Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Reported consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024-2025 Toyota Tundra, Tacoma Hybrid, Tacoma, RAV4 Hybrid, Land Cruiser Hybrid, Lexus GX550, 2024-2026 Tundra Hybrid, 2025 Sequoia Hybrid, Crown Signia, Grand Highlander, Lexus TX500 Hybrid, Lexus NX350 Hybrid, 2025-2026 Grand Highlander Hybrid, and Lexus TX350 vehicles. The load carrying capacity modification label may display inaccurate added weight values. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."

Reported consequence: A vehicle with an incorrect maximum capacity weight value may be overloaded, which can increase the risk of a crash.

Remedy: Dealers will replace the load carrying capacity modification label, free of charge. Owner letters were mailed May 26, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB04, 26LA04, 26TB07, and 26TA07.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 RAV 4 vehicles. The front seat brackets for the seat rails may be welded improperly.

Reported consequence: Improperly welded seat brackets can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and repair the seat bracket welds as necessary, free of charge. Owner notification letters were mailed June 19, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB09 and 26TA09.

ELECTRICAL SYSTEM: INSTRUMENT CLUSTER/PANEL

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Reported consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. After starting the RAV4, the Apple CarPlay quits unexpectedly both while stopped and while driving. Once it quits, the only way to reactivate Apple CarPlay is to stop the car, shut the engine off, then restart. This is extremely dangerous especially while using the map for guidance and it suddenly quits. The issue was reported several times to the local Toyota dealer, but they could not replicate the problem, even after leaving the car at the dealer overnight. The issue is not infrequent, and occurs almost every time the car is driven.

The multimedia screen which shows the map and direction guidance became to blank occasionally, just white grey since February 2026. I immediately lost the direction. It could be back by restarting the engine at the beginning. The problem got worse, happened more often, some times the screen couldn't be back by restarting engine, but I found that it could be reset by pressing the volume button for long time. I reported to Toyota local dealer - Hendrick Toyota Apex and Toyota Brand Engagement center, the case manager is Freddy. The local dealer run diagnosis twice, couldn't detect any problem. On May 11, the screen totally died, black, no any light. Restarting engine or pressing the volume Botton couldn't get the screen back any more. the audio system is still working, I tried to use the map on my iPhone, the direction map couldn't be showed, only the text direction shows. Because the system let map show on screen, it assumes that the map shows one screen but it is not. I lost car screen and the phone direction at the same time. Plus the backup camera doesn't show on the screen. Therefore, it is underivable in any unfamiliar area, and dangerous to drive in neighborhoods. I reported to local dealer and Toyota Brand Engagement center with videos. May 18, the local dealer run diagnosis again, found nothing wrong. But before I leave, the screen became blank. This morning it happened again. The problem is not fixed. Freddy told me he suggested to local dealer to replace the screen, I am waiting Hendrick Toyota Apex call me for the appointment. I was driving the speed limit (75) when I heard an explosion. I immediately pulled over to see if I had a flat tire or if someone had shot my car. I could see no damage and nothing was showing up on my dashboard so I continued to drive but heard a whistling sound. I checked all my windows and then my sunroof. Somehow the sunroof had exploded. There were no cars close to me and no overpasses or trees in the vicinity. I have had the car for almost 3 months and have never opened the sunroof.

During an AWS outage on 10/20/2025, the connect app which can turn on or off the vehicle, unlock/lock doors, etc had issues where it performed an unlock on our vehicle 5 times in 24hrs without us doing anything. I was driving on CA Highway 20 on Thursday afternoon, Oct. 2, when the sunroof spontaneously shattered. I was not driving under a bridge or overpass or tree. There were no vehicles directly in front of me. I heard a loud pop, like an exploding bottle, but could see no damage inside the car. Within a few miles, glass was flying off the roof and striking cars behind me. That's when I pulled off the road and saw the roof was shattered. The shape of the break mounded up in the middle, implying the roof had broken upward. There were no rocks inside on the sunroof cover, nor any other debris than shattered glass.

Unintended/unwanted sudden acceleration - we are aware of this happening 5 times while I was driving, all times coasting to proceed into a turn. (I DO NOT DRIVE WITH 2 FEET). In each instance, while coasting my right foot covered (above) the brake pedal. BOTH feet were off the gas and brake pedal. Tachometer suddenly and rapidly spiked and car appeared/sounded like it would surge forward. In hopes of avoiding this sudden surge forward, I quickly put car in neutral and applied brake. Car slows and tachometer returns to normal. Brought to dealer, they stated from black box inspection that I was applying brake and gas simultaneously. THIS IS NEARLY IMPOSSIBLE FOR ME; as I demonstrated to folks at dealer I physically cannot do this without causing the arthritic pain in my ankle/foot to rise to a very noticeable level. The pain would be prolonged while doing what they claimed I was doing. I vehemently deny their explanation for these occurrences. No accidents occurred, nobody was injured (in these incidents). Obviously those in my car and others near my vehicle on the road should this happen again could be injured. No warning lights, messages etc were displayed. Sporadic occurrences. Purchased vehicle July 11,2025, first occurrence (I noticed) at July 31, 2025. Dealer stated their case is closed on this as of 9/04/25. Today 9/17/25 another occurrence happened. Car is available for inspection upon request.

The vehicle's CarPlay is malfunctioning, showing incorrect vehicle location and navigation directions. The vehicle's screen often shows incorrect location and directions, and changes directions as it updates where it thinks the car is. This is available for inspection upon request. The safety of the driver and passengers is put at risk because the screen showing incorrect location and directions causes a constant distraction, taking your eyes away from the road to figure out if the map is showing you the correct location or not. Sometimes, when the carplay is looking for the correct location, the map spins around and shifts location which is also distracting even when you try to ignore it. The consistent distraction this causes while trying to navigate through unknown streets has caused both my wife and I to get lost, take the wrong turns, or get stressed and confused while driving because we don't know what the directions are telling us, or we don't know if we are following the right directions. We are worried we will hit a car or person during these times of confusion. This has been repeated multiple times at Toyota dealership and is a known issue acknowledged by Toyota corporate. The vehicle has been inspected by Toyota's regional technician who installed an unreleased software update at my last visit, but the problem persists without improvement. There isn't a warning sign. I have requested a replacement or reimbursement for this vehicle from Toyota but their Dispute Resolution team says I have not given enough opportunity for Toyota to repair the car. I then brought the car back in and they kept it for three weeks and installed an unreleased software update. The loaner car they provided me was the same car different year and had no carplay issues. I am trying to bring it back but the dealership cancelled my appointment telling me they won't accept the car because they can't fix it. I am trying to escalate again to corporate.

I was driving on the highway and heard a very loud crash/explosion noise. Glass fell all over the inside of the car, the panoramic sunroof exploded. I believe this is the spontaneous explosion issue that has been reported in other rav4s, as there was no known road hazard seen or found, no overpass where anything fell, and there is no clear impact point on the broken glass, no other body damage anywhere else.

When I try to opt out on the app, I get an unexpected error and to try again. It seems as though the DCM in my car has stopped communicating with Toyota’s servers.

After purchasing a new vehicle, I have become aware of a persistent and widely reported problem with the Toyota/Lexus battery maintenance/charging logic for the 12V batteries in their HEV and PHEV vehicles. This results in chronic under-charging of the 12v battery due to low charging voltage even during normal use and produces a failed 12v battery within 6-24 months. The problems are so widely known and investigated by owners that there is now a complete white paper on the subject: [XXX] Toyota/Lexus needs to respond to this and provide a software patch that addresses the well-documented issue so that new owners like me are not confronted with a choice between (a) driving the vehicle and risking getting stuck in an unpleasant or dangerous situation when the vehicle fails to start; (b) modifying the manufacturer's system and risk voiding my warranty; or (c) spending additional money on diagnostic equipment for something that is a clear manufacturer error. The contact learned that the manufacturer had outsourced the manufacturing of the windshields from a company called AGC. The windshield was not replaced a second time. I was fearful so I drove further down to the fire dept to assess to the damage and ensure no one was following me. Called 911 for help because I was so shaken. The power rear liftgate will close on a person or object and cannot be stopped except if one presses on a very specific part of the edge of the liftgate, towards its center. This happens no matter how much upward force is applied to the area of the pull handles and the edges. When my wife pulled the handle, the liftgate closed with force that she could not resist. This obviously creates a serious safety hazard for small children, pets, or really anyone trying to close the liftgate or who may have inadvertently hit the button or pulled the handle.

I noted the body is made up of soft materials that slightly bend when push lightly with hand. I put a magnet to see if it’s made up of metal but noted a weak attraction of the magnet to the body. When I used my buttock to push the door to close ( my hands were not free and held several grocery bags), my buttock sunk to the part of the area I pressed on. I heard and felt the bending and clicking sound of plastic then it lifted back to its shape. I almost lost my balance since I’m not expecting the door that soft. I was scared that I dented the door. So how safety is this 2025 RAV4 ? We also have 2023 RAV4 of same model XLE AWD Premier. I wonder what will happen if it has accident with another vehicles on the road. With this body materials will save me from any kind of collisions ? I just noticed this brought by my experience today.

The contact stated while driving 70 MPH, the rear passenger’s tire detached from the rim. The contact stated that the failure had occurred while driving on a three-lane highway. The vehicle was driven over to the shoulder of the roadway. The contact inspected the vehicle and noticed that the passenger’s side rear tire had detached from the rim. Roadside Assistance was contacted and replaced the wheel with the spare tire. The manufacturer was made aware of the failure but provided no assistance. The contact was advised that without a part of the tire, no assistance would be provided. The failure mileage was approximately 7,000.

I was driving on the Highway at 70 mph and the rear passenger tire blew out. Thank God I was able to stopped the car safely and no one was injured. It was a scary moment and it is still is.My safety and other was at risk It is a brand new car with only 4397 miles. I took the car to the dealership and they replaced the tire. Upon inspecting the vehicle the following morning, the contact became aware that the tire was flat. The local dealer was contacted. The contact stated that the tire was taken to a tire repair shop, and it was determined that the tire could not be repaired, and a stone was removed from inside of the tire. The tire was replaced. The manufacturer was not notified of the failure. The failure mileage was approximately 800.

Service Brakes4 complaints

On April 29, 2026 around 10:00 PM, my 2025 Toyota RAV4 experienced sudden brake failure. The pedal became hard and I lost stopping and steering power. At the same time, Check Engine, battery warning, steering warning, and ‘Not Ready to Drive’ all appeared simultaneously with no prior warning. I brought it to a Toyota dealer the next morning and they found no fault. The vehicle is available for inspection.

The brakes have been overly sensitive... but then, without warning, braking power is completely lost. This has happened on 2 separate occasions... the pedal goes all the way to the floor and provides ZERO braking power. We can no longer trust this car on the road. Toyota sent someone to inspect the vehicle and they claim that they have found no problems or issues and that they have not been able to reproduce the problem. There were no warning lamps or message; there were no prior symptoms, other than the brakes were overly sensitive. These symptoms occurred immediately after purchasing the car. Toyota refuses to purchased the car back from us, and now we are stuck with a very dangerous vehicle. Interestingly, I have found others talking about EXACTLY the same symptoms on other Toyota Rav4 vehicles.

On November 9 evening, I drove to the Sugarloaf Mountain Trail Head (GPS: 40.05, -105.425) for a time-lapse photography. Date/Time: 2025-11-09 6:29 PM Location Description: Uneven dirt parking area commonly used by sightseers. Visibility was completely dark at the time of arrival. After arriving at the hilltop, as I parked the vehicle, while applying very light pressure to the accelerator pedal with my right foot, I felt a gradual drop of several inches on the passenger side of the vehicle. Instinctively, I moved my right foot to the brake pedal and applied pressure. However, I immediately sensed that the vehicle was not decelerating; instead, it was accelerating forward. I pressed the brake pedal more firmly, and the vehicle accelerated further until it came to a complete stop after colliding with a rock on the driver's side approximately 15 feet from the point of initial acceleration. Once the situation stabilized, I exited the vehicle and observed a red, non-oily liquid of normal viscosity leaking from underneath the vehicle on the front driver's side, which I later identified as coolant. Upon opening the hood, I confirmed that approximately half of the coolant had been depleted. Stopped to allow a car to pass. Once the car passed, there was acceleration of the engine but the car would not move. When the car gear was changed to park and then back to drive, it would not move. In order to get the car out of the traffic lane, the car was placed in neutral but the car brakes were locked. All this time the red 'park' on the dash and the red button on the console kept flashing. We called AAA and they arrived in 40 minutes. The man got in the car and stated they sometimes reset themselves. The car did start and he was able to drive it as the brakes were no longer locked. The windshield is warped and distorted. The dealership also told me that they can also see the warp in it. The original windshield was replaced, and the new one also has the same issue. The brand engagement rep I spoke with at Toyota told me that he has received multiple calls about the windshield issue. This vehicle was purchased 7/30/2025, and only has 271 miles on it and is on the original tank of gas. I have to rely on other people to give me rides places, as I am unable to drive this vehicle. The safety risk is due to the dizziness, nausea and migraine it causes me to drive it. My eyes feel like they are going cross eye, and I have to look out the driver's side window to get any kind of relief, which isn't allowing me to be alert while driving. This causes a huge issue for my safety as well as others. The Toyota Brand engagement rep that I spoke with said it is an issue and he has received multiple calls about it. The dealership has also acknowledged that they were able to see the defect.

The multifunction display does not automatically dim at night as indicated in the manual and in the settings. The display stays at the level manually set. This means that it can be so bright at night that the driver is blinded to the road ahead which could lead to an accident. It is very difficult and unsafe to try to dim it manually while driving.

Toyota USA and the dealer are refusing to honor the 60,000-mile Powertrain Warranty and Federal Emissions Warranty. The headlights on my RAV4 do not shine far enough in front of the vehicle and have a straight line cut off at top of beam. While driving in rural areas of the California foothills this issue is dangerous. As you go up and down and wind around rural roads you cannot see far enough ahead to see what might be lurking in the dark. We've had this car Since August 2nd 2025, 3500 miles to date and don't believe it is safe to drive at night on winding roads. The lack of visibility in front of you gives you the desire to slow way down below safe speed limits because you can't see out infront of you. LIving in the foothills for many years with deer, other animals or fallen rocks, it has become very scary driving at night with our new RAV4. Searching for other consumer opinions I have found many online forums with simular experiences, suggesting going to the dealership to have the headlights aimed hirer. This would change the stock legal safety settings and I'm afraid of possible legal reprocusions if we get into an accident. I have gone to my Facebook RAV4 group also to ask if anyone else has had this problem. It was unanimous, this is an issue with everyone. Others stated they won't drive at night. I also have a GMC truck and a stock unaltered, low to the ground 2017 Chevy Volt. I've never experience this issue with any other vehicles, present or past.

The vehicle completely lost forward propulsion when placed in Drive mode, rendering it entirely immobile. When placed in Reverse gear, the transmission violently over-pressurized, causing the vehicle to suddenly surge and jump uncontrollably. Simultaneously, the dashboard displayed 6 critical safety system failures: 1. Check Engine — electronic control malfunction 2. Vehicle Stability Control System (VSC/TRAC) malfunction 3. Secondary Collision Brake System malfunction 4. Pre-Collision System malfunction 5. Parking Brake System malfunction 6. Advanced Ultrasonic Detecting and Ranging System malfunction Ourisman Toyota Chantilly (RO# XXX) has confirmed complete transmission failure. Fault codes found: P28167F — Pressure Control Solenoid H Actuator Stuck Off, P08BA7F — Pressure Control Solenoid L Actuator Stuck Off. Estimated repair cost: $11,347.42. While this failure fortunately occurred on a local road, had it occurred while merging onto I-66 or I-95 at highway speeds, it would have inevitably caused a catastrophic high-speed collision. 2 incidents of Sudden Unintended Acceleration. No warning lights etc in either. Car damaged but available for inspection. Now encountering difficulties initating Toyota investigation. 1st incident (4/18/25; mileage 967) occurred after a hard left turn. After turn onto uphill street, I took my foot from accelerator to slow and maintain distance from a car backing down hill ahead. Instead, car self-accelerated and shot uphill, over speed bump with increasing speed. Did not respond to braking until it was close to hitting a parked car. Stopped with a slam; probably brake assistance engaged. No damage or injury, but I was shaken. Took car immediately to Toyota dealer to inspect. Was told RoB confirmed an event but not its nature or cause; identified nothing wrong. They hypothesized cruise control might have been triggered accidentally (no evidence, not plausible). Claimed they submitted report to Toyota national center. After 2nd incident, Toyota national said they had no record of 1st event. Their standard safety inspection protocols were not followed. Returned car to me without repair or follow up. 2nd incident ensued. 2nd self-acceleration event (2026) also occurred after hard left turn to make U turn at low speed, braking while completing turn. When I took my foot off brake, before I could press it on accelerator, car shot diagonally across street. Jumped curb and traveled at considerable speed 100' up landscaped slope. Destroyed 6 large shrubs and 3 trees 9' tall. Did not stop despite constant driver braking. Forward collision prevention system did not engage to prevent uprooting trees. Car stopped only after I turned steering wheel sharply to left to avoid hitting a full-size tree trunk. Then car responded to braking, acceleration ended, car stopped next to tree without impact. Terrifying life-threatening event but only damaged landscape and car. Police came, assessed incident, determined not a police matter, did not file police report.

The vehicle brought itself to a stop and the engine turned off. She reported that there were no vehicles or other object close to the front of her vehicle. The incident did not cause a crash or cause anyone to be injured. It was, however, a very scary situation that almost caused her to be rear-ended by the vehicles that were following her. The car was taken to Cobb County Toyota in Kennesaw, Georgia for an inspection the morning of May 28, 2026. As of this submission, the vehicle is still at the dealership service department for inspection.

This 2025 RAV4 will randomly move or jerk to the left while driving straight. It does not matter where you are in relation to the center of the lane. Sometime the movement is gentile and other time it is very strong bordering on violent. It will do the same thing with the lane keeping assistance, and pre-collision warning active or disabled. This issue could cause an accident if the jerky movement happened at the wrong time with another car alongside or approaching from the opposite direction. The movement has pushed me across the line a couple of times and it has happened at 15 mph all the way up to 65mph. The dealer did an alignment which did not change the problem. Now they want 425 to recalibrate the forward camara which 'might correct the issue'. This could get very expensive very fast when it should be covered under warranty. There are no warning lamps or messages to indicate the problem. This is a dangerous issue not just an inconvenience.

I purchased this car used in December 2025 with 10k miles on it. After 15k mileage, the rear tires were bald and I had to buy new tires. I then took it back to the dealer and had to pay to get an allignment, as I was concerned that is what was causing the tires to go bald so fast. That was less than 2 months ago and now I'm in getting the 20k service appointment and the rear allignment is now off way more than when I paid to get it checked. Apparently the rear wheels cannot be adjusted? The right rear is way off and this is for sure causing uneven wear on my tires. It is for sure an issue and should not be, in that the car is new. It is obvious it is a fault in design and for sure can cause major safety issues, let alone burn my money up in needing new tires every less than a year.

It’s a semi circle crack. Two employees at the dealer glass department concurred that it’s not damage from a foreign object hitting and that it’s a stress crack. INCIDENT: On 2/28/2026 at 7:45 AM, while entering a private driveway at under 5 MPH to park, my 2025 Toyota RAV4 Hybrid SE (VIN [XXX] , 3,164 miles) suddenly and involuntarily accelerated without driver input. I categorically affirm I did NOT press the accelerator. The vehicle struck a metal gate, causing frontal damage. Airbags did not deploy. OEM floor mat was properly secured; no aftermarket parts installed. SAFETY RISK: Sudden unintended acceleration at low-speed parking maneuver. Had a pedestrian, child, or another vehicle been in the path, this would have caused serious injury or death. The defect is unpredictable and the driver cannot anticipate or override it. INSPECTION: Vehicle inspected 3/20/2026 by Engineering Analysis Associates (EAA), a firm contracted and paid by Toyota — NOT independent. EAA downloaded electronic data and concluded the accelerator was pressed 1 second before impact. I dispute this finding: I did not press the pedal. No independent inspection has been allowed. Full EAA report was never provided to me. On 4/23/2026, Toyota Motor North America (Ms. Delve L., Legal Claims Administrator) declined assistance based solely on EAA's data. WARNING SIGNS: UNKNOWN. No warning lamps, messages, or prior symptoms appeared before the incident. The vehicle had functioned normally since purchase on 4/25/2025. On [XXX], at approximately [XXX], in Greenwich CT, while I was very slowly driving into a parking space, the engine suddenly and unexpectedly surged,. The vehicle overcame the brakes and raced forward and collided with a stone wall, about 3 ft away. This was a new (since May 2025) vehicle with only approx 3,500 miles on it. I had not had any previous problems with it and there were no warning lights on the instrument panel. At the time, I was proceeding at a very low rate of speed, 1 - 3 mph. I drive only with my right foot which was on the brake, not on the gas. The vehicle accelerated with NO action on my part and collided with a stone wall approx 3 ft away and bounced back several feet. Fortunately I was close to the wall so the vehicle did not have a long distance to accelerate and I was wearing my seat belt which probably protected me from injury. No other vehicle was involved. I was the sole occupant of the vehicle. I was wearing my seat-belt, the OEM rubber mats were securely in place. The airbags did not deploy and I was not injured. I had the car towed to the Toyota dealership by AAA because I was afraid it might accelerate into traffic at a red light or stop sign if I drove it before the problem was repaired. I have reported this incident to Toyota Motor Corporation USA, through their Brand Engagement Center (as instructed by the dealership) and they are sending out an engineer to the dealership to inspect the vehicle. 2025 RAV4 - While driving the car, changing from reverse to drive the steering wheel got stuck and would not go back to straight position. This causes an accident and hit another car. There was not indicator that there is a problem. Inspection was done by the dealership and asap and they identified and confirmed of faulty steering wheel angle. This car was bought end of June and accident happened first week of October. The car is since with the dealership with no resolution yet. Dealership have us call Toyota, but we were not provided with any help. One and half month in, we continue to pay insurance but we do not have the car.

With the car stopped & the transmission in the Drive or Reverse position with foot off the brake pedal: 1. Pushing the Power button once to turn the car off results in the transmission automatically shifting from Drive or Reverse to Neutral without automatically setting the Parking Brake. 2. If the car's front-end is on a slope pointed up-slope the car will freely roll backwards. If the car's front-end is pointed down slope the car will freely roll forward. The above sequence is very dangerous when on a steep slope. This problem was first experienced in a Home Depot parking lot that was uneven. Parked with the nose up-slope in a small depression. The Power button was pushed while the transmission was still in the Drive position & the car immediately started rolling backwards. Fortunately, nobody was behind the car. I quickly pushed the brake pedal to stop the rolling. The proper way to shut off the car, of course, is to put the transmission in the Park position first, which automatically engages the Parking brake, confirmed by a light on the manual Parking Brake switch. I also own a 2007 Prius, bout new. That car has a paddle trans lever, Parking Brake & Power switches. The parking brake is all mechanical with a parking brake pedal. Whereas the 2025 RAV4 does NOT have a parking brake pedal -- it has a switch. Pushing the Prius' Power Switch (off) automatically locks the transmission regardless of the trans mode (D, R, N) such that the front wheels lock& the car will not move, regardless of the parking brake setting. Thus, the Prius is intrinsically safe with locked front wheels unable to turn on a slope regardless of the parking brake setting. I discussed this with Toyota & my dealer. 1)I purchased my car during daylight hours. A few days later I drove my car in the evening and could not see street signs or the road 1/3 of the way up through my windshield. There is a definitive line where the windshield turns black. It is extremely dangerous. The Toyota engagement center told me to take my car to the dealership where it was purchased. I also took photos of what I could not see while driving. The dealership said there is nothing they can do and this is normal. The engagement center called back after they received the photos and told me their head technician could not see the street signs looking at my photos either. I was told to take the car back again to the dealership and once again told " there is nothing that can be done." They told me to put on my automatic brights. This feature does not help. The dealership also told me I could just put on my brights manually. This blinds other drivers on the road and then they put on their brights driving towards me, which decreases my ability to see while driving. the Toyota engagement center dropped the ball after 3 months of trying to report how dangerous this situation is. I am now reaching out to your department because of serious safety issues. I did not buy a car to be placed in a dangerous situation where I can't see to drive at night up. It is very dangerous driving up and down hills. 2)I also want to report - most of the time my gas milage ranges between 18 - 22 mph. The front camera fails which does not allow for the cruise control to operate. The speed odometer does not specify the correct speed when driving the car. The feature of telling you how fast you are supposed to go does not operate correctly specifying the wrong speed you are supposed to go. The tires have already been replaced because the metal was coming out of the tires causing the car to not operate correctly there was wobbling in the tires causing the car to feel like the tires were about to come off the tires. The transmission not shifting properly in eco mode.

Check engine light came on as well as hybrid malfunction message. While on the highway could not accel or go past 60 mph, gas pedal would then not work and speed went down, I pulled car over to breakdown lane and car abruptly stopped and would not move. Many warnings flashed and car needed to be towed. Temperature rose and ignition messages came up, steering wheel locked and car was not drivable.

The contact owns a 2025 Toyota Rav4 Hybrid. The contact stated that the vehicle jerked abnormally while driving at slow speeds with the transmission shifting gear. Whenever the accelerator pedal was released, the vehicle jerked abnormally. Additionally, the rear-view digital mirror was inoperable. There was a black spot on the mirror. No warning lights were illuminated. The contact stated that an unknown local dealer was contacted, but the vehicle was not diagnosed or repaired. The manufacturer was contacted and filed a complaint for the failure and referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 3,400.

I have owned this new vehicle for only approximately 2 months and have had at least 4 instances where the vehicle does not respond immediately to my foot on the accelerator when attempting to restart from a stop. Three times I was at a stop light and when I attempted to go as the light turned green there was no acceleration response from the gas pedal for a couple of seconds, and in two of the instances because I had my foot pressed far on the gas attempting to get power, there was a sudden acceleration and lurch forward. Most recently, on the fourth occasion I was turning out into traffic from a parking lot after being at a standstill and got no response and instead rolled slowly out in front of an oncoming car until after a few seconds the accelerator finally responded and I could go. I believe this is a safety risk as it could happen when I was attempting a left turn in traffic, which would subject me and my passengers/children to an extreme risk of being hit. I took the car into the dealership after the first two instances and they were not able to reproduce the incidents and offered no help. There are no warning lamps or indicators that appear when this occurs. The first incident occurred about three weeks into my ownership of the vehicle.

When remote starting the vehicle. The vehicle allows you to put the vehicle in gear from inside and turn off the parking brake allowing the vehicle to roll freely. The vehicle does shut off 1-2 seconds after but the vehicle is already in gear. To reproduce the defect you must remote start the vehicle using either the Toyota app or smart key fob. You then must be sitting in the vehicle once the vehicle is started you quickly depress the brake pedal shift into gear and turn off the parking brake quickly. The vehicle then shuts off while in gear allowing the vehicle to roll freely. This increases the risk of accident as the driver may not be in the vehicle and a child or other person may accidentally put the vehicle in gear while remote started. I believe this issue affects many other Toyota and Lexus vehicles as many share the same platforms and systems.
