Complaints Filed with NHTSA

The following are consumer-reported complaints filed with NHTSA. These are unverified allegations and are not proof that any defect exists.

Power Train49 complaints

At a stoplight the truck began running rough. When the light turned green, the truck failed in just 3 blocks (approximately 1 minute). Power was greatly reduced. Check engine illuminated on dashboard, then another red icon, followed by a stop safely now message. Immediately I attempted to pull over from the left lane to the right lane but the truxk Immediately stalled across both lanes, blocking all traffic. Ford dealer has ordered a new fuel pump and the repair is pending.

NHTSA #11741724

The transmission had a lengthy delay shifting into reverse when shifted from park. I removed my foot from the brake and all of a sudden it jumped into reverse and I ended up in the road. Fortunately there were no cars or pedestrians behind me.

NHTSA #11740249

Truck fails to accelerate when transmission is in second gear, travelling at low speed (typically under 10 MPH) and at low RPMs (typically ~1,500). Regardless of amount of pressure applied to accelerator, the truck will remain at low RPM and speed for up to 10 seconds. This problem occurs frequently and has been reproduced by a Ford dealer. Ford TSB 25-2302 describes this problem, but in my case a DTC is not present. Because a DTC is not present, the Ford dealer refuses to proceed with corrective actions as outlined in TSB without charging me for diagnostics and labor, although the truck is still under warranty. Failure to accelerate puts me and others at risk when attempting to merge, when accelerating in slow traffic conditions, etc.

NHTSA #11736298

With less than 44,000 miles on my truck it suddenly began to shift in an eradic manner. It is undriveable. This is the Ford 10 speed 10 R 80 transmission. The truck is only 5 years old but is out of warranty by 17 days. Seems strange.

NHTSA #11733635

When starting from a stop or rolling slowly under 15 mph, pressing the accelerator pedal — even flooring it — produces no engine or motor response for 1–3 seconds. The vehicle then suddenly accelerates. This occurs intermittently on both electric and gas power. Safety concern because it delays merging or pulling out into traffic.

NHTSA #11730848

Trans is hanging shifting into 2nd gear, 4th gear, 7th gear, and 9th gear. Also has gotten stuck in 10th gear 3 times. Had to roll over to the side of the road turn the truck off and restart. Also have hard shifts from 1-3 and then have hard downshifts from 5-4-3.

NHTSA #11730873

Unreasonable Delay in Remedy for Recall 23V896 / 23S65 I am filing this complaint to report an unacceptable delay in the remedy for NHTSA Recall 23V896 (Manufacturer 23S65). This recall was issued in December 2023, yet as of April 2026, the manufacturer continues to claim that parts are unavailable for repair. This delay has persisted for nearly three years, during which time the manufacturer has continued to produce and sell new vehicles while leaving existing owners at risk. 1. Component Failure & Inspection: The components involved are the rear axle bolts. The vehicle and the specific components are available for inspection upon request. 2. Safety Risk: The safety of myself, my passengers, and others on the road is at significant risk. A failure of the rear axle hub bolts can result in a total loss of motive power (the ability to move the vehicle) or cause the vehicle to roll away while in park. Both scenarios significantly increase the risk of a high-speed crash or injury to pedestrians. 3. Confirmation & History: The necessity for this repair has been confirmed by an authorized dealer service center. However, the vehicle has not yet been inspected by the manufacturer’s corporate representatives, police, or insurance. 4. Symptoms & Warnings: There were no warning lamps, messages, or physical symptoms of the problem prior to the failure. I only became aware of the defect through the official recall notification provided by the manufacturer and the NHTSA. Conclusion: It is unacceptable that a known safety defect remains unremedied for this duration. I am requesting that the NHTSA investigate the manufacturer's failure to provide a remedy within a reasonable timeframe as required by law, especially considering the continued production of new units using similar components.

NHTSA #11729807

The contact owns a 2021 Ford F-150. The contact stated that while driving at an undisclosed speed, the vehicle experienced a loss of automotive power. No warning light was illuminated. Additionally, while depressing the brake pedal, the vehicle jerked, and the transmission unexpectedly downshifted to first gear. The failure had occurred twice. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The contact associated the failure with NHTSA Campaign Number: 22V188000 (Power Train); however, the VIN was not included in the recall. The manufacturer was not made aware of the failure. The failure mileage was approximately 83,000.

NHTSA #11733991

Transmission is shuddering at speeds up to 60mph. Can cause the vehicle to jump repeatedly.I am worried at such a high speed the vehicle could lose control and put my life and others at risk. Also can shift very hard when coming to a stop so much so that it feels like I’ve been hit from behind by another vehicle. I’ve never had any type of messages or warning lights prior or post. After searching up issue I have found hundreds and hundreds of other vehicle owners with the 10r80 transmission to be experiencing similar problems. Ford has acknowledged the problem and have a fix but at a high cost. Some people are not able to repair vehicles at that moment could be putting themselves and other at risk of death or injury.

NHTSA #11729134

My truck started shifting gears hard while driving and wouldn’t go into reverse when parking. Performance transmission shop is telling me I need a new transmission. Getting multiple error codes.

NHTSA #11729128

Two of my catalytic converters have failed twice. The first time was at the end of January 2024, the second time was at the end of March 2026. All regular maintenance on the vehicle has been completed at a Ford dealership. The only explanation I have received for the failure of the catalytic converters was "the parts were defective." I have owned five Ford trucks in my lifetime and this is the first time the catalytic converters have failed, and they have failed twice on a five year old truck. This is, however, the first hybrid truck I have ever owned. Perhaps there is a defect in the design of the motor that the general public may not be aware of? Hopefully you can look into this situation to prevent someone else from going through the drama that I'm having to deal with. Lightening has already struck twice and I'm just praying that it will strike the third time for me before the two year warranty runs out on these catalytic converters that were just installed in my truck.

NHTSA #11729276

Vehicle: 2021 Ford F-150 XLT Issue: Transmission Malfunction and Safety Concern I am submitting a formal complaint regarding a serious and ongoing transmission issue with my 2021 Ford F-150 XLT. The vehicle has exhibited multiple dangerous and unpredictable transmission behaviors that raise significant safety concerns. First, the truck experiences a pronounced 3–4 shift flare. During this shift, the engine RPM rises abnormally high before the transmission engages the next gear. This creates a noticeable delay in acceleration and reduces driver control, particularly when merging into traffic or accelerating from lower speeds. Second, there is a severe hard engagement when shifting from Park into Drive or Reverse. The vehicle produces a loud and forceful clunk, accompanied by a jarring motion. This issue occurs consistently and gives the impression of excessive internal wear or mechanical failure within the transmission system. Third, the transmission intermittently becomes stuck in gear or behaves erratically. This includes sudden, harsh downshifts without driver input, bringing the vehicle to a stop, and occasions where the vehicle resists changing gears altogether. These events are abrupt and can be dangerous, especially at highway speeds or in heavy traffic conditions. Additionally, there have been instances of delayed engagement and hesitation when selecting Drive or Reverse, further compounding concerns about reliability and safety. These transmission problems are not only disruptive but also pose a real risk of accident or injury due to loss of predictable vehicle operation. I am requesting that this issue be formally investigated, as it may indicate a broader defect affecting similar vehicles. Thank you for your immediate attention to this matter.

When stopping for a redlight the 10r80 transmission locks itself into 10th gear. I have taken 7 or 8 actual videos for proof of that. Also when leaving a intersection the transmission runs up to the rev limiter in 1st gear then slams into 2,3,4 and 5th gear. Unfortunately I don’t have that on video because I am already moving when it happens. Also unfortunate for me is I called the local dealership and shared with them the issues and asked for a transmission service to be done on it and between the phone call and the service time the factory warranty ran out. The day after the transmission service and 8 days after the factory warranty expired the pickup left me stranded with no forward gears. So here I am with a truck with only 52,000 miles on it scratching my head… no check indicator lights have shown up either.

NHTSA #11739631

My truck has harsh shifting when I accelerate and when I slow down. It skips out of gear into neutral and then goes back into gear. I believe the problem is that my system isn’t communicating with the transmission correctly. There weren’t any warning messages before or after this problem started. I did some research, and my model and year of my truck do have a problem like this no dealership or service center has not inspected this problem

-The hub bolt in the rear axle broke. Truck had to be towed to dealership. Problem was confirmed by dealership. There was no warning lamp and only found out due to flat tire. -In 12/2022, power train failure left vehicle "dead" in the middle of the highway with family of four stranded 400 miles away from home.

I put the truck into reverse and tried to back out of my driveway. The truck didn't move for several seconds, then started to move. I brakes, shifted into drive and the truck continued to move backwards. I applied the brakes and the transmission seemed to spin before it moved forward. I drove down the street to the corner. I applied the brakes and the was a shudder and bang as the transmission downshifted from 4th to 3rd. This occurs intermittently over the last 3 months. When I searched online, I learned that these symptoms are consistent with those described in Ford TSB 23-2350. I had an independent mechanic confirm the problem and he noted the fluid was dark. Whrn I spoke to the dealership, they tried to explain the issue was due to the transmission being cold. I'm the original owner of the truck.

The transmission shifts hard when downshifting from 10th gear on the highway. There is a loud audible clunk that you can also physically feel. Also when shifting around 3rd gear during city driving there is the same problem. Both of these could lead to loss of traction/vehicle control.

NHTSA #11721102

When the truck is put into reverse especially when cold the transmission does not initially go into gear. When it finally does the vehicle lurches rearward violently and I have to slam on the brakes to not hit anything. The transmission was just "bench" rebuilt under the Ford powertrain warranty supposedly with a CDF drum and valve body replacement along with software updates including adaptive reset and relearn with the appropriate drive cycles however the vehicle still exhibits this condition. I called Ford customer service and filed a complaint with them last month. They told me to bring to another dealer which the vehicle is currently at and has been for a month. They claim that it is normal for this transmission and refuse to fix it. I have video evidence of the truck being put into reverse and the RPM rising before it slams into reverse. I also demonstrated first thing one morning to a technician after a cold soak overnight and he agreed that the vehicle has a delayed engagement in reverse and when it did finally engage it did so violently. The dealer told me to record it and that would be the best evidence and I did along with presenting it to them. After this they changed their mind and said actually a technician has to witness which I also did. Again they said this wasn't good enough. A field engineer supposedly looked at the vehicle and they wont tell me what he said. Very odd. I feel like im getting the run around and have an unsafe vehicle that Ford refuses and it trying to worm their way out of safetly fixing! In the description below is when the truck can be seen in the video recreating the problem however it has done this countless times.

There is a hard jerk that happens when moving from a dead stop and then it will shift into gear. Its been reproduced by a maintenance shop. They stated it was the transmission. Loss of power pulling onto a major highway -- luckily I was able to pull off onto a side street but had I not I could have been rear ended. Vehicle has been at Athens Ford since January 15th -- Ford states on their recall notification that parts are readily available -- they are not. I have been waiting for 2 months to get my vehicle back and have no resolution in sight. Dealer states they have no ETA on parts availability; Ford Customer Service also does not have an update.

10 Speed transmission jumps at highway speeds, clunks when stopping, and slips when cold. I've had it to the dealer and they replaced the torque converter, and I've had it back several more times. The garage refuses to allow another appointment since Ford engineers say the slipping and jumping is within specs and since no lights or warnings are noted I just have to live with it. The mechanics can feel it but are helpless to do anything about it. Ford refuses to fix it and it is a well-known problem.

I had my 2021 F-150 into the local ford dealership not even a year ago due to a shifting issue. I was on the interstate and passing a car in the passing lane on the interstate and my truck shifted so hard I thought I lost something and then kinds lost power for a minute. The dealership ran a test on it, and it failed. It was some type of valve that was bad I guess ford and GM are having the issue. Now I am starting to have the same issue on now it is down around 4th to 3rd or 3rd to 2nd gear, but when it shifts once in a while it is a very hard shift and it scares the hell out of you. Now once in a while when I go to shift from drive to park or drive to reverse the shifter will not move. Like I said I am getting ready to get ahold of the dealership again and get it in to be looked at I am just waiting for winter to be over.

NHTSA #11716678

Incident Description:The vehicle experienced a complete internal transmission failure while driving, creating a severe and sudden loss of power on the roadway. A professional technician's diagnostic assessment confirmed active fault code P07F7 (Incorrect Gear Ratio 10th Gear) and explicitly cited Ford Technical Service Bulletin TSB #25-2474, pointing directly to a failed internal mechanical CDF clutch drum sleeve. On October 31, 2025, the dealership attempted to patch the issue by replacing only the external electronic transmission solenoids. However, this did not address the true internal mechanical defect, causing the transmission to fail again immediately with the exact same symptoms.Ford has acknowledged this severe design flaw via an internal Customer Satisfaction Program but has strictly capped coverage at 70,000 miles. Because this identical mechanical failure occurred at 81,893 miles, Ford has denied all financial and safety assistance to fix the actual broken mechanical hardware. This widespread manufacturing defect affecting 10R80 transmissions causes vehicles to suddenly lose propulsion in traffic, creating an imminent crash hazard. This campaign must be expanded to a mandatory safety recall.

NHTSA #11741332

I have received multiple letter from Ford indicating the solution for this recall 23S65 is available at dealership. However, every ford dealership I have reached out to says there are no parts available to fix the recall. My ford dealership has placed the order on November 2025 but they have no ETA when the parts will become available.

NHTSA #11713555

My vehicle has been with the Ford dealership for about a month and the mechanic and the engineers still have not found why my vehicle is giving a service wrench warning. When driving the vehicle will flash red warning lights which makes the vehicle unsafe to drive. I have paid for an Auxiliary Transmission Pump and battery. This did not fix my truck. The dealership said they replaced the PCM, but it didn’t fix the truck. I seen that these trucks had recalls.

NHTSA #11710031

My 2021 Ford F-150 has been inoperable for over three months due to a safety recall involving the rear axle hub. The dealer and manufacturer have been unable to remedy the recall within a reasonable time due to parts unavailability, and no repair timeline has been provided. I requested a loaner vehicle from the dealer and was told there were six to seven customers ahead of me. The dealer reported to Ford corporate that I declined a loaner vehicle, which is not accurate. No loaner was made available to me, and I was forced to obtain alternate transportation through my employer. The dealer initially required payment to inspect my vehicle for the recall and held the truck for approximately one week before stating the issue was not recall related. When I asked how this determination was made, I was told additional payment was required to disassemble the rear hub for further inspection. After escalating the issue and expressing concern about the handling of the recall diagnosis, the dealership director contacted me and later confirmed that the issue was, in fact, related to the recall. However, I was told it would take a couple of months to repair due to parts backorder. At this point, my vehicle has been unusable for over three months with no repair timeline, no manufacturer provided loaner or rental vehicle, and no resolution. I am continuing to make payments and maintain insurance on a vehicle I cannot safely operate due to an unresolved safety recall. This appears to be an unreasonable delay in remedying a safety defect.

The transmission will downshifting and the transmission starts jerking then the engine starts missing for about 30 seconds to a minute but the ford dealership said it's not showing any codes so it's not happening now I can't get it fixed they didn't think having a vehicle that starts missing for no reason isn't a problem but the torque converter won't throw codes and it's attached directly to the crankshaft so it's messing with the engine to get it to start missing

NHTSA #11701590

I have taken my truck to two seperate ford dealerships. Both acknowledging something strange on the transmission with the hesitations, the jerkiness and the hard shifting that happens. The first replaced the trans fluid and the filter. The second dealer replaced the valve body. During the 5th visit. They had me drive around their foreman to show him the problems. While he did feel the jerkiness and hesitations I feel, he said that its pretty much not bad enough yet and that the dealership would be held responsible for the cost of a new transmission if they replaced it and ford deemed it not needing to be replaced. Ford corp told me they wouldn't intervene with their attempt to fix it. The ford dealer sent me home and didn't fix anything the 5th time, even after explaining I felt harsh jerking on the way home to the point it felt like it was kicking me and a bunch of white smoke came out of the tailpipe. I even provided them a video of the white smoke when completely stopped after had been driving for 3 hours. The service on this truck is completely up to date and absolutely no reason at 41k miles I should have all these issue with this transmission. Ford has been resisting to replace this transmission but is willing to sell me a $4k extended warranty. Its getting old. This saga needs to end. Ford needs to just recall these transmissions already. This fight has been ongoing long enough! And ford corporate wont even help me. During the 3rd and 4th visit they replaced the valve body sent me home and the next day I had to bring it back after coming out of my work and transmission fluid being all over the ground right after they "fixed" it. They didnt give me a tow truck but rather made me drive almost 20 miles to their dealership. They claim the transmission pan warped but they just didnt screw the pan on correctly and ensure the pan seal was in good shape.

The transmission was slipping in 2nd and 10th gear. This was confirmed by the dealer. A full transmission had to be replaced at 66,000 miles. The vehicle had an excellent maintenance record and well kept. Ford did pay half of the cost but I still ended up spending $6k out of pocket and my vehicle was in the shop for 27 days.

The 10r80 transmission is failing. I have safety concerns for myself and others due to loss of acceleration and violent down shifts. The issue has been reproduced by my mechanic shop. The dealership will not help me with the issue. There have been no dash lights until the failure occurred. The only indication was some occasional rough shifting leading up to the incidents.

We have taken our F-150 to the dealership multiple times for a clunking issue under the truck, and have been told that it is related to the recall issued. Although, it has never been repaired. We take it in a couple of weeks ago for the exact same issue that has been going on since October of 2024 and suddenly it is a transmission issue. We are told that they can gladly repair the transmission for $10,000 now that they put us off until the extended warranty expired. I think this is happening a lot, as the exact thing happened to my friend's daughter. Also, it appears upon doing a little research that 2021-2024 F-150s and Explorers have this issue frequently well before they reach the 100,000 mile mark. Many times before the buyer has even paid off the loan for said automobiles. This surely poses a safety risk, as well because you never know when the vehicle is going to fail. Also, my husband took his truck in telling them that it wouldn't go into gear and the shifter had a problem in February of 2024, it took the truck's shifter literally breaking off in the driveway (thank Goodness) and being towed in to the dealership before it was repaired.

Ford has released multiple TSBs related to a known design flaw with their 10r80 transmission that causes the transmission to experience hard shifts while moving and often occurs in traffic. The hard shifts can cause the vehicle to stall which creates a very unsafe situation, especially on highways and interstates. The transmission defect has been redesigned in newer vehicles, however, there are thousands if not hundred of thousands of these vehicles on roadways that can break down suddenly causing a major accident. The ford owners forums have been riddle with concerns about this issue related to the CDF drum in the 10r80 transmission.

We own a 2021 Ford F-150, that is down since the first of September, 2025, due to recall 23S65. We've been in weekly contact with our local dealer since then. They are doing everything possible to get the parts in to repair our truck, but parts are and have been on backorder, with no ETA from Ford. We have also been in contact with Ford customer service in Dearborn and had an open case with them. They have closed our case because they can't help and told us the local dealer is responsible, then they moved us to recall department now and no longer respond. We feel 4 months is a ridiculous amount of time on Ford's part for these parts to be on and still on backorder. We have no option for help and wondering if you could. It's has been an extremely difficult situation for us to be without our truck for this extended amount of time.

I'm reporting that my driving/fog lights aren't working and haven't for some time. My truck only has 58,000 miles on it and is only 5 years old, I have been guided to report this issue to the NHTSA. I drive allot in the early morning and am NOT getting the lighting I should, it's agravating that you pay so much for a vehicle and the lights don't work properly. I have read 100's of post/emails and vlogs, there are thousands and thousands of these trucks with that same issue, what can I do to repair or replace these lights? Thank you, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The passenger side day time running light is dimmed drastically and almost not visible anymore on the upright section. After doing research , this seems to be a very common issue and not a cheap repair. Some states day time running lights are mandatory to work to pass inspection. Ford has not acknowledged this issue and/or making profit from changing out the whole headlight assembly to fix. It seems like it is a bad design or low quality product for a supposed safety issue.

NHTSA #11723768

Both Quad LED side strips (vertical daytime running lights) on my 2021 Ford F‑150 have burnt out. The failure occurred gradually, with the left strip failing first around [insert approximate date] and the right strip failing shortly after. These LED strips are a safety feature, and their failure reduces visibility while driving. I have attached a photo of the burnt DRL, a photo of a working DRL for reference, and a screenshot of my Ford Live Chat showing my ticket number and that I am tracking this issue publicly in a Facebook group of other owners experiencing the same failure. This appears to be a recurring defect affecting multiple trucks in the 2021–2023 F‑150 range. I am filing this complaint to document a safety defect that may require investigation.

NHTSA #11715973

Driver side LED DRL light has failed. This is a common problem occuring with many people with 2021+ year models of the Ford F-150. Truck is barely outside of warranty and with 51,000 miles and should not have a failing LED DRL. Additionally, Ford has made this part unrepairable and according to the dealership closest to me will require a full headlight assembly replacement.

NHTSA #11713065

The daytime running lights only partially work and are very dim, blue, or completely off.

NHTSA #11708626

Day running lights vertical ones seem to be going out on the F150 and recently mine too.

NHTSA #11706949

The driver's side Daylight Running Light (DRL) is out and there is not a bulb to repair this, it's a $2000+ repair per the dealership and it's 4 years old with 80,000 miles on it. This needs to be a recall. These lights should last way more than 4 years.

NHTSA #11696268

right running light has went out. yes, it will be available for inspection. lack of lighting causes trouble seeing at night while driving. brought to the dealership was told they would need to change the entire headlight. that woul cost around $2000.00. (both sides are now out). from research the 2021 ford f150 running lights are known for going off. $2000.00 all the time get to be big. the running lights do dim a little right before they go completely out. the running lights are all connected to the head light. there is no way to replace the running lights with out change a $2000.00 head light. not to may people have $4000.00 to replace headlights on a 4 year old truck.

NHTSA #11699336

There is a known problem with the DRLs burning out, which poses a safety issue. Many people have had this problem causing an expensive repair out of warranty. The entire headlamp assembly has to be replaced.

Fuel/Propulsion System9 complaints

Luther ford mobile technician came to my place and cleared my existing FUEL PUMP failure recall. I called their attention and insist they did not cleared my recall when I immediately notify them. NHTSA #11731969

There is no fix or remedy available for this recalled fuel problem. The recall and fuel pump replacement isn't available and is taking too long. There is hesitation due to this issue.

I own a 2021 Ford F-150 that is part of recall 25S75 for the low‑pressure fuel pump. Ford announced this recall years ago, but there is still no fix or parts available. This is a safety issue because the fuel pump can fail and cause the truck to stall or not start. I rely on this vehicle, and it’s frustrating that there is still no remedy after all this time. Many other F-150 owners are reporting the same problem and also cannot get their trucks repaired. I am asking NHTSA to push Ford to release the repair and make parts available.

My vehicle loses power when turning into traffic, when stopping and taking off. There have been SEVERAL close calls when pulling into traffic as my vehicle will stall out and won't accelerate causing traffic to have to swerve to miss hitting me. I am going to end up getting hit one of these days. Also, it is very dangerous on snow and ice. When trying to accelerate, it cuts out losing power. Keeping my foot on the accelerator, it will eventually get power amd take off unexpectedly and fast. This causes a hazardous issue as well. I have tried to get Ford to buy back the truck or trade for a different one but they refuse to do anything. This is my only transportation and I have to drive it. I am going to end up seriously injured or dying if my truck stalls out in traffic. I can't travel with my grandson because I would be risking his safety. Please make Ford Company replace or buy back my truck and full loan pay off.

In July 2025, i received Ford's Recall Notice stating that is it possible for fuel pressure and flow from the fuel tank to be reduced due to a fuel pump failure- the same condition that caused my breakdown on 11/5/2025 towing the vehicle to the dealer for repair. Because this repair occurred prior to the official recall repair availability, or remedy as is it referred to. The repair cost was deferred to us because there is no official remedy to the recall, and reimbursements are not being entertained until June of 2026.

Vehicle suddenly locked the steering wheel at approximatelyn20-25 MPH dash displayed a front camera failure and the motor stopped. I had to put the vehicle in park and restart. Two things, the dash stopped displaying speed. As I was driving the dash glitched out and the digital speed would not display. It started only showing the top half of how fast I was going. Then it completely went out. Second, the radio also glitches. Won't let me change station or sources. I usually have to turn the truck off for 10 to 15 minutes, then it starts fixing the issues. This has been an ongoing problem. And no recalls for this issue have been placed

The hybrid battery in a 2021 Ford F-150 Powerboost failed due to apparent shorting. (P/N ML3Z-10B690-D) Pictures were taken by service department at the Ford dealership, and can be provided upon request. There is concern that the system could catch fire and spread under the vehicle. There have been other similarly identified failures reported in online communities, but cannot verify the information. Failed component is currently with dealership service department, therefore, cannot verify if there has been further inspection of part. Prior to failure, there were no indicators of problems or faults. As the failed part is not easily accessible and there is no routine maintenance, there is no way to validate when the part failed.

My Ford F150 was traveling on I95 North in Maryland when an emergency notice on the dashboard told be to pull over. The truck would not restart and had to be towed to a Ford dealer. I have since been told a controller module caught fire and damaged the hybrid battery and both would need to be replaced at a cost of over $10,000. This is a know issue and other Ford F150 Hybrid trucks as well as other Ford cars have already been recalled. Ford claims that my VIN was not included in the recall even though I experienced exactly what the recall was suppose to prevent. My wife and I could have been killed while traveling on the highway at 70 MPH and the vehicle was on fire without our knowledge.

We have already filed a report regarding multiple unresolved warranty defects that Ford Motor Company has refused to address for over a year. Among these is a recurring fuel pump failure—an issue that has now occurred multiple times. The most recent incident happened just a couple of weeks ago in the Twin Cities after being told by our dealer that there was not an issue or fix for the fuel pump issue, where the truck stalled in active traffic, creating a situation that could have resulted in serious injury or worse, death! Despite repeated attempts to resolve this matter, Ford’s latest response is unacceptable and has directly led to this additional complaint. We have been told that no action will be taken unless we personally drive this unsafe vehicle to a dealership for inspection. When we made it clear that doing so would put us at risk, we were dismissively told to have it towed if we were unwilling to drive it. Let us be absolutely clear: we will not endanger our lives to accommodate Ford’s refusal to honor its warranty obligations. We are being asked to operate a vehicle with known, unresolved defects that have already proven dangerous. This is not only irresponsible—it is indefensible. If this matter is not addressed immediately and appropriately, we will have no choice but to escalate further.

Engine,Fuel/Propulsion System3 complaints

While driving my 2021 Ford F-150, the vehicle suddenly began shaking, lost power, and shut off completely while in motion. This created a dangerous situation as I was left without acceleration while driving. Prior to the failure, there were no major issues with the vehicle. After the engine shut off, a battery warning appeared, but the battery had been in good condition and less than a year old. The vehicle would not restart and had to be towed to a Ford dealership, where it has remained for over two weeks. Independent mechanics and the dealership have both indicated the issue is related to the fuel system. I later discovered this matches an active recall for the low-pressure fuel pump. Despite this, I was informed there is currently no remedy available for the recall. The vehicle is still at the dealership and has not been repaired. I have not been provided with a loaner or rental vehicle and have been left without transportation for work or to care for my children. I have also been asked to pay out of pocket for towing and battery replacement, even though the failure appears to be related to a known recall issue.

I have been driving it after 4 -5k miles of this and I was alerted today after replacing ( All Injectors, Coils, Spark Plugs, ETC ) That I may need a new engine. This could have caused a crash and is now costing me an entire engine due to no fix or parts availability. Thousands of Dollars for a new engine out of my pocket due to fuel starvation. I started smelling gas in the cabin of the truck when I came to a stop. As I was driving on the freeway I noticed my gas gauge going down rapidly. Within minutes I ran out of gas. Got the truck towed to Ford dealer. After diagnosing the issue I was made aware by Ford service department that one of the high pressure fuel injector cracked and was spraying gas out which caused me to run out of gas. With gas spraying in the engine I was at severe risk of a fire. No warning lights or messages appeared when this happened. After investigating I noticed Ford has a recall for this issue for other vehicles but not the F-150.

Visibility/Wiper3 complaints

Here is what happened on Feb 21st 2026 at about 8:00 AM MST. We were driving east in the mountains on I-70 in Colorado approaching the Eisenhower tunnel. The truck had been parked in a heated garage overnight. There had been some snow the night before which had been melted by sand and salt, so the road was wet and messy. The vehicles ahead and around my truck were kicking up a mist of water and sand and dirt. My windshield quickly became covered and obstructed. I attempted to apply some windshield washer fluid, but nothing came out. To make matters worse, the wipers automatically turned on turning the speckled dirt and sand to be smeared across the windshield making it almost impossible to see. The smear was too dry that any further wiper action did nothing to clear the windshield. This happened while entering the tunnel. In the dark or the tunnel I could barely see the road – only by ducking way low and peeking through a small portion of the windshield below the wiper smear. There is no place to pull over inside the tunnel. As we exited the tunnel, we were facing the east sun which lit up the smear and made the windshield completely obscured - even my last tiny piece. We were still at 50 MPH with surrounding traffic. My only option was to ease right and hope the emergency lane was clear. I had to hang my head out the side window to determine how far to ease right. It was a miracle we were able to pull off and not hit any other vehicles and come to a stop (I was completely blind). I tried the washer fluid many more times but nothing. I opened the hood and verified I had plenty of fluid and that it was liquid and not frozen. I checked the spray outlets and made sure they were not blocked in any way. Nothing. The washer spray would not work. After turning the truck off and back on - the washer/wipers worked fine. I believe this is a software issue. My family and I were quite shaken up.

The windshield wipers on my 2021 Ford F-150 are inoperable. The wipers suddenly stopped working with no warning. The vehicle had already received Recall 23S10, which was supposed to replace the defective wiper motor arm. However, the wipers have failed again, suggesting the recall remedy was inadequate or that defective components are still being used. Ford and the servicing dealer refuse to reopen the recall or provide coverage, even though this is clearly the same safety issue. Without functioning wipers, the vehicle is unsafe to drive in rain or snow. I request that NHTSA investigate whether Ford’s recall 23S10 remedy is effective and whether additional corrective action or a new recall should be issued to address continued wiper motor failures on these vehicles.

This is a continuation/escalation of NHTSA complaint 11689917. My 2021 Ford F-150 has experienced repeated fuel-system/electrical fuel-pump failures. The vehicle previously stalled and later cranked but would not start, returning code P0627 “Fuel Pump A Control Circuit / Open.” The in-tank fuel pump was replaced at owner expense for approximately $1,400 at an independent service center in September of 2025, but the repair did not permanently resolve the issue. The vehicle has now again lost fuel pressure/propulsion while traveling approximately 50 mph on a two-lane highway in a similar manner to prior failures. I was able to get the vehicle off the roadway, and it required towing back to my residence. This is approximately the fourth or fifth fuel-related failure, including stalls at low speed and non-start conditions related to fuel in parking lots. The symptoms appear consistent with Ford/NHTSA recall 25V455 / Ford recall 25S75, but my VIN was not included in the affected group. I am concerned that the recall population may be too narrow or that the remedy does not address the full defect pattern.

Guessing the date below Truck shuts off when trying to take off and go. This has happened a few times and even pulling onto a main highway. This is a very dangerous situation, and I have contacted Ford and they have stated there is no fix for this. I have contacted Carvana which is where I recently purchased the truck within the past month and they state the recall was listed and there's nothing they can do it's up to Ford to fix a recall. They will not take the truck back and now we are stuck paying for over $30,000 on this truck and it keeps stalling on us. We cannot afford for it to just sit in our yard and pay for it if it's not running. We need help to get this fixed. We have no idea what's causing it but assuming it could have everything to do with the open recall or the battery. That battery is over $4000 if we have to replace it. Another thing we noticed is the LED lights on the headlights went out randomly and haven't come back on. Another very expensive fix for a truck we just purchased less than a month ago. We took the truck in the shop within the first few days of owning it due to a few issues we felt were not right like pulling to one side, feeling as if it were going over rutted areas on the roadways with a bump and jerk to it as well like it was a transmission issue. The insurance company with Carvana choose the place to take it to and we dropped it off and the next day they called and stated nothing was found to be wrong with it and Carvana knew we were out of return in time to replace it when they had us drop it off it was like another day.

I’ve had the transmission replaced along with the torque converter when both failed less than a year ago. The transmission AND the torque converter are now doing the same thing it was doing prior to the replacement. The transmission slips in 4th, 7th and 8th gears. The torque converter whines loudly. Ford will not fix the issues with these transmissions without a hefty out of pocket fee. It’s become very well known that all of these 10R80 engines/transmissions are a major problem that ford is not remedying. If this transmission gives out when I’m on the interstate it’s going to cause a major wreck. I live in the mountains, so a bad transmission can quickly turn into a deadly accident. These transmissions need a permanent fix, not for us owners to continue paying thousands of dollars for replacements or rebuilds. This is fords problem.

NHTSA #11725769

The vehicle is experiencing a chronic, documented loss of engine coolant with no external leaks identified, accompanied by a persistent odor of burning coolant. Despite nine months of notification and two separate Ford authorized dealerships attempting repairs, the defect remains unresolved. To date, dealerships have performed only 'surface-level' pressure and dye tests. They have refused to perform thorough diagnostics despite the clear evidence of consumption. This creates a severe safety hazard: coolant loss on this engine platform is a known precursor to sudden engine stall and potential engine fire. By failing to identify the internal leak path, Ford is requiring me to operate a vehicle with an unpredictable mechanical deficit. The absence of a Warning Lamp (CEL) does not mitigate the risk; it confirms a mechanical failure that the current onboard diagnostics are failing to capture before a catastrophic event occurs.

My 2021 Ford F-150 Lariat has a door latch defect that occurs in freezing or near-freezing temperatures. The failure occurs without warning in cold conditions, causing the door latch to freeze and not fully engage. When this happens, the door is able to freely swing open while the vehicle is in motion and also freely open when the vehicle is parked. This creates a serious safety hazard, including the risk of occupant ejection and loss of vehicle control. The vehicle has no aftermarket modifications and is maintained per manufacturer recommendations. The issue is temperature-dependent and has occurred on multiple occasions. Ford has issued recalls for similar door latch freezing defects on multiple other Ford vehicles and model years. Despite this, my vehicle has not been included, even though it exhibits the same failure mode. This defect presents a significant safety risk and should be investigated for potential recall inclusion.

I was in the parking lot of my local grocery store, and the truck all of a sudden gave me an indicator that there is a "park brake fault-service now." My truck thinks my parking brake is engaged so it constantly flashes the “brake” light, constantly chimes while driving and says parking break malfunction, talk to dealer. I brought it to Webber family Ford in Detroit lakes MN 56501 and they checked it over including the safety recall location for recall 23S35. They said it was not the recall location and I would have to bring it in Monday for another appointment. When I got it home that day I crawled under to check that location. I see visible green which usually means corrosion and the protection around the harness seems warm. I also noticed the "recall" was not completed how it is shown on fords recall paperwork using the zip tie as a wear location so it isn't the harness itself. That is not how mine is. I have photos to prove that. I spoke with corporate ford today and they said they can't re open the recall because it has been closed and it is past its 12 month warranty being it was done in October of 2023 before my ownership and at a different dealer. I don't believe it is fair to me the current owner to pay for the cost of a rerepair of a recall that was done incorrectly.

2021 ford f150 PowerBoost hybrid. There is a LOT of internet chatter and forum chatter about KNOWN issues with the following, and it recently almost led to a major and likely serious injury accident. Specific Parameters: Low speed (~5 mph) slow roll Under internal combustion power, not battery In 2nd gear only When gas pedal is pressed vehicle does nothing. Does not respond. Two choices are to hammer the gas, or hit the breaks. Does two things, slams forward eventually after 3-4 second delay if gas pedal depressed OR shifts down or into electric mode (internal combustion off) and then will once again go from first to third in normal acceleration progression and pace They must have an issue with the standard 1-3 skip programming with ICE power and 2nd gear being a hang-up I almost got T boned by a semi truck due to this with family in car. This could have been deadly. It happens as an annoyance at least once per day but this one was scary. Online, and based on what I can find - Ford is aware of this issue as they have the 1-3-5 programming in the transmission but when it finds itself (due to real world driving conditions) in 2nd gear under Internal combustion, it does not know what to do. I brought it to Ford last year in Anchorage and they are attempting to blame it on normal hybrid system operation. That is NOT the case. It would be something that could be duplicated but with effort. A basic, simple test drive is NOT going to find this. They are aware of this being an issue. If you have any situation where you are rolling through, merging into traffic, or other scenario where you end up in 2 under ICE you are in TROUBLE. Dangerous, scary, and totally a manufacturer issue that is known and needs to be taken VERY seriously.

I ended up paying for the repairs caused by this recall of over 1000.00 dollars. I have drove it now for two days and already the brake light engaged. Luckily it released and I was able to drive home. I have called Ford on this multiple times as well talked with supervisors and need escalation at this point.

Electrical System,Service Brakes,Back Over Prevention1 complaint

My 2021 Ford F-150 intermittently displays the warning message “Reverse Brake Assist Not Available – See Manual” when the vehicle is placed in reverse. When this message appears, the reverse brake assist / parking collision warning feature is not available, which may reduce safety while backing up. The vehicle was inspected by a Ford dealership. The dealership confirmed DTC U3000:49 in the IPMA module and identified Ford TSB 23-2040 as applicable to the concern. The dealer stated that the repair requires an IPMA software/module update and calibration. The quoted repair cost was approximately $1,100. This appears to be a software/programming-related condition affecting a driver assistance and brake-related safety feature, not normal wear and tear or a failed mechanical component. The warning is intermittent, but it has appeared multiple times. The vehicle is available for inspection upon request. No crash, injury, or fire has occurred. Ford Customer Support was contacted and case number XXX was opened. Ford advised that no active warranty, recall, or customer satisfaction program currently provides repair cost coverage, and that because this involves Reverse Brake Assist, I may choose to file a safety concern report with NHTSA. After a day of heavy rain, the vehicle stalled at a stop light and would not restart. The steering wheel and transmission locked, preventing the vehicle from being placed in neutral, creating a safety hazard as the vehicle was immobile in traffic. No warning lamps appeared prior to the failure. The vehicle required a tow. The dealer confirmed water had intruded through the passenger side cowl/firewall seam due to inadequate factory sealing, tracking down and pooling directly onto the Body Control Module (BCM) mounted on the passenger side firewall. The BCM failed due to water damage, causing PATS desynchronization and the no-start condition. The BCM failure subsequently shorted out the starter motor. Dealer replaced the BCM, repaired the leak, and replaced the starter. Total repair cost approximately $3,200. Failed components are available for inspection at the dealership. This identical failure; passenger side cowl water intrusion leading directly to BCM damage — is documented across numerous 2021+ F-150 owner forums, indicating a widespread factory sealing defect at the cowl/firewall/inner fender seam junction.

Steering,Lane Departure1 complaint

The EPAS (Electric Power Assisted Steering) module experienced an internal sensor failure, causing a sudden and complete loss of power steering assist. The vehicle is available for inspection upon request and the EPAS module has been retained. The loss of power steering occurred without warning while the vehicle was in motion, requiring significantly greater physical force to maintain control of the steering wheel. This creates a serious risk of losing vehicle control, particularly at higher speeds, during lane changes, or in emergency maneuver situations. Because the failure occurs randomly and without any prior indication, there is no opportunity to safely prepare or pull over before it happens, putting both the driver and other motorists at risk. Yes. The issue has been acknowledged by multiple Ford dealerships and independent mechanics, all of whom confirmed this is a known and recurring problem with this vehicle’s EPAS system. Numerous online automotive forums also document other owners experiencing the identical failure, suggesting this is a widespread issue rather than an isolated incident. The vehicle was inspected by an authorized Ford dealership, which confirmed the EPAS module failure and acknowledged familiarity with the issue. No warning lights, dashboard messages, unusual sounds, or handling changes occurred before the failure. The power steering loss happened suddenly and without any prior symptoms, leaving no opportunity to anticipate or react to the malfunction before it occurred.

Service Brakes1 complaint

When I'm driving my truck, usually when I have come to a stop on an incline, the parking/emergency brake will engage on its own. For example, most recently I was pulling out of a driveway with an incline. I was stopped at the bottom of the driveway to wait for the roadway to clear before I pulled out. When I hit the gas to go, the rear wheels skidded and the truck came to a stop in the oncoming traffic lane. I looked at the dashboard and saw that the parking brake symbol was on, indicating that the parking brake had engaged. I did not engage the parking brake- the truck did it on it's own.

Brought F150 in for scheduled service - oil change. Also asked for inspection of a problem with the fuel door hatch that was causing "Refuel Error" messages on the instrument panel and several grinding noises every time the fuel release button was pressed. Note that I have an active extended Ford Premiumcare warranty. Dealership diagnosis was that the fuel door solenoid was bad, verified that the gears could be heard grinding with this part. However, there is no separate motor part, therefore the entire fuel door assembly must be replaced. Ford warranty system does not cover this part because it is considered "Body and Structural". I called Ford Protect to discuss this, and explained the motor defect and problem, that this has nothing at all to do with wear & tear or body damage. They explained that in this case, the dealership should call and get the part pre-authorized with the justification. Seelye Ford called and tried to do this on 28 April 2026, but ultimately was still denied by Ford. The attached service report from Seelye describes the technician findings and confirms the bad solenoid and audible gear grinding sound coming from the fuel door assembly - and that the entire assembly must be replaced. As this is related to the fuel system and has a defective motor, this is an example a safety defect that should not only be repaired on this vehicle, but for other vehicles with similar problems. Note that the dealership also pointed out that Ford appears to have created this part layout with the intent of having the motor be a separate part, but ultimately did not offer that option, only a full housing replacement, which is excluded because it is mostly made of plastic and metal, which is considered to be structural in nature, despite being a part of the fuel system that has active motor buttons and instrumentation sensors for driver interaction.

Vehicle Information 2021 Ford F-150 PowerBoost VIN: [XXX] Complaint Description While driving approximately 70 mph on a highway, my 2021 Ford F-150 PowerBoost suddenly displayed a “Stop Safely Now” message and immediately lost propulsion. The engine shut down without warning while I was in active traffic. After the shutdown, the vehicle automatically shifted into park and would not allow me to shift into neutral. This left me unable to move the vehicle out of a live traffic lane. I was forced to shut the vehicle off while still in a dangerous roadway position. This incident created a serious safety hazard, as I was traveling at highway speed and could have been rear-ended due to the sudden loss of power and inability to move the vehicle. This issue appears to match Ford Technical Service Bulletin (TSB) 23-2317, which describes “Stop Safely Now” warnings, no-start conditions, and gateway module (GWM) software faults. This indicates a known defect that can cause sudden loss of propulsion. A claim has already been opened with Ford Motor Company under case number [XXX], and the vehicle is currently at a Ford dealership for diagnosis. At this time, the issue has not been resolved. This failure occurred without warning and presents a significant risk to driver safety and others on the road. Power Train,Engine,Fuel/Propulsion System1 complaint

Several times, the vehicle has failed to accelerate or respond to accellerator pedal input for several seconds. It seems to happen when switching from electric to hybrid modes, and when shifting between lower gears simultaneously. Once the switch to hybrid occurs the engine will rev high with some acceleration even if the accelerator pedal is no longer being pressed. This acceleration has happened when I had my foot over the brake pedal in anticipation of pulling over to stop since the truck would not accelerate. There are no warning lights or messages that appear on the vehicle. It is dangerous because it seems to happen when trying to accelerate from a lower speed. If it happens when trying to change lanes in stop and go traffic or while pulling into traffic other cars would have to brake hard to avoid hitting our vehicle that has experienced stalled acceleration. It is also dangerous because once the transition happens there is brief uncommanded acceleration.

This report is being made within 1 1/2 hours after the incident. It has not been inspected by a dealer, service center, nor any other agency. I was stopped at a local fast food restaurant waiting to pay for my purchase. The 2021 Ford F-150 was in drive with my foot on the brake. No warning lights or other indication were present. Without warning, the engine revved to a high rpm and the truck shot forward about 15-20 feet before I could react by putting my full weight on the brake pedal. The engine quickly slowed. I then put the vehicle in reverse and backed up to the window and placed the transmission in park. The truck is equipped with adaptive cruise control but it was not active at the time of the incident. Had there been another vehicle in front of me, I would have rear-ended them. I work from home and use the truck a couple of times per week and it is available for inspection most days.

Parking Brake1 complaint

The contact owns a 2021 Ford F-150. The contact stated that the vehicle was serviced under NHTSA Campaign Number: 23V509000 (Parking Brake), but the failure persisted. The contact stated that the failed to respond upon shifting to reverse(R) or drive(D). Additionally, the parking brake had seized. The messages "Parking Brake Fault" and " Parking Brake Service Required" were displayed. The dealer was contacted but refused to repair the vehicle under NHTSA Campaign Number: 23V509000(Parking Brake). The contact stated that the dealer had previously inspected the vehicle but no repair had been performed under the recall. The dealer instructed the contact to check the wiring harness. The contact followed the instructions and found that a wire had rubbed through and was fractured. The contact spliced the wire together with a wiring butt connector as a temporary repair. The manufacturer was not made aware of the failure. The failure mileage was approximately 134,000.

Electrical System,Seat Belts,Service Brakes1 complaint

1. Electrical System Failure: While traveling on highway, alternator failed suddenly within 2 minutes of “service soon” and “check electrical system” warnings. Caused complete loss of motive power, electronic parking brake locked up while driving, loss of power steering, all gauges failed, transmission shifted into park while in motion. Battery was damaged (“fried”). Engine continued running for several minutes after stop without ability to shut off ignition. Incident documented by Ohio State Highway Patrol. 2. Seat Belt Issue: Driver-side seat belt interlock sticks in unlocked position, creating risk of failure to lock during a crash. 3. Electronic Parking Brake: Intermittently engages while vehicle is moving downhill to a stop (mostly at or below 5 mph). Parking brake light flashes three times then turns solid while in motion. Ford TSB indicates vehicle not eligible for related recall. 4. Infotainment System: Intermittently freezes, requiring forced reboot. Preceded by audio glitches, laggy dash computerized systems. 5. Transmission: Severe clunk and physical jolt when decelerating from ~30 mph to 8 mph. Also occurs during acceleration, particularly upshifts from 2nd to 3rd and 3rd to 4th gears, more pronounced when turning. 6. 4WD System: Intermittently fails to disengage from 4WD HI to 2WD HI. “Failure to shift” message appears several minutes after command to return to 2WD HI. 7. Front Differential/Axle: Previously, faulty front differential housing fractured due to system malfunction, causing intermittent loss of front driveline engagement in 4WD LO (250-500ms delay then re-engagement under throttle). Ford replaced front differential and axle under warranty after vehicle out of service for 2.5 months. Abbreviations: 4WD HI = 4-Wheel Drive High Range; 2WD HI = 2-Wheel Drive High Range; TSB = Technical Service Bulletin.

Ford has refused, on multiple occasions and at multiple dealerships, to address the NHTSA Recall Number 23V896 (MFG. Recall Number 23S65) issue with the Wheel End Hub Bolt in the rear axle. Well, this weekend, the bolt on the passenger side rear axle broke off, rendering the vehicle undriveable. I brought it in to the Lithia Ford Dealership (who's online information said they were open) to get it addressed. They were closed, no one was there, and they won't even give me a loaner vehicle while the F-150 is inoperable. This recall has been out for two years now and they still won't deal with it. Absolute neglect of their obligations and an unsafe practice!

The contact owns a 2021 Ford F-150. The contact stated that while driving approximately 45 MPH, the contact was cut off by another vehicle that had pulled out in front of the vehicle. The contact depressed the brake pedal with force to keep from crashing into the vehicle. The contact stated that the seat belt pretensioner held firmly in place; however, the passenger's seat belt failed to restrain the passenger in the seat. The contact stated that the passenger almost crashed into the front passenger’s side dashboard. The vehicle was taken to a local dealer, where it was diagnosed, and determined that the passenger’s side seat belt retractor and pretensioner needed to be replaced. The vehicle was not repaired. The contact researched online and related the failure to NHTSA Campaign Number: 21V653000 (Seat Belts); however, the VIN was not included in the recall. The manufacturer was not informed of the failure. The failure mileage was approximately 46,000.

While driving my 2021 Ford F-150 PowerBoost, the vehicle issued a pre-collision warning followed immediately by a restraint system warning. The airbag then deployed without any collision or impact. There was no crash, obstacle, or reason for deployment. The airbag deployment shattered the windshield and caused interior damage. This appears to be a false activation of the pre-collision system and unintended airbag deployment, creating a serious safety hazard. This could easily result in injury or loss of vehicle control. The vehicle needs to be investigated for a potential defect in the airbag system, sensors, or pre-collision assist system.

At a stop light vehicle will turn off and nothing electrical will work. It will have to be restarted, While turning at low speeds the electronic steering completely stops working and almost hit another car. All electrical power failure until modules reset then will start. This all happens randomly so it could happen at any point and cause serious injury. Ford can't seem to find a repair for the intermittent issue.
