendering my property. Guidance on my rights under federal law, including any steps I may take toward resolution or compensation. Applicable Laws: Under the National Traffic and Motor Vehicle Safety Act (49 U.S. Code § 30120), manufacturers are legally obligated to remedy safety defects at no charge and in a timely manner. I recently purchased a 2021 Toyota Corolla SE. After a few days the "Engine Maintenance Required - See Dealer" notification appeared. See as it was purchased very recently, I assumed this was the oil maintenance light, and reset it. Later that day, it came back, and I reset it a different method under the assumption I did it wrong. The next day it was back, and that is when I realized it must be something else. So I started doing some research and found lots of other people have had this same issue: 2020-21 Corolla, greater than 30k miles, usually less than 60k, and while the Check Engine Light *DOES NOT TURN ON* the car is throwing an error code: P268115 - Engine Coolant Bypass Valve. Sure enough, I have the codes pulled and I have that code showing up. So I took it to the local Toyota Dealership. They stated that the issue was not a warrantable, so I would likely have to pay out of pocket. After a slight fuss, they agreed to fix it as a "one-time goodwill gesture, performed in the interest of customer satisfaction..." So in the end, they did fix it; I did still have to pay for a diagnostic of the issue which is frustrating, especially since I was dead on in my analysis of the issue. To answer the guideline questions: Engine Coolant Bypass Valve Leaking engine coolant could cause the engine to overheat, which can lead to fires, not exactly ideal. It can also cause electrical shorts, leading to more issues. Yes. Lancaster Toyota confirmed the issue. Yes, Lancaster Toyota confirmed that the part was faulty. They appeared shortly after buying the car. There were no error lights like I'd expect, but there was the constantly appearing "Engine Maintenance Required - See Dealer" message which the Owner's Manual states is simply the 5000 mile Oil Change warning. Hidden behind this message is the error code P268115.

Seems like it there's a need a recall for such a thing since it's a defective part they knew about since my car was screaming at me for an engine maintenance. However, the check engine wasn't on. I went to Toyota at Elk Grove, CA and knew what the issue was pretty quick and charging me $700 to get it replaced. I feel like it's intentionally being handled poorly so they keep charging me 700 every few years to replace a $80 part. As well my story is very common with other customers of Toyota Cars made after 2020. I wanted to report this since that's potentially harmful to put out a car that could have engine problems easily. Especially it's a fairly newer car. Also, they have yet to replace the part for the known recall on this vehicle.

The error message "Engine Maintenance Required - Visit Your Dealer" was consistently popping up and I couldn't see my speed or dash or how many miles I had till empty even after acknowledging the message and pressing ok & back. It would stay on while my car was on no matter what. I couldn't use or see any of my information on my dash. The problem has come up multiple times by different people in the auto forum and I was warned that I would have to replace it again in 15 months. There was no light that came on on my dash just the message that would pop up and block my speed so I couldn't see how fast I was going or when I was going to be out of gas. I'm not sure if it's been inspected by the manufacturer. It first appeared after 3 years of owning my brand new car. I was very shocked that I already needed to replace a part in my engine after only having and driving my car for a short amount of time and 30,000 miles on it. It was an expensive part and caused me to have to guess how fast I was going while driving as I couldn't see or get the message to go away even after acknowledging it. At 51k miles the check engine went on and said immediately to go to the dealer. One night shortly after starting the car, I went to put the car in first gear amd as I let out the clutch, but pedal stayed flat to the floor. I was able to lift the pedal with my foot and drive the car back to safety. The next day, I drove the car to the nearest shop before anything catastrophic happens. This time, right in the shop parking lot, I lost all hydraulic pressure in the clutch and I could not take the car out of gear. I had to turn off the car to prevent it from stalling as the clutch slowly engaged without my doing. I had the car inspected by two repair shops—one quote at $3900, the other at $6600. The issue is a failed seal on the clutch slave cylinder, located inside the transmission. The seal is much too thin from factory to withstand normal use. There are no recalls for this issue, and MANY affected owners. This is a safety issue because the failure can be almost immediate and will leave the driver unable to take the car out of gear, leading to out of control driving.

Because the part is internal to the housing, a full transmission removal was required, resulting in a total repair cost of $5,600. No warning lights or symptoms appeared prior to this sudden failure.

At approximately 5 years and 2 months of ownership, with approximately 53,000 miles, the clutch slave cylinder failed in such a way that contaminated both the clutch and flywheel with brake fluid. Repair of this issue requires replacement of the slave cylinder, clutch, and flywheel, as described by Toyota of Pullman in Pullman, WA who performed the inspection and quoted the repair. The symptoms of this failure included an inability to shift the manual transmission between gears, most notably out of 1st gear to get to 2nd. This inability to change gears resulted in limited speed in traffic, and created an obstruction. In less fortunate traffic conditions, this speed restriction could have created a collision. Other owners of the same model vehicle online have reported active failures in traffic, some of which have resulted in damage to the transmission as well as the clutch and flywheel. Toyota appears to have redesigned the slave cylinder at least twice, implying that they know the parts for these vehicles are subpar, and there are no flywheels available for the repair, which also implies this may be an all too common repair.

NOTE: This has not happened to me yet, but it appears that most Corollas manufactured before 2022 or 2023 have this issue. However I did recently flush the fluid from my clutch system and it was very clearly contaminated, possibly indicating early evidence of a failure. Clutch system, specifically the slave cylinder is faulty on these vehicles. It is a known issue on these cars that the clutch slave cylinder (inside the transmission) is made out of resin and fails. When the slave cylinder fails, brake fluid from the shared clutch/brake reservoir leaks out onto the clutch and flywheel, damaging them and resulting in an expensive repair. In addition to this, the brake fluid leak results in a "low braking power" message on the dashboard, indicating a problem with the braking system. Obviously this is a safety issue that could cause an accident. This issue has been confirmed by many dealers and the solution is to replace the slave, clutch, and flywheel and flush & refill the brake fluid reservoir. The vehicle has not been inspected by the dealer as of yet, but in the interest of safety I am considering an early replacement of the cylinder. The only warnings of this issue are a clutch stuck to the floor, and in some cases a "low braking power" warning on the dashboard. A low brake fluid level may also be an indicator of this issue if caught in time. Incident report below is of the clutch flush, showing the contaminated brake fluid from the clutch hydraulic system. It's important to note that flushing the clutch is not a recommended maintenance item at any point on these vehicles as it is expected to be done with the braking system, although that won't remove contaminants from the clutch cylinder.

Yes the car can be inspected. Yes took to dealership today and they don’t know what’s causing it but what between $855 - 1,100 to fix. When it rains the front wipers automatically come on as they should. Then without warning, they stop working and on three occasions a wiper flew off and my teenager had to stop and run into middle of road to get the wiper. She was on a rural highway and could not see well without the wipers working. I replaced the wipers, but when it down poured rain again, like before they just stopped working with no warning. She had to drive 25 mph on a road that was ranges 45 -55 mph. She had a very hard time seeing and could have wrecked. This could have caused an accident had she been on the freeway going 70 mph. A Google search shows others have experienced this issue with their car. The only person who has inspected the issue / car is the Toyota dealership today. They said they don’t know what’s causing it and would cost more money to try and diagnose, but they said it appears if you run the wipers at the very slowest speed it does not malfunction. I don’t have the money to fix it right now, so we are going to try and not drive in rain and if caught in a storm while driving, like before, we will try running wipers at lowest speed as they suggested. First incident was April or May 2024. Most recent incident was July 2024. Sorry I don’t have exact dates.

At around 0750 AM on 20Nov2023, the sun roof exploded while turning roughly 15 MPH onto an onramp of a highway. Nothing hit the car from the outside. The sun roof glass shattered and fell onto the sun roof cover. Glass could have fallen inside as well as onto me and could have obstructed my vision, causing a crash. No pot holes or noticeable bumps were struck prior to or after the malfunction. Car was at a red stop light for a minute or so prior to the malfunction. The temperature was around 45 degrees Fahrenheit when the malfunction occurred. The heat was not turned on in the car at any point. When speaking with the dealership service center, they said no on else has come in with this issue. Over the past year and a half I’ve had to deal with unknown reasons for at least 13 tire problems. And one of those problems was a complete blow out on the highway. When I went to get my spare tire and change it there wasn’t anything in the trunks spare compartment. The airbag light has been on most of the time letting me know the passenger airbag isn’t on or working properly. The engine light has been on for at least four months now. I understand Warranty for regular checkups and maintenance but this feels a bit more serious than me making sure I’ve secured my warranty. I have called and taken the vehicle into both Toyota of Visalia and Toyota of Selma where I got financing for the vehicle. Toyota of Visalia wasn’t very much help at all and preceded to talk down to me. Toyota of Selma keeps giving me the run around and telling me different things about my warranty. This car isn’t worth the trouble my credit score goes through to secure it each month just to have something else fail me in it. At this point I’m trying to figure out how I’m going to replace the DC to DC because the car died a week ago and hasn’t turned back on since. And yes I’ve tried jumping the battery already. Primewell PS830 205/55/R16 - These tires were discontinued by the manufacturer one or two years ago and replaced with an upgraded tire because of tread skid and wear issues. The tires were on a Certified Used Toyota Corolla I purchased on September 29, 2023 from Toyota of Lincoln Park, Chicago, IL. I mentioned on the test drive that the brakes were making noise at every stop, with an occasional thud sound. The salesman, Dustin Dolan, first told me the noise was because the car hadn't been driven recently, then that the noise was normal and then that he agreed the sound wasn't right. He said he'd have the brakes "looked at." They weren't. The car consistently made skidding noises when braking. The dealer continued to tell me the brakes were fine and the tires were fine because they would never sell at car with unsafe tires. I also found a slit in the rear passenger sidewall. Took car to a reputable dealer. They found the tires with uneven wear and choppy condition. They also discovered rusted rotors. I have brought my car to Toyota of New Orleans (the same place I bought it) regarding an issue where if my kids are in the backseat unbuckled the seat belt alert does not ring to indicate that the seatbelts are not hooked also if I undo the seatbelt while driving it does not alert either. While driving my car I noticed a sloshing noise like water, I thought it was a bottle of wiper fluid until I opened and closed my passenger rear door. The door was full of water, I looked it up online and found tips to loosen a plug on the bottom of the door to let the water drain out. I did this and it was pouring out of the door. I called for service, took the vehicle in, asked about damage to electrical inside the door like speakers and locking systems. The man explained due to the way the door is made IF any water got in it would not affect these systems. They advised the plugs can get full of debris and to loosen the plugs after heavy rain. This has now happened several times, I have to check all my doors and drain them after it rains. Note there are three plug holes, all have plugs so no water can freely drain on it's own.

On December 20, 2023, a safety recall was officially generated for my 2021 Toyota Corolla. It has been over a year, and there is currently no remedy for this recall. I was originally notified that there would be an option to fix this by the third quarter of 2024. However, that was not the case. On December 26, 2024, I contacted the service center at Sam Leman Toyota in Bloomington, Illinois by phone, and they confirmed that there is still no remedy for repair.

On December 12, 2025, I was in a head on car accident in my 2021 Toyota Corolla nightshade. I was slammed into the steering wheel which caused injury. My car has over $14,000 worth of damage to the front yet my airbags did not deploy. I have taken my car to Toyota several times over the past few years and every time I’ve asked about a remedy for this recall I’ve been told there is no remedy as early as June or July 2025 I had to bring my car in for a manufacturer defect with the engine that Toyota fixed for free. I asked them about the recall on the airbags. They again told me there was no remedy.

While attempting to change a flat tire on a 2021 Toyota Corolla Hatchback, two wheel studs sheared off completely, leaving the wheel unsafe to drive. My wife was stranded alone in the mountains with no cell service. She managed to loosen three lug nuts, but the final two were seized. When a passerby assisted, both seized studs snapped. The spare could barely be mounted. The vehicle has been serviced exclusively by Toyota dealers since new. Lancaster Toyota later replaced the broken studs free of charge, yet simultaneously claimed that another dealer, Frontier Toyota, had “switched the tyres.” so Lancaster are not responsible. That claim is false — Frontier’s invoice (Sept 25 2025) clearly shows only an oil change and inspection, no tire or wheel service of any kind. Replacing the studs at no charge while denying responsibility suggests an attempt to quietly resolve what may be a dealer-induced over-torque or defective-stud failure. The broken studs were not returned to us despite multiple requests — effectively withheld by the dealer, which constitutes unauthorized retention of customer property and prevents independent verification of failure cause. Toyota Corporate (case #251103002581) was notified but refused to investigate, referring the issue back to the same dealer. This complaint is being filed so NHTSA can document a potential wheel-stud safety defect or service-related failure and dealer mishandling of critical evidence that could conceal a wider safety problem.

The problem of windshield wipers going off either once, or at varying speeds has not been able to be reproduced by my Toyota dealership. It goes off right when I turn the car on 50% of the time and is very inconsistent. Wipers typically go off after using the turn signal, making long turns (such as into a parking spot), and driving on long curved roadways. It has been inspected and nothing has been found. My vehicle was backed into while ì was inside of it in a parking lot. Due to my horn failing., I was unable to alert the other driver to see me. The clock spring is what failed the first time at 95k they replaced it. I'm now at 147k and it failed again . Now I'm discovering that if it does not work, there's an electrical circuit failure and that the airbag will not deploy if I get in an accident. This is a serious safety issue and I feel Toyota should be responsible for it for the life of the vehicle . A horn should never go out. Especially if it not working will keep my air bag from functioning properly . 850 out of my pocket is way steep. The replacement part lasted half as long as the original part did when I bought the car that to me seems like a faulty part this should be obvious and they should just fix the problem cuz I might have to just consult an attorney should they choose not to. The attached photo shows what happens when you can't beep your horn at somebody who's about to back into your car !!! my car still has not turned on any kind of warning lamp for the airbag or any kind of malfunction light, no warnings on my radio. I have noticed that my Lane departure warning seems to work intermittently along with the automatic high beams that like to flash my high beams in people's face for some unapparent reason which also seems like a safety issue. If I had to take my car to an inspection station it would not pass an inspection due to the horn not working ! You cannot take your child to the Department of Motor Vehicles and let them take their drive test if your horn does not work ! which happened to be my case !! Very disappointing to say the least especially when you've paid $30,000 for a car.

I have been taking my Toyota to Sun Toyota for routine maintenance and inspections. Upon today’s inspections they are claiming the battery looks like it leaked on the axial, some of the undercarriage components look broken and rusty. Possibly needing replacement. The overall car is less than 60k in mileage so that seems odd to me. I also brought the car in at 45k and they made no mention of these issues during that vehicle inspection. I asked about my brakes and braking issues. I do not slam on my brakes and the technician said I should stop braking too hard. There is noticeable wear now on them. I asked them to change them at the previous visit and they refused saying it did not need it. I am concerned for my safety and lack of care at this sun Toyota.

Vehicle has numerous electrical/mechanical issues that seem to stem from an autonomous learning remote sensor. The problems include, but are not limited to 1. Seatbelt sticking on driver’s side, problem resolves itself, possibly after someone has had a good chuckle on satellite watching the driver struggle to disengage the safely belt, 2. Trunk remotely popped at speeds ranging from 0 (park) to 70mph on highway, 3. Ignition light on and off, usually correctible by disengaging and reengaging battery lead, 4. Car shutoff and burglar alarm engaged while in park, again , correctible by disengaging and engaging battery lead, 5. Car gas gauge reading false on the light side ( less than E), then after disengaging and engaging battery lead, reverts back to 1/4 tank, 6. Clock time changed, presumably after vehicle shutoff and restarted in the middle of the night. Clock reset via auto computer, etc. All incidents are usually corrected by disengaging and engaging the battery lead, after a brief pause to reset the vehicle’s ECU.

My car shows that the vehicle needed to be serviced, in checking the Toyota app I saw there was a warning for the adaptive lighting system/electrical concern. The warning light had been on for some time, thinking I was due for regular maintenance like an oil change I had ignored it for a bit. Everytime I would start the car this warning would appear. I too the car in for service, they also did a 20k maintenance service and kept the car overnight overnight as they were unable to locate the issue same day. I am making a complaint about my 2021 corolla SE because I am constantly running into issues that cost and are not my fault as to why these issues have come about. I bought my car in March 2021, in May 2021 my car hit a bump in the highway and cracked my rim and bust my tire. Luckily I have alloy and tire coverage so I did not have to come out of my pocket. Next, my two front tires are constantly wearing down on the inside of the tread making the wires be exposed and causing me to have a blow out or needing to replace my tires at least 4 to 5 times since I have had my car. My car only has 71,000 on it to need that many tires replaced. I go and get my car serviced where I purchased my car at the toyota dealer so If I needed and alignment or tires rotated they would have let me know that and the only time I was informed I needed an alignment was when I got all new tires. Then, in November of 2022 my front lighting sytem when completely out ( signals lights, blinkers, brake lights, etc. I went to get it checked on and the dealer made me feel as if I had an entire electrical issue going on with my car and I would have to pay $160 x2 in order for them to just figure out what was wrong with my practically new vehicle. They came back and explained it was only a fuse and the fuse is $5 but still attempted to charge me for diagnostics. After this encounter I had to be ugly just to not come out of pocket for a large sum. At that visit there was an "engine malfunction" light on as well that they did not mend and continues to blink to this date. Prior to my last servicing appointment last week my entire radio system went out ( backup camera, radio, gps system, bluetooth). I was told at the dealer that I would have to pay $160 for them to diagnose my issue and and then pay for the repair. I asked my service manager if there were any recalls on my car for these issues and he stated no but as for the 2022-2023 models there are recalls. I need help with this.

I was rear ended. During the impact, which was enough to accelerate my vehicle from 9 mph to 36 mph, the steering wheel came completely off of the car and hit me in the head. The steering wheel was in the back seat immediately following the impact and my vehicle was traveling at 36 mph with no way to steer out control it and no airbags in front of me in case of a secondary collision. This happened on a busy interstate with dozens of drivers endangered by my out of control vehicle. Traffic had slowed for construction and it was a miracle I wasn't further injured or caused injury to anyone else. Toyota sent an inspector to look over the vehicle, and it may still be available on the wrecker lot.

My 2021 Toyota Corolla, with 3000 miles and less than a year on it, is repeatedly exhibiting a "check gas cap" warning message followed a day or two later by a "check engine, visit your dealer" warning. This has happened twice. I have returned to the dealer once to have the system reset, after which the same sequence of warning messages recurred. This message wipes out my dashboard display and keeps me from using my cruise control and radar distancing feature.
