Complaints Filed with NHTSA

The following are consumer-reported complaints filed with NHTSA. These are unverified allegations and are not proof that any defect exists.

Engine28 complaints

The Coolant Bypass Valve on my vehicle failed, leading to a risk of sudden engine overheating and potential coolant leaks into the electrical harness. This is a documented defect (CSP 23TE03) that Toyota has failed to address for vehicles over 100,000 miles, creating a safety hazard for high-mileage owners. This is a major concern on a known issue and those that don't have funds to pay for this known failure. They need to do a recall for ALL!

NHTSA #11738596

The contact owns a 2019 Toyota Rav4. The contact stated that while driving at an undisclosed speed, the message "Engine Maintenance Required - Visit the Dealer" was displayed on the screen. The vehicle was taken to a local dealer, where it was diagnosed, and determined that the coolant flow control valve needed to be replaced. The vehicle was repaired out-of-pocket. Upon further investigation, the contact related the failure to Customer Support Program: 24TE04. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 116,000.

NHTSA #11727186

I was driving down a road and my engine just stopped working lights came on saying there were several issues including the engine. I researched online and found there were recalls due to this same problem for the same make, model and year as my Vehicle, but there is no recall on my vehicle. The dealer confirmed the engine was seized up or whatever wording they used. No one has inspected it other than Rodland Toyota. There were no warning lights or messages or symptoms before the issue.

Was insinuating engine was over heating even in -4° weather has no hot air plenty of coolant. This is causing the vehicle to "buck" amd having to have to pull over and waiting several minutes to let the engine "cool off" I can make it about 3 miles in-between pull overs. After some research I feel like the issue falls under a current lawsuit against Toyota regarding the coolant valve based on reading other issues and what my rav is doing. It is a several thousand dollar fix through a dealership and one my family can not afford especially if it is a poorly manufactured part of the car. It is one of two vehicles we have and need to daily living especially having a child and no means yo offered another safe vehicle at this time. The coolant pump itself

NHTSA #11703331

2019-2023 model year Toyota RAV4s and Toyota Corollas are equipped with defective coolant bypass valves prone to premature failure which can cause coolant to leak out of a vehicle’s closed-loop coolant system, harming its ability to properly regulate engine temperature. This can cause overheating, stalling, increased emissions, and total engine failure and also allows coolant to leak into and damage surrounding engine components, including a vehicle’s electrical system, the complaint relays. The warning message that common appears for this issue is "Engine maintenance required Visit your dealer "

NHTSA #11676682

Scan Report done ! odometer 25266.Engine coolant bypass valve "A" control circuit failed test. Engine Control Module Code P2681-15. Message: factory service require !

NHTSA #11674099

The water pump failed at 49000 miles left me with a repair of $1500 at Kendall Toyota dealership

NHTSA #11674646

The engine maintenance light came on and I took the car to my independent mechanic. The "soft code" indicates that the coolant system valve/hose (engine coolant bypass valve) is showing in the closed position when it should be open. My mechanic has tried to order a replacement part, but is having trouble finding one. After my personal research, I see that there is a Class Action Lawsuit regarding this part from 2024 indicating that it can cause the car to overheat, stall, and potentially severe engine damage. Toyota has no formal recalls on this, but it has been indicated that some owners have received support from Toyota on models built up to 2021.

NHTSA #11675668

Faulty coolant bypass valve at 115,000 miles. Seems to be a repeat issue for many Toyota owners. Not covered by the powertrain warranty, but a known issue by the manufacturer.

NHTSA #11662167

The Engine Maintenance required light comes on when I use the air conditioner. I suspect the coolant bypass hose is not working properly.

NHTSA #11661674

Check engine light came on and code P268115 was shown for coolant bypass valve internal failure. Valve was replaced by dealership and I was told it would not be covered under the Toyota Customer Support Program because that only covers valves that leak externally, not internal failures.

NHTSA #11660223

Coolant bypass failure

NHTSA #11656905

An error code titled “Engine Maintenance Required. Visited your dealer” appeared on my dash screen. The error code was unable to be removed. I attempted have the car diagnosed by a private auto shop, however no faulty codes pop up. After research online, the problem was determined to be the engine coolant valve, which are faulty on many Rav4s. I eventually get a letter from Toyota for a Customer Support Program regarding the Engine Coolant Valve. When brought in for maintenance, Toyota pulled the error code “P268115” which is for an internal failure of the engine coolant valve. However, Toyota will not replace the part due to their Customer Support Program (24TE04) only covering the part for “small engine coolant leak[s]”. Essentially, Toyota will replace the part for being faulty if it had small leaks, but not if the part itself is faulty. When I asked the service member I was working with stated that this is a known problem and “[Toyota] should have just replaced the whole part, but they only do it for small leaks.” Currently, litigation is proceeding regarding the engine coolant valve, alleging Toyota knowingly put faulty parts in their Rav4s. I urge you to investigate this claim.

NHTSA #11655552

Engine Maintenance Required warning came on. Took car to dealership, diagnostic informed the issue was the coolant bypass valve. Repair cost is $781.28 + diagnostic $160.

NHTSA #11654152

The contact owns a 2019 Toyota Rav4. The contact stated while driving at approximately 35 MPH, the vehicle stalled. The contact pulled over and was able to restart the vehicle. The contact stated that the failure was a recurring failure. The contact stated there was no warning light illuminated. The contact had taken the vehicle to a local dealer. The vehicle was not yet diagnosed and was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 118,000.

NHTSA #11650086

The Engine Maintenance Required message is constantly on. The Engine Coolant Bypass Valve needs to be replaced.

NHTSA #11650703

Got brand new transmission put in then right after the Coolant leak started into cylinder number 2

NHTSA #11688332

Engine maintenance required when AC is turned on. This is an issue with the coolant by pass valve, many rav4 owners have reported but a recall has not been established

NHTSA #11622773

My coolant bypass valve gave out and Toyota is insisting it’s not covered under warranty my 10 year warranty. Toyota knew there was a defect and didn’t tell consumers.

NHTSA #11621662

The coolant bypass valve has leaked. It is available for inspection upon request, however this seems to be a larger ongoing problem across the country with this make, model and year. The check engine light did go on.

NHTSA #11614965

The “Engine Maintenance Required” light came on, indicating issues with the coolant valve. This is become a common issue with the RAV4. If this is becoming a common issue for owners, it needs to be looked into!! I commute and my engine can overheat.

NHTSA #11613661

My 2019 Toyota RAV4 had been making a loud noise at random intervals when driving at high speeds such as freeway speeds. The dealer said they could not reproduce the issue. A couple years later at around 65000 miles my engine maintenance required message came up and would not go away. I took my vehicle to get the code read and there were no codes. I kept smelling fuel or different unpleasant smells when driving the vehicle. The engine maintenance required notice would turn on every single Time I used the AC. I later went to the Toyota dealer and they said the coolant bypass valve had been leaking and causing the engine maintenance notice to come up. This is a new vehicle that has been maintained properly and rarely driven far. The part and labor was $824.

NHTSA #11612445

I got an alert on my screen that would not go away. It said Engine Maintenance Required.” Went to auto one & there was 0 diagnostics associated with any codes. The guy said that it is the coolant bypass valve and that there is a class action in the works. He said most places charge $600 - $700 to replace and to hold on to my receipts.

NHTSA #11611776

Coolant valve failed ($700)shortly after warranty ended; extended warranty did not cover repair. Coolant valve then caused harness to be unusable causing for a $4500 part replacement to be required. During the whole process the Toyota dealership maintenance told me that the engine repair required alert was for normal maintenance and directed me to continue driving with it until my scheduled service every time after speaking with three different people and a mechanic at the dealerships maintenance shop. I was then told after bringing in the car that I could leave although the alert was still present. Finally after the maintenance supervisor looked at my car I was told it was not safe to have been driving. Toyota made a faulty part that they expected to break and still put it in vehicles. They then are not even aware/ do not acknowledge this widespread issue in 2019 and 2020 rav-4s and Carrolas. This defective part can cause the engine to have catastrophic issues as I was later told by the maintenance manager at a Toyota dealership. However, Toyota is leaving car owners and leaders out of thousands of dollars in repairs and not acknowledging or warning customers of a dangerous issue.

NHTSA #11612290

Check engine light came on. Ran codes in vehicle and showed code P2681-15 engine coolant bypass valve needs replaced.

NHTSA #11609549

A warning message appeared that stated: “Engine maintenance required - Visit your dealer”. At that time my vehicle had 38,796 miles on it. I contacted the Service Advisor at Toyota of Newnan who advised that it was likely the Coolant Bypass Valve on the engine and that they have had issues and repairs relating to that condition on their RAV4 and Corolla models. Their mechanics confirmed that this was the fault and that failing to have it repaired / replaced could result in engine overheating and engine seizing which could result in lose of vehicle control and endanger other vehicles on the road. Sensing the urgency, I had the repair / replacement completed at a cost of $846.68. Service advisor stated that many of the other vehicles with the same problem were still under warranty and repaired at no cost to the consumer. In my opinion this recurring defect among two Toyota model lines is occurring with enough regularity to warrant Toyota issuing at least a Service Bulletin and covering the cost of the repair and reimbursement to those that had to pay for the repair.

NHTSA #11610629

2019 rav4 at 59,000 miles engine maintenance required popped up. Went to the dealer, service advisor knew exactly that coolant control valve needed to be replaced because it being so common. I looked it up and coolant bypass valves in the 2019–2023 RAV4 and Corolla models. Some owners have reported that this defect could affect the vehicle's performance and longevity. Now I have to spend almost 1,000 for a repair I didn’t even cause because Toyota built a faulty valve.

NHTSA #11609023

Engine error message showed up after extended warranty expired by 600 miles. To fix the error message Toyota charge $871 total combined diagnostic, part and labor. The problem was coolant by pass valve shorted.

NHTSA #11607975

The rear door (lift gate) hinges are failing and tearing away from the vehicle body, causing misalignment of the rear door, operational failure of the automated lift gate, and water leaking into the interior. This has occurred under normal use conditions. It appears that the hinge design is insufficient to handle the load and forces of normal operation of the door. The failure has been documented in pictures. The Toyota dealership where the vehicle was purchased stated that the vehicle is out of warranty and there is no recall for the hinges, therefore the owner is responsible for repair costs. Had the issue not been found and the hinges failed completely, it could have been extremely hazardous.

NHTSA #11737542

I have a 2019 Toyota Rav4 and my liftgate hinges are breaking . The welds are breaking and its causing my truck to no close all the way. Also is truck itself had misaligning itself to the driver side and its also breaking my tail light from the truck hitting it. This is a known problem with this model of car. Toyota has refuse to fix this problem. Leaving people to pay for this repair out of their pocket. The price i have quote for is $2,288.27 that's alot of money. This problem needs to be fix without us coming out of our pocket to fix something that Toyota has known about for some time now. I have been back and forward with my local deal ship snice 3/20/2026. Today i s 3/24/2026 and nothing has changed. I have called Toyota corporate to see if i can some help with is repair. All they keep saying is my car" doesn't meet the reequipments. There should be a recall cause this isnt one two RAV4s its hundreds maybe even tens of thousands.

NHTSA #11726863

I have noticed visible rust forming on the roof, specifically the front drivers side adjacent to the inside edge of the roof rail that runs forward to aft on the vehicle. The same stated rust spot is near where the roof meets the windshield. It is available for inspection upon request. My 2019 Toyota Rav4 along with several other 5th generation Rav4 owners online are experiencing rust in the same exact spot. This rust may be a result of the Toyota Customer Service Program (CSP) #22TE05 and T-SB-0016-23 that came of a result of the Fishkind v. Toyota Motor Sales, USA et al. No. Case 2:23-cv-02279 Class Action Lawsuit. This rust may have been formed by the leaking roof rail gaskets that have a slow enough leak rate to penetrate the roof rail clip holes and create moisture and rust over an extended period of time but not high enough of a leak rate to cause excess leakage into the vehicle that would immediately alarm the driver to have the vehicle taken into the dealership for immediate repair before the corrosion and rust can have a chance to spread. After several conversations with Toyota Corporate and my Toyota dealership they have denied me CSP inspection and any coverage or good will assistance through the CSP# 22TE05 & T-SB-0016-23 despite evidence of water residue buildup on the headliner visible through a UV blacklight. Even if the rust is not caused from the roof rail gasket leakage, there are still multiple other instances online of other 5th gen rav4 owners experiencing roof rust in the same exact spot adjacent to the drivers side roof rail near the windshield, which points to a manufacturing defect. My safety will be put at risk when the corrosion worsens and leaks through into the electrical components of the vehicle. My dealership witnessed the rust but denied performing an inspection of the roof rail clips since there is currently visible rust, likely due to the roof rail clips leakage. No manufacturer inspection. No noticeable warning symptoms.

The hinge has broken from the body, causing the liftgate to fail to close the back trunk area of the vehicle. The defective hinge has caused misalignment of the door and an unsecure compartment which is a violation of NC State Law according to NC GS 20-116. This puts the passenger's and driver of the vehicle and other surrounding vehicles at risk by failing to stay aligned or seal properly. I fear that glass may crack due to the currently misalignment. I have sought out further information from a Vehicle Repair Center, who confirmed there is a current Lawsuit on the Case No.: 5:24-cv-00912ZACH HUGHES, individually and on behalf of all others similarly situated, Plaintiffs, v. TOYOTA MOTOR SALES, U.S.A., INC., TOYOTA MOTOR CORPORATION, and TOYOTA MOTOR.

NHTSA #11717300

The rear trunk door would occasionally fail to open or shut for the last several years. It was inconsistent with this behavior and I was never able to get it to fail when at the dealership servicing department. This weekend when at the store, it failed shutting and I had to force it to shut. While driving home it rattled itself open. I had to pull over to inspect it and could not get it to close. When I looked closer, the hinge of the door had been ripped from the body on the passenger side of the vehicle. I have never been in an accident nor have I ever had any impact on this door.

NHTSA #11715053

Rear door liftgate hinges defective. They are too week combined with electric power struts. They are being pulled away from body of vehicle and become misshapen (bent, deformed, etc) and causes door/liftgate to be unbalanced and and stick out too far away from body. Now the door/liftgate can not close and be secured

NHTSA #11703424

Lift cylinders of power liftgate are operating at different times causing gate to twist. Liftgate fails to open and close automatically, causing prolonged beeping while liftgate is open. Hinges holding liftgate in place have started to bend and crack apart from the body of the car. An independent auto body shop inspected this issue and recreated the tailgate failure. Quote to repair included replacing hinges, lift cylinders and to recheck electronic connections.

NHTSA #11701480

The contact owns a 2019 Toyota Rav4. The contact stated that while the vehicle was parked, the contact noticed fluid leaking from the driver’s side rear door. An unknown dealer was contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and a case was filed. The failure mileage was approximately 40,000.

NHTSA #11677218

The contact owns a 2019 Toyota Rav4. The contact stated that while stopping the vehicle, the contact heard an abnormal slushing sound coming from the front passenger's side door. The vehicle was taken to the dealer, and it was determined that the door seals had failed and needed to be replaced. The vehicle was not yet repaired and remained with the dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 64,000. The VIN was unavailable.

NHTSA #11668150

Back doors are completely water logged (full of water) and won't drain .

NHTSA #11665188

1st: veh purchased Jan 2023 with new set of tires. Within 11k miles, about 9months: Rear tires unable to be aligned properly due to no camber adjustment. Rear tires have excessive inner rear (belts showing) with 11k miles on tires. Took veh to dealership and was told I just need a set of tires and an alignment. When the tech was questioned if it was an accurate alignment, we were told it would be off a couple degrees because the OE rear control arms did not have any adjustment. This was repaired by an independent shop using aftermarket set of rear control arms, a proper alignment and 2 new rear tires ($1200)--because Toyota dealer would not own any accountability of an error nor do the PROPER requested repair since proper repair did not involve OE Toyota parts. This was repaired, case closed and tires have 20k miles on them with beautiful, even wear. 2nd: Rear door hatch will not close electronically. 2 incidences of bent (left) hinge from forcing the tailgate closed and damaged rear taillamp from mis-alignment. Post hinges replaced: re-sets have been done, disabling door and re-enabling, and disconnected battery. Still will not close electronically. This had been occurring over a year and no one knows the fix. I will not be going back to my local stealership and the next one is 30 miles away.

NHTSA #11663561

I bought my Rav4 in 2019 brand new. For years, anytime there is a hard rain, the doors fill up with water. Water does not drain properly. I brought my car into Toyota a couple of years back. They told me that there was no fix and that when that happens, just to lift up the weather stripping on all four doors, to let the water drain. They acted like this is completely normal. Just last week, I brought my car in again to Toyota to see if they had a fix for it. They ended up charging me about $700 to replace all of the weather stripping on the doors. This is very clearly a manufacturing default and the customer should not be charged for what is a common issue with this car! If this is a well known issue at this point, why has this not been recalled? Again, the doors have had issue draining the water properly since day one and the car was brand new! Anytime the doors fill with water, you can literally hear the water splashing around in the doors while driving! I am not a happy customer. Toyota never takes responsibility!

NHTSA #11661564

The contact owns a 2019 Toyota Rav4. The contact stated that the liftgate hinge had fractured into two pieces, causing the liftgate to fail to operate as designed. The vehicle was not taken to the dealer to be diagnosed or repaired. The manufacturer was made aware of the failure and confirmed that the repair was not covered under recall or warranty. The failure mileage was approximately 60,000.

NHTSA #11653142

The rear hatch hinges cracked almost all the way through and had to be replaced. We were afraid to drive the vehicle because we didn’t want the hatch back to fall off while driving and create an accident.

NHTSA #11652159

The contact owns a 2019 Toyota Rav4 Hybrid. The contact stated while the vehicle was parked, the contact heard a water-slashing sound inside the vehicle. The contact performed an inspection and noticed that the driver's side rear door panel was filled with water. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The vehicle was scheduled for a diagnostic test. The manufacturer was not made aware of the failure. The failure mileage was approximately 54,000.

NHTSA #11626610

The design of some Rav4 grilles (particularly 2019-2024 models) allows rocks and road debris to easily pass through and damage the vulnerable A/C condenser, or radiator behind it, which leads to very expensive repairs. While driving at interstate speed and cruise control set, the cruise control stopped working and Cruise Control Malfunction errors, as well as Hybrid System Malfunction errors appeared. With other cars driving so closely behind me, the quick deceleration caught me off-guard and could have easily been hit from behind. I took the car to Toyota Dealership and appears there's already a known issue for a corroded traction cable and since not under warranty (due to mileage >120,000 miles) will cost $4668 to fix. Why is this not a recall if the problem is known?

The trunk door has been having major issues, where the hatch hinges and power liftgate strut are not operating correctly. The door is unable to close without manual assistance. Ghettel Toyota of Bradenton's service center has inspected and confirmed the malfunction. There were no warning messages or other symptoms of the problem prior to the failure; it first appeared in December 2024, and this car is a 2019 model with 37,000 miles on it. A second opinion was provided by a local body shop, prompting me to report this issue, as they informed me that this has been happening to multiple RAV4 vehicles.

Coolant leaked out of a vehicle’s closed-loop coolant system, harming its ability to properly regulate engine temperature. This can cause overheating, stalling, increased emissions, and total engine failure and also allows coolant to leak into and damage surrounding engine components, including a vehicle’s electrical system, the complaint relays. Coolant leak also corroded the sensor on the RAV4 coolant bypass valve, which can caused it to remain in the open or closed position and be unable to properly direct coolant throughout the engine.

A coolant leak from the flow shut‑off valve caused the engine maintenance warning light to remain on for an extended period. This issue is covered under Toyota Customer Support Program 24TE04. The warning light is a safety concern because it indicates an active engine‑related malfunction and can mask other warnings. The defect was first diagnosed by a Toyota dealership at approximately 97,000 miles, well within the CSP coverage window. The dealership stated the vehicle “did not meet criteria” and refused to perform the repair, even though the symptoms matched those described in the CSP. The issue continued until a second Toyota dealership later confirmed the same defect and replaced the coolant bypass valve. Because the vehicle had since exceeded 100,000 miles, Toyota refused to cover the repair under the CSP, despite the defect being documented earlier within the coverage period. Multiple attempts to resolve the issue with Toyota corporate have resulted in no returned calls or meaningful response. The defect caused prolonged operation with an illuminated engine warning light, which presents a safety risk and indicates a failure to properly address a known issue covered under Toyota’s Customer Support Program

The DCM is nonfunctional, which renders the safety connect features of the car inoperable. Additionally, the Bluetooth microphone is powered by the DCM causing the driver to have to switch the call output on their phone manually taking their focus off the road. This seems to be a widespread issue with many reporting this on online threads and Toyota offering many internal service bulletins. To me this is a major safety concern that is extremely costly to repair.

Got a "Hybrid System Malfunction" notification on my dash and took it to the dealer. I was notified that my high voltage hybrid cable is corroded, and that the vehicle is unsafe to drive. I understand that customer support program 22TE09 exists for this problem, but because my vehicle has over 100,000 miles, it is not covered. If this is a safety issue that Toyota has acknowledged and admitted is an issue, there should be a complete recall. I was FORCED to get the work done at a dealer because Toyota has a hold on the part. I attempted to get an independent garage to do the work, but they were not authorized to even purchase the parts necessary. Ultimately I had no choice but to spend almost $5000 for a safety repair on a 2019 vehicle that Toyota themselves have admitted to the engineering mistake, while also making it impossible to fix outside of a dealer. This is completely unacceptable. Thank you.

The contact owns a 2019 Toyota Rav4 Hybrid. The contact stated while driving 35-40 MPH, the message "Hybrid System Malfunction" was displayed. The contact pulled over to the side of the road and contacted the dealer, and was informed that the vehicle was safe to drive. The vehicle was taken to the dealer, where it was diagnosed with excessive corrosion to the high voltage wiring harness between the high voltage traction motor and the high voltage battery. The dealer informed the contact that the failure could lead to a vehicle occupant being electrocuted. The dealer determined that the wiring harness needed to be replaced. The vehicle was not repaired. The contact related to the failure of the Toyota Campaign Number: 22TE09. The manufacturer was notified of the failure and informed the contact that the vehicle could not be covered because the mileage had been exceeded. The failure mileage was approximately 115,000.

My 2019 Toyota RAV4 Adventure suffered a sudden catastrophic transmission failure while driving on the highway on April 28, 2026, resulting in loss of forward gears and a dangerous situation. On April 28, 2026, while traveling in [XXX] , my 2019 Toyota RAV4 Adventure (159,173 miles) experienced a sudden transmission failure with no prior warning symptoms. The vehicle began slipping, made a whining noise, would not properly upshift or hold speed (became stuck in 3rd gear), and eventually lost all forward propulsion, creating a safety hazard on the road. I had the vehicle towed to Zanesville Toyota in [XXX] . Their diagnosis (copy of repair order attached) revealed severe internal damage including: Torque converter clutch actuator stuck off Multiple pressure control solenoids stuck Transmission slipping and complete loss of all forward gears Fluid full but contaminated with metal debris and clutch material Whining noise The dealer quoted approximately $15,000 for a full transmission + torque converter replacement and denied goodwill assistance. This appears to be part of a known issue with the 8-speed automatic transmission (UB80) in 2019+ Toyota RAV4 models. I am concerned this is a safety defect that can cause sudden loss of power while driving, increasing the risk of a crash. Safety issue: The vehicle experiences intermittent loss of acceleration and transmission “slip/rev” events, especially when cold, from a stop, or at low speed. At times, the vehicle revs as if in neutral, then re-engages. The driver sometimes must press the accelerator fully to gain speed. In one instance, the vehicle was driven slowly for several miles, pulled over, shut off, restarted, and the function temporarily improved. When it started: Symptoms began immediately after a routine service visit at a Toyota dealership ([XXX]). First noted deceleration going uphill with recovery. Next day, multiple warning indicators/messages appeared and vehicle was towed to the same dealership. Dealer response: Dealership advised transmission replacement based on diagnostic codes and quoted approximately $10,000, stating fluid check was not necessary. Second opinion: Independent shop found transmission fluid level extremely low (reported approx. 1/4 full). They refilled fluid; initially vehicle drove normally, but intermittent slipping/revving continued after pickup (worse when cold/low speed). No external leaks observed by second shop at that time. Current status: Vehicle continues to show intermittent loss of acceleration/slipping behavior, creating a safety risk in traffic. No current check engine light, but symptoms persist. There is a known TSB out for 2019 toyota rav4 and some select other years. The issue is a slow speed transmission issue. It is due to a programming issue. At slow speeds, if you try to go from the brake pedal to the gas pedal too quickly, there is a serious lag before power is delivered to the wheels. Which is very dangerous when making specific turns. An example is what happened to my wife and myself the other day. We were in a median turning lane, cars were coming, as we were coming to almost a stop, my wife went to apply the gas. There was a serious lag before power was delivered and it almost resulted in a T bone collision. My wife went to apply gas, there was no power for several seconds, which caused her to panic and hit the brakes again. In that process, the power was finally delivered to the wheels and caused the car to pull about halfway into the oncoming traffic before it stopped from her applying the brakes. She then hard to quickly reverse to avoid the collision. The TSB calls for a flash of the TCM and a relearn procedure to be done. Which I am having done today due to the severity of the incident we had. This could have 100% caused a serious or fatal accident. The two dealerships I spoke with severely down played it as an issue. This deserves a investigation and a recall to be sent out. I was never advised of the TSB or anything.

I’m reporting a safety issue with my 2019 Toyota RAV4 that has recently started and is getting worse. The problem seems related to the start-stop system or something in the powertrain. When I come to a stop—at a stop sign, red light, or roundabout—the car doesn’t always respond normally when I try to start moving again. Sometimes there’s a hesitation, and other times it lurches forward unexpectedly when I lightly press the gas pedal, like you normally would. It feels like a jolt, almost like whiplash. It’s especially dangerous in roundabouts or tight traffic situations because I can’t predict how the car will respond. I’m worried I could accidentally hit another car, or get rear-ended myself because the car doesn’t move smoothly or reliably when restarting from a stop. Even when I turn off the start-stop system, the issue still happens. It feels like a hesitation followed by a sudden surge. The RPMs don’t change, but the car still lurches. Yesterday, for the first time, I noticed the car slightly accelerated on its own after stopping, which was very concerning. I found out through Toyota’s official website that there is a technical service bulletin about this issue that came out before we owned the vehicle. But because it’s not a full recall, the dealership said they wouldn’t fix it unless we paid the $80 diagnostic fee. We’re now just out of warranty. We’re making $580 monthly payments on a car that I don’t currently feel is safe to drive. There are no warning lights or messages on the dash. I’ll be taking it to Le Mieux Toyota in Ashwaubenon, Wisconsin, in two weeks to have it inspected, but I wanted to report this because I believe it’s a safety issue that should be looked into. I have two small kids and I’m genuinely worried about getting into an accident, especially when merging onto highways or driving in busy intersections. I’ve seen people say this system can be recalibrated, but the fact that this is happening without any dashboard warning and is worse.

The rear differential stopped working in January, 2025 and went into two wheel drive only. The message lit up on the dash board saying the vehicle was driving in two wheel drive only. We were driving home and the roads were slippery. We brought it to Mountain States Toyota and they confirmed there was a technical service bulletin (0086-19) issued in 2019, due to a Toyota manufacturing problem. It was issued three months after we bought the new vehicle. We were never notified and a recall was never issued in 2019 when Toyota knew about it. I contacted Toyota Brand Engagement and they agreed to pay for half of the $4,800 cost to replace the entire rear differential even though they acknowledged it was a Toyota manufacturing problem and possible safety issue. We don't understand why we were not notified about the known manufacturing problem that affected the drive train of an all wheel drive vehicle. We also don't understand why they won't cover the entire cost of the required repair to make the all wheel drive operational when they acknowledge it is a known Toyota manufacturing problem. We believe it is also a safety issue. Thank you! Toyota repair Invoices #1178424, #1169684

Toyota RAV4 5th Gen Power Lift Gate Hinge Issues Unpredictable Failure: If the lift gate or its components fail unexpectedly, it could cause injury to someone loading or unloading items, especially if they are unable to react in time. Obstructed Visibility: If the lift gate detaches while in motion, it could obstruct the driver’s rear visibility, increasing the risk of an accident. Risk to Other Road Users: A detached lift gate or related debris could pose significant danger to pedestrians, cyclists, or other vehicles on the road. Vehicle Stability and Control: A sudden failure or detachment of the lift gate components could destabilize the vehicle, especially at high speeds. Damage to Emergency Equipment: If the lift gate collapses or detaches, it could damage essential safety or emergency equipment stored in the trunk, leaving occupants unprepared in an actual emergency. Child and Pet Safety Concerns: Families frequently use RAV4s as family vehicles. A collapsing or failing lift gate could pose a serious risk to children or pets while loading or unloading. Potential for Escalating Damage: The failure of hinges or struts could result in cascading damage to other parts of the vehicle, such as electrical systems or the rear windshield, creating additional safety hazards. Delayed Emergency Response: If the lift gate malfunctions or becomes stuck in a partially open or closed position, it could impede access to the cargo area in emergencies, such as retrieving a first aid kit.

The vehicle began displaying intermittent warning messages and slipping into "Mud & Sand" mode without cause. Later, multiple dash lights illuminated, including 4WD malfunction, check engine, and coolant system warnings. The vehicle became undrivable due to overheating. Dealership diagnostics indicated a failure in the coolant bypass valve actuator the electronic water pump was receiving a signal but not responding, resulting in loss of coolant circulation and overheating. There may also be a shorted control panel causing related system failures. This problem has been confirmed by a Toyota dealer, and the vehicle remains at the service center pending resolution. The issue appears related to a known Customer Support Program (CSP 24TE04), which covers the coolant bypass valve actuator. There was no prior accident or abuse of the vehicle, and the vehicle only has 52,000 miles. Safety was compromised due to unexpected power loss and overheating while in motion. The issue has not yet been resolved. The vehicle has also experienced prior water intrusion through the roof rails, which may have contributed to electrical damage.

I am submitting this complaint regarding a recurring issue in the coolant bypass valve of my 2019 Toyota RAV4 XLE Premium. Although there was no check engine light, the vehicle displayed a persistent warning: “Engine Maintenance Required – Visit Dealership.” This message repeatedly appears on the instrument cluster and blocks critical driving information, including the speedometer, fuel economy, and other key displays. Even when the driver manually cancels the message, it returns within minutes. This creates a constant distraction and obstructs visibility, which poses a safety risk during driving. I took the vehicle to the dealership, where they diagnosed the issue as an internal short circuit in the coolant bypass valve and retrieved diagnostic trouble code P268115. This code relates to a malfunction in the valve’s electrical control system, which affects the engine’s ability to regulate temperature effectively. Toyota currently offers a Customer Support Program (CSP) for the coolant bypass valve, but only if the valve is leaking coolant. However, my vehicle's failure was due to an electrical defect, and Toyota does not cover this under the same program. This creates an unfair burden on owners and fails to address the full scope of the issue, even though the results—loss of proper cooling function—are the same. Many RAV4 owners have reported similar problems online, but no recall or technical service bulletin has been issued for this kind of failure. I believe this defect raises genuine safety concerns, including: Driver distraction due to persistent warning messages Obstruction of critical dashboard information Potential for engine overheating or reduced performance I respectfully request that NHTSA investigate this issue as a potential safety-related defect and determine whether Toyota should be required to expand support or issue a recall for this component, regardless of whether the failure is due to coolant leakage or internal electrical malfunction.

The recall covered the manufacturing period 4/26/2019 - 6/3/2019 and a recall notice was issued on 7/24/2019 for the affected vehicles and owners were notified on 9/20/2019. We purchased our vehicle on 9/21/2019 with manufacture date August 2019 and on 12/19/2024 we suffered a brake failure causing damage to the vehicle and our garage. For years Toyota has manufactured this water pump that freezes up due to manufacturing defect that allows the magnetic base housing to rupture. (This was late in weekend night - repair shops not open. I do see a recall investigation started (no recall yet) and just wanted to add my complaint to the issue. At first the temp gauge read normal but later started climbing. The engine light did come on the the DB2 said P26CB.

Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls (microphone inoperative). - Safety Connect / SOS light became intermittent or inoperative (directly impacts emergency calling, crash notification, and roadside assistance). This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. I request NHTSA investigation and urge Toyota to issue a recall or service campaign for all affected 2019–2025 RAV4 Hybrids.

NHTSA #11733750

The Data Communication Module has failed in my vehicle. This causes several major distractions and increases the risk of an accident. The media screen in the center of the dashboard will send an alert message every 15 seconds saying that ‘the connected device is not compatible’. At night, this alert message is especially bright and distracting. The lights on the SOS button are constantly blinking. More significantly, the SOS button does not work anymore. The only known solution to this problem is either to disconnect the DCM, or to pay more than $1300 to replace it.

Toyota RAV4 LE gas pump burst and caught fire leading to total fire damage along with major water damage from the firefighters putting out the fire. The vehicle was idle for hours at the time of explosion of the fuel pump. This was a major safety hazard/risk especially being that myself and two others were minutes away from going out to the vehicle before it all happened as I was getting ready to take a friend home for the night. A claim representative will look into this matter in coming days.

While traveling back from out of state, the fuel sensor and fuel gauge failed causing me to run out of gas when the fuel gauge stated I had just a tick under a 1/4 tank of gas left and the app read 35 miles left. While driving on freeway at freeway speed, the car sputtered, bogged down and then stalled, placing me in a very dangerous situation broken down on the shoulder of the freeway in the desolate country at night with semi trucks and cars flying by at estimated between 70-80 mph. While stalled and waiting for help, all the lights except the hazard flashers went out. The Sheriff assisted and added 2 gallons of gas that finally got the car restarted and I was able to get to the gas station which was 1 mile ahead of where I had been stranded. I kept filling the gas tank not allowing it to go under 1/2 tank on the fuel gauge the remainder of the trip home. I took the car to Tansky Sawmill Toyota for service, who thought it was a fuel sensor in fuel tank, which had to be dropped, resulting in a tube from the fuel sender being disconnected and not taking fuel from the other side of the tank and corrosion was cleaned out. They had to go through electrical system to check the fuel gauge which had corrosion on the electrical harnesses and sensors not caused by water or rodents - still unknown cause - resulting in $6500.00 to replace both electrical harnesses 82152-0R370 wire FR Door LH and 82141-oRB80 wire Instrument Panel! Known Toyota defect as the 2019 Rav4 Hybrids have already been recalled. Issues of this type with the regular Rav4s are posted all over the online Toyota boards. My platinum extended warranty on my well cared for 2019 RAV4 with 45,131 miles (purchased at 3 year lease end) refused to all of the charges and the dealership did not budge, sticking me with 6 hours of diagnostic fees over what the warranty allowed in the sum of $1014.37. These harnesses need recalled so no one else is stranded in a very dangerous place on the side of the freeway!

Electrical System,Unknown Or Other,Engine2 complaints

My 2019 Toyota RAV4 experienced a dangerous electrical and transmission failure that ultimately caused the vehicle to stall while driving and made me lose control. For several weeks before the incident, the vehicle showed subtle hesitation when accelerating but no warning lights. After the car stalled and spun out coming off the Ben Franklin Bridge onto I-76 near downtown Camden, police responded, confirmed the vehicle had unexpectedly shut off, and verified there was no crash damage. I later brought the car to the Toyota dealership, where they discovered corrosion on the battery cable, failed ABS sensors, and a weak battery. I paid $668 for a new battery, new cable, and ABS sensor repairs. At that time, the dealership did not inform me of any transmission issues and released the vehicle back to me. I made it only a short distance up the road before realizing the vehicle could not accelerate past 40 mph and was still malfunctioning, so I immediately returned it. The next day, the dealership initially claimed there was nothing wrong with the vehicle, but after I insisted they re-evaluate the car, they then informed me that the transmission was failing and needed replacement. This sequence of electrical instability, combined with the transmission malfunction, ultimately led to the earlier stall where I lost all power steering and braking assist and spun out into the median. The vehicle, diagnostic records, and replaced components are available for inspection. The sudden loss of power and control created a major safety risk to me and other drivers in a high-traffic area.

On 01/29/2025 at around 5:30pm I was driving home from work when suddenly the car stalled. I was going about 45 mph on a secondary to Lane Highway that was completely black because there are no street lights in that area. There were all kinds of warning lights flashing on the dash. I let my car drift to the shoulder was able to break and put it in park. I tried turning the vehicle back on but it would not start. My cell phone showed no Wi-Fi connection but I know the area very well and I knew I could walk several feet to a connection. Therefore spite concerns for my safety I had to leave my vehicle in order to call for help. I walked about 30 ft in the pitch black on this highway dressed in dark clothing on a shoulder that was very narrow. There was a pickup truck coming my way who flashed his high beams in my face but did not stop to see if he could help. Actually I was grateful for this because it's a pretty secluded area and I was very frightened for my safety. I was able to get a connection about 30 ft away from the vehicle and called my husband who said he would be on his way. I walked back to the vehicle got in and tried to start it. It would not start but all the warning lights were flashing on the dash. I decided I should take pictures of the warning lights in case we needed that for the dealer. After a couple of minutes I decided to try to start the car again and it did start. I was able to drive down the road to a Wi-Fi connection called my husband to let him know I was on my way. Once I got home the car seemed to be in order but there was one warning light telling us to take it to the dealer because there was something wrong with the hybrid system. We made an appointment to go in on January 31st. They inspected the vehicle to the tune of almost $200 and diagnosed the problem as follows: hybrid EV battery voltage system isolation internal internal electronic failure rear motor corrosion on both hybrid harnesses. Cost to repair $5,500.00

After a few minutes the car started. About a month later, my husband stopped for gas and the car killed right after he left the gas station, about 10 minutes later, it started. Took it to a mechanic, they couldn't find anything wrong with it because the check engine light wasn't on and the car wasn't giving any problem codes. About two weeks ago, I stopped for gas, the car wouldn't start. Finally got it started, and the check engine light came on. However, it's still not reading any codes.

NHTSA #11680269

Complaint Summary: Seatbelt failure on 2019 Toyota RAV4. Complaint Description: I own a 2019 Toyota RAV4, and the driver-side seatbelt has begun failing. The retractor and locking mechanism are not working reliably. Sometimes the belt won’t retract, sometimes it locks up randomly, and other times it doesn’t lock at all. This is a major safety hazard. I’ve owned many vehicles over my lifetime—older cars, newer cars, even vehicles over 50 years old—and I’ve never had a seatbelt go bad. This is the first time I’ve experienced a critical safety component failing like this. Toyota has issued seatbelt-related recalls on other RAV4 model years, but not on the 2019, even though the symptoms match what others have reported. This failure puts me at risk in the event of a crash, and it needs to be investigated. I am reporting this because I believe this is a safety defect, not normal wear, and it may affect more 2019 RAV4s than just mine.

Carry Handle, Shell, Base,Insert, Padding1 complaint

I am reporting a serious concern and safety issue regarding the SlimFit3 LX car seat. We purchased it for a [XXX] old [XXX] male. After adjusting the head rest to the proper height so that the harness straps were slightly below his shoulder, we placed him in the seat and his head was unnaturally pushed forward and down so that his chin was pushed into his chest. This was caused by the headrest sitting too far forward of the back padding so there was no way for him to keep his head in natural alignment with his back. The contributing factor is the minimal back padding and its inset into the car seat. There should be more padding to move the back forward or the headrest should be retracted into the seat more. We immediately returned the two car seats we purchased because of this and went with a different manufacturer. Not only is this an ergonomic/comfort issue, but it could introduce airway obstructing breathing issues if the child fell asleep. It is an extremely unnatural position. Also, I would be concerned about long term damage to a child's posture after repeated use. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On February 2, 2026, while driving approximately 65 mph on the freeway in Wisconsin, the panoramic sunroof glass on my 2019 Toyota RAV4 XLE Premium (VIN: [XXX] ) spontaneously exploded. The ambient temperature was approximately 20°F. Crucially, the glass buckled outward, indicating the failure was caused by internal pressure or a structural defect, rather than an external impact (such as a rock). There were no other vehicles immediately ahead of me that could have kicked up debris. Safety Risk: The explosion created a loud, gunshot-like noise that caused severe driver distraction at highway speeds. While no crash occurred and no injuries were sustained this time, the sudden structural failure posed a significant risk of loss of vehicle control. Status: No warning lights or messages appeared prior to the failure. The vehicle had no prior damage to the roof area. Power Train,Engine,Fuel/Propulsion System1 complaint

My 2019 Toyota RAV4 Hybrid (VIN: [XXX] ) suffered sudden engine failure due to coolant intrusion. On Nov 20, 2025, Johnstons Toyota in Harriman, NY (RO#XXX) diagnosed the issue and recorded DTCs P26CA31 and P26CB, which indicate failure of the coolant flow control/bypass valve. The dealer confirmed heavy white smoke from the exhaust with a strong coolant smell and advised the engine had failed, recommending a replacement. This vehicle is part of the early 2019 Japan-built batch widely known for premature electric water pump and coolant-flow system failures. Internal impeller swelling, seal leakage, and bearing seizure can cause coolant pressure spikes and ultimately head-gasket failure with coolant entering the cylinders. This defect has been reported by thousands of RAV4 Hybrid owners (2019–2021). Toyota has addressed it quietly through internal TSBs such as XXX and XXX but no public recall has been issued. The safety risk is severe: the engine began emitting thick white smoke and ran roughly, risking sudden power loss, overheating, or engine seizure while driving. A stall at highway speed could easily cause a collision. Toyota USA opened Case #XXX but declined assistance despite the defect being well documented. This failure was sudden, with no warning lights prior to the incident. Given the widespread nature of this defect and its clear safety implications, I request that NHTSA open a formal investigation into the electric water pump/coolant system failures in 2019–2021 Toyota RAV4 Hybrids and consider requiring a safety recall. I was driving to Kansas on a road trip and we stopped for gas when we noticed the rear drivers side tire tilting inwards (top of tire angled towards the car). We then drove it to a mechanic in Kansas to see if it is safe to drive further. She noticed the bolt for the rear suspension loose and almost out entirely. She then told us we could not drive the car any further because a few more bumps and the bolt would fall out and create major safely concerns. She also noted that she could see where the bolt was rubbing on the rear suspension sub-plate and could see the old rusted damage along with the new damage from recent driving. She also noted the uneven wear on the tire with the tread worn significantly more on the inner side of the tire. We towed it to Denver to have it looked at and the said the bolt and nut need to be replaced along with a tire alignment. There were no warning signs other than my car felt more bouncy than normal. The car bolt has been replaced but you can inspect the surrounding damage if needed. I have been experiencing water intrusion with my vehicle, I first noticed it by a mildew smell. Acknowledged under Toyota’s Customer Support Program 22TE05. This water leak has led to multiple secondary issues, including: Interior water damage Clogged A/C drainage system Sunroof drainage issues I reported this issue to a Toyota, but they denied repairs under 22TE05, claiming the issue was only due to a sunroof leak and clogged drains. [2/14] – Took my car to AutoNation Toyota due to a persistent water leak. The dealership diagnosed the issue as a clogged sunroof leak and clogged drains but did not provide clear documentation that they properly inspected the roof rail gasket. [2/18] – Dealership denied repair under 22TE05 but then stated they could replace the roof rail gaskets if my insurance paid for it—which contradicts their claim. [2/18] – I escalated my case to Toyota Corporate for review, but I am still waiting for a response. My car was left outside in the rain, potentially worsening the water damage. The dealership never provided me with a written repair estimate. Safety Concerns & Impact on Vehicle Use: I am worried about driving my car due to potential electrical system failure and airbag malfunctions caused by water damage. According to industry knowledge and Toyota customer experiences, a roof rail leak can directly contribute to all of the issues the dealership claims my car has: Clogged A/C Drainage System – Water leaking from the roof rails can enter the A/C system, causing drainage backup, mold, and excessive moisture buildup. Sunroof Leaks & Drainage Problems – If excess water enters through roof rail failure, it can overwhelm the sunroof drainage system, making it appear clogged when the root cause is actually the failing roof seal. Interior Water Damage – If the roof rail clips fail, water leaks into the headliner, side pillars, dashboard, and footwells, leading to electrical issues, carpet damage, and excessive moisture inside

However, the Toyota dealer (Crown Toyota, 1201 Kettering Dr, Ontario, CA91761) declined my request with the reason that my vehicle has passed warranty time. The vehicle was just inspected in the Toyota dealer a few days ago. The dealer's inspection report shows that the vehicle does not have any problems besides suggesting to replace tires and front brakes.

Electrical System,Structure,Forward Collision Avoidance1 complaint

Initially thought a seal on the windshield was faulty, so contacted the business who replaced the windshield. Business owner came to owner’s house Mon, Nov 4, 2024. He did a thorough inspection of all areas where water was identified by owner (dash, under the dash, on the driver side floor) and confirmed it was not the windshield. He then inspected the roof rack and determined the roof rack was defective and leaking water from the ceiling of the vehicle, down into the dash, and ultimately onto the floor. He recommended contacting Toyota because this was a known issue. By 12pm on Mon, Nov 4, 2024 owner contacted Toyota alerting them to the issue. An appointment was made to bring the vehicle in for inspection on Mon, Nov 11, 2024. Owners do not have a garage or carport, so a canopy was pitched over the car in an effort to prevent any further water damage and moisture absorbing canisters were placed in the car. The vehicle was not driven in the rain during the period after first noticing the leak and bringing it in to the dealership. Evidence of Electrical Damage: On Friday, November 1, 2024 the driver side seat became extremely hot, to the point of needing to pull over and turn the car off. The seat heater was not turned on at any point during this time. On Saturday, Nov 9, 2024 and Nov 11, 2024 while driving on the highway, the car (normally driven in Eco Mode) unexpectedly switched into Mud&Sand mode and all safety features switched off. Owner was unable to manually override this occurrence. The “Normal” button did not work. After a few minutes, the vehicle switched into the default setting and owner was able to put it back into Eco Mode. On Nov 11, 2024 owner brought vehicle into Toyota for diagnostics. Owner requested that the electrical components also be inspected and was told it was not covered under the Customer Support Program.

Engine,Forward Collision Avoidance,Lane Departure1 complaint

I was stopped at a red light. When the light turned green and I pressed the gas, the engine sputtered and almost shut off. Just before I came to a stop, the engine revved up and the vehicle accelerated. The uneven acceleration is concerning. I recently replaced the gas cap and serviced the fuel system, which leads me to believe there is a defect with the engine. Later in the day, the check engine light went on and AWD system, cruise control, and lane assist were disabled.

Engine And Engine Cooling1 complaint

Toyota has a coolant control valve problem on the RAV4 that it has known is a problem but will not issue a technical service bulletin or recall. It is only covered by the extended warranty. Any other is 800 dollars out of pocket. The warning ️ on the dash blocks the speed indicator and cannot be cleared until the fix is made.

No air bag deployed, wearing seat belts of course, still bumped head on ceiling causing small laceration, neck trauma, and Dr found I had also suffered a Vitreous Detachment (eye). Did not lose consciousness. Vehicle damage appeared to be front left corner but enough underneath to total vehicle. Very fortunate no other vehicles involved and that debris brought car to a stop instead of a big tree or wall. Two witnesses heard brakes... Service Brakes,Engine1 complaint

Engine: I got the 100k engine maintenance done. I replace my coolant and got an oil change. The engine maintenance required warning is still on my dashboard and will not go away because the diagnostic reader states I need a new coolant bypass valve. This is not apart of the factory maintenance, alot on rav4 owners are dealing with this, and it is very pricey. I think it is best for this type of situation to be recalled. Brakes: I got my front brakes replaced and I had issues with my front calipers. My mechanic said both calipers had holes in them and caused my brake fluid to leak which has affected my braking alot. It takes a long time to brake and I’ve experienced this even after I’ve paid alot of money to get my driver said caliper replaced, still need to get my passenger side replaced. My mechanic only uses toyota brand supplies so I’m not sure why there isn’t any longevity in the quality of supplies. I’ve only had the vehicle for 2 years.
