The following are consumer-reported complaints filed with NHTSA. These are unverified allegations and are not proof that any defect exists.
While charging my electric vehicle with this Enel X JuiceBox 40 charger, a burning plastic smell was noted. Upon examining the Enel X charger plug that was attached to my home's NEMA 14-50, it appeared partially melted and very to touch. Using a towel, the plug was removed and a small fire and sparks flew out of the NEMA outlet. The right half of the outlet's plastic shell was melted and the inner metal was charred. Same charring and melting was noted on the corresponding side of the Enel X plug. I turned the circuit breaker off and the sparks and fire subsided. Model of JuiceBox: 2JBO401RNA-PJWX-300

On [XXX], my 2021 Ford Mustang Mach-E (VIN [XXX] ) experienced a complete electrical failure related to its V12 battery system. The vehicle required roadside assistance twice during the same service call — first to jump the car to release the hood latch and again to restore enough power to unlock the doors. An elderly occupant was trapped inside until power was restored. After exiting, she became light-headed and fell; EMS responded and transported her to the hospital. This represents a serious safety defect because total electrical shutdown prevented door operation and normal egress. I am requesting investigation and possible recall review. My 12-volt battery died and due to my car being an Electric Vehicle there was no way for me to get inside my car without jumper cables. The safety concern was that it happened without noticed when i stepped out the car to get my [XXX] out of the back seat. Completely locking me out the car and no way to get inside. The car doesn’t have a conventional door handle and cannot open without electricity. I had to have emergency services come and break into the car. Melted High Volt Junction Box after DC Fast Charging. Car became compromised while 3 hours from home. Limiting speed and ability to recharge. This vehicle had the High Voltage Junction Box melt back in the summer of 2022. The original was replaced under Warranty. The replacement part they installed is also faulty. I do not DC Fast charge very often. Maybe 12 times in the lifespan of this car. The original recall for this issue was a Software upgrade in the summer of 2022. There appears to have been a recall for hardware in fall of 2023 but for some reason this car was not part of that recall. The part that Ford installed in July of 2022 has since been redesigned several times, but I was left with the old version and that Melted down. Please recall the faulty HVJB part installed in these cars that were impacted when the originals melted. This costs $3,000 to have them repair and is not covered by warranty any longer.

My cameras will not work and the screen will be black. Anytime I reverse or come too close to an object the cameras will show, but since there is a problem with the camera wire the image is just a black screen. This is a serious safety issue that Ford has yet to address and I have notified them about it, with an image and video of the problem.

The fuse blew for the 12v power port (cigarette lighter type) in the back hatch area of our 2021 Mustang Mach E. We went to try and replace it. Couldn't find the fuse panel anywhere. Researched it online and discovered it was under plastic panels in the frunk. I didn't want to break a panel so I went to the local dealer. We removed 4 or 5 panels and finally found the fuse box but we still couldn't check or replace fuses. The fuse panel is mounted with the face with the fuses pointing away from you. There is a small panel you can open but this only exposes the back of the fuse box where the connectors are. The dealer couldn't do it either with casual help. They said I need to make an appointment for a technician to block some repair time to replace the blown fuse. This is a terrible safety problem. If a key fuse blows when a dealer technician isn't available, the car could be very unsafe with no way to remedy this. In 50 years of owning cars I've never seen one with a completely inaccessible fusebox. It is true of this car.

9 months ago I received a safety recall notice from Ford Motor Company regarding the Mustang Mach E battery terminals could possibly melt together at high speeds and that the car will lose power. Considering the severity of this issue I took it to a dealership right away where it sat unable to be fixed for over a month. They did not offer me a rental and having 2 kids and no car was devastating. I had to go pick the car back up without the recall being repaired and have had to use the vehicle in the meantime because I have no other option. I filed a case through the Ford BuyBack Program and was denied. I filed a complaint with the Better Business Bureau in hopes of a remedy or mediation with Ford and was also denied. I just called 3 dealerships this week after receiving a letter in the mail that the parts were available and all of them said they do not have those parts. Because of this unrepairable safety recall, the cars value has depreciated and I am upside down my 20,000$. I can't sell it or trade it in without getting another loan just to pay it off and I refuse to do that for their mistake. There has to be another way. I need help and I'm praying I can find it here. I'm paying 735$ a month for a new car scared to drive every day.

while driving, the range on the electric battery went from 150 miles to only 64 miles in less than a minute. when i went to the DC fast charger it gave error and could not charge even after i verified that the fast charger was operational because the next car was able to charge car with no issue. after a while when i charged the car, it started charging but drastically reduced the battery range. i have attached the picture. at 78% it only gave me 76 miles range. usually at 78% it gives me a 180 miles or higher. took my car to the dealer to run diagnostics and dealer don't seem to agree to any battery issue.

Display gave warning - Service Vehicle soon and car did not start after being parked for about an hour. Outside temp was 65F. Car was driven about 400 miles that day including DCFC once. Had to be towed to dealership where it has sat for over 3 weeks with no repair and no information about what is wrong. I had someone familiar with the Ford system look up the codes who found that there was likely a problem with HVBJB that now is recalled. Not sure what to do anymore since dealership has car for 3 weeks now.

I want to draw attention to a significant concern: U.S. auto safety regulators announced in August of this year that they are currently investigating whether Ford Motor's 2022 recall of nearly 49,000 Ford Mustang Mach-E electric vehicles comprehensively addressed the associated safety concerns. The basis for the 2022 recall was that the high voltage battery main contactors could overheat, particularly from DC fast-charging and repeated wide-open pedal events. This overheating presents a very real danger as it could result in an immediate loss of power, markedly increasing the risk of a vehicular accident. Furthermore, after the recall, Ford issued a technical service bulletin, directing the replacement of the High Voltage Battery Junction Box on the recalled vehicles. However, the National Highway Traffic Safety Administration (NHTSA) has since opened a recall query. This action was prompted by 12 consumer complaints related to Mach-E vehicles that had supposedly received the necessary recall fixes. It is worth noting that the High Voltage Battery Junction Box has been replaced on my Mach-E already, and I am still facing problems. This is my third encounter with this particular safety issue.
NHTSA #11548102
An error message intermittently pops up on my vehicle that says, "Front Camera Fault", causing collision assistance and adaptive cruise control to suddenly stop working. This is a major safety issue, as a failure of adaptive cruise control while under operation may lead to a risk of an accident. Ford is aware of this issue and has issued TSB 23-2155, but they are treating it like a standard repair and not covering vehicles out of warranty. This should be a safety recall. See here for many others with the same issue: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
NHTSA #11726068
The car would intermittently give an error saying pre-collision assist not available and now this error is permanent. There is a technical service bulletin for it 23-2155 Lots of people have been complaining about this in the mach e forum https://www.macheforum.com/site/threads/front-camera-fault-pre-collision-assist-not-available-errors.18710/page-69#post-1026803 This causes people to drop out from the automatic driving bluecruise without notice and adaptive cruise control becomes unusable.

There are many reports of the Mustang Mach E "accelerating unexpectedly." This usually happens in parking lots or other situations involving low speed and a rapid turn of the wheel. I know exactly what the problem is, because it happened to me this morning. While parking in my parking garage, my car rapidly accelerated uphill and I only narrowly avoided hitting a wall. Here is why: There are two interconnected problems. The first is that the "cruise control on/off" button is located on the far left of the interior of the steering wheel, which makes it easy to accidentally press when doing a sharp, quick one-handed turn of the wheel (like when parking). When this happens, CC engages while in the act of parking. This is exactly what happened to me. I accidentally hit the button while doing a sharp twist of the wheel, I saw the CC engage, the car rapidly accelerated uphill without my foot on the gas pedal, and I would have hit the wall in front of me if I had been a split-second slower in hitting the brake. The second problem is that there is supposed to be a failsafe for accidentally activating CC at low speed, BUT IT DOESN'T ALWAYS WORK. Page 224 of my manual says "Press the button to activate system. When the system activates, the set speed is equal to whichever is greater, the current vehicle speed, or 15 mph...." This part is true, and this is how the unexpected acceleration to 15mph occurs when parking. But the manual continues: "If the speed is too low, or other conditions are not correct for adaptive cruise control activation, the system enters standby mode. When you are below 15 mph ... adaptive cruise control does not activate unless you are following another vehicle." This is FALSE. This did not work. Clearly the CC mistook the wall in front of me for "following another vehicle." And so, when I accidentally activated CC while parking, it lurched forward UPHILL to 15mph toward a wall a few feet away. Ford needs to move the button and/or fix the failsafe.

The front camera system stopped working, so I lost access to Pre-Collision Assist—(e.g., Collision Warning, Automatic Emergency Braking) as well as all other driver assistance feature. This would occur suddenly while I was driving (sometimes at highway speeds) where I'd have to swerve during a turn while using hands-free driving. Every time I start the car now, I have a permanent error message telling me the safety systems aren’t available. I’ve tried restarting the vehicle and checking for any obvious problems, but nothing has worked. The dealer told me it’s a hardware problem with the camera or its wiring, but fixing it would cost more than I can afford ($4K+). Since Pre-Collision Assist is a key safety feature, not having it working puts me—and possibly other drivers—at greater risk of an accident. This seems to be a common issue with Mustang Mach-Es, and I’m worried about the safety impact. I hope NHTSA can look into this and see if a recall or some other action is needed.
NHTSA #11669486
Front Camera Fault & Pre-Collision Assist Not Available. Vehicle displays these warnings every time a speed bump is driven over. These warnings take over the screen leading to distracted driving.
NHTSA #11633639
I frequently get front camera fault warnings from my car and then it disables the adaptive cruise control and the collision warning and assistance features. I have attempted to fix this and the Ford dealer held my car for about 14 days (also damaging it twice). They attempted to replace the module but could not get the new one to work and then opted to put the old module back in and apply a software update. There is a TSB for this issue to replace the coaxial cable that goes between the camera and the IPMA module. For some reason the dealer did not opt to replace this cable after attempting to replace the module. There are a large number of reports of this problem on Mach-E forums and Reddit and clearly a recall is necessary because dealers are not adhering to the TSB (perhaps to wait out warranties). They're also not providing loaner or rental cars as required as a way to discourage consumers from having their vehicles repaired (while keeping vehicles for a long period of time). A recall would resolve the fact that that this is a serious safety and quality issue and protect consumers who are out of warranty. The fact that I'm continuing to get these errors after two weeks of service is disturbing. I'm including a copy of the TSB and the detailed error message from the FordPass app.

The car detects changes in speed limits, presumably with cameras. When that happens an and cruise control is on, the car will change speed automatically. However, it often thinks “65” is “85.” Thus, sometimes, the car will unexpectedly begin to accelerate very fast. This poses an obvious risk: the car is speeding up a lot, past the speed limit.
NHTSA #11614979
Front Camera Fault Service not available. Pre-Collision Assist Not Available. These faults have been showing up for over a year. I have taken my car to the dealer about 10 times, to no avail. Every time you hit a bump it happens. As I write this my car is in the shop again. The internet is rife with people complaining about this problem.
NHTSA #11679923
Vehicle received "Front Camera Fault" and "Pre-Collision Assist Not Available" warnings multiple times. This is a known issue at Ford as they have a service bulletin (TSB 23-2155 [XXX] ) on it and is widespread. There are forum posts regarding this with 64 pages of people posting about it happening to them [XXX] ). When this happens, it disables Adaptive Cruise Control, Pre-Collision Assist, and Forward Collision Warning. If you happen to be using Adaptive Cruise Control or Ford's HANDS FREE BlueCruise, the hands free system immediately stops working. If you weren't to take over when this happens, it would be a disaster. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
NHTSA #11606721
The vehicle suddenly had a front windshield camera error light up on my dashboard from just driving regularly on the road. I currently have a crack windshield which I am having difficulty to replace because the front camera sensor error codes wouldn't allow the windshield camera to be recalibrated. My vehicle is currently experiencing this exact service bulletin but since is is not a recall, my dealership charged me to diagnosis this issue for $300 and claimed to fix this issue but is currently not solved. Pre-Collision Assistant was faulty and needed to be replaced, apparently there are multiple EVs having the same issue thus causing a safety hazard for vehicles expecting it to work. The warning lamp first came on two years ago for 30 seconds than it disappeared, and again last year however it stayed on so I took it to the dealership and they had collaborated the camera and the issue was resolved, however a month after that the issue came back for 30 seconds than disappeared. It didn't stop there, a week ago that same issue appeared and I took it to one dealership and they said that they had to replace the module which was currently on backorder, so I took it to a second dealership in which the technician updated all modules but was still showing the same issue so they replaced the module and the module was faulty so they had to order a new module which I am currently still waiting for the dealership to finish with the service.

There is a permanent fault on the front camera of the vehicle, making it unavailable. This has permanently disabled the vehicle's pre-collision safety feature. The adaptive/normal cruise control is also unavailable because of this. Prior to the permanent issue, the fault would occur randomly and without warning, creating an extremely hazardous condition, especially while utilizing the vehicle's "one-pedal" feature. Under this condition (using adaptive/normal cruise control and the one-pedal function), the fault would occur abruptly and without warning, cruise control would fail and the vehicle would brake simultaneously, without driver action. Several times, while driving on the highway, this happened to me. I was fortunate enough to not get in an accident. The dealerships have been unwilling to address my safety concerns. Please see to it that this is resolved. I have seen many consumer forums with this same issue.

The front camera system does not function which causes all forward collision safety features to become unavailable, as well as any cruise control or automatic high beam lights. Seems to be a common issues and is related to faulty fiber optic cables.

Pre-collision assist not working, despite multiple trips to dealership for this problem. Starting about 6 months ago, the Mach-E would periodically alert that the "pre-collision assist" was not working as a result of a front-camera fault. This was indicate by a visual indication on the dash of the front camera fault followed by "pre-collision assist unavailable" Recently, since recall 23B50 was performed on the vehicle, the pre-collision assist feature seems to not be working at all now. It's a safety risk to not have adaptive cruise control and pre-collision assist/emergency braking be unavailable.

The Adaptive Cruise Control cancel button on the steering wheel frequently fails to cancel ACC operation. The problem is that if the ACC is set at 70 mph, but you are following a vehicle that is going only 55, the ACC slows you to 55. If I intend to exit the interstate, I tap the cancel button, but the ACC stays operative. I change into the exit lane, perhaps rated at 35 mph. As soon as I am no longer behind the slow vehicle, the ACC starts to accelerate to 70 on a 35 mph off-ramp. This gets hazardous, especially in an EV with almost unlimited acceleration. One could easily be off the road before recognizing the problem. This is a problem that needs to be solved by Ford via their Over The Air Software Updates. On the internet user/owner forum this problem has been observed by numerous members, but no action taken by Ford who regularly monitors this forum. The software system is just sluggishly slow (compared to other vehicles, like my Honda Ridgeline) that it doesn’t even recognize there is an issue. Therefore no warning messages about a problem.

Ford had a recall on the brakes. The brakes were services 3 times in relational to this issue. Now again the front end shutters when braking especially at highway speed. The dealer says the pads need to be replaced and the rotors resurfaced .This is the 4th time in 42,000 miles. they will not warranty this on going issue. If the pads need to be replaced 4 time in 42,000 mile on a vehicle with regenerative braking there is a obvious issue. I feel this is an on going issue that was never corrected .

Lack of power assist braking and power steering. Anti-lock braking, Electronic Stability Control, and Traction Control System, as well as Hill Start Assist and Pre-Collision Assist are disabled. I bought a certified pre-owned 2021 Ford Mustang Mach-E GT. The brakes always kind of made a grinding but i figured it was the brake dust on the rotor since the car had sat on the lot for a while. I took the car into Maplecrest Ford in Union, NJ for this issue and they replaced the rear brakes under warranty. Shortly after that I felt a vibrating on the highway, I figured it could be the brakes bedding in. After that it became more persistent and I took the car to the dealer on [XXX] and they told me the front brakes had to be replaced. The car was purchased with 16,500 miles, went to the dealer at 17,000 miles and has 17,500 miles right now. It is impossible to believe that brakes on an EV (where most braking is regenerative) need to be replaced down to the caliper. I read Ford TSB 22-2023: Vibration/Pulsation From Brakes - Brake Pad Material Stuck To Rotor - Built On Or Before 06-Jan-2022 that describes this very issue. Ford essentially notes that pad material will become dislodged and get attached to the rotor. That they're not replacing this under warranty and that they haven't issued a recall for brake pads they've already redesigned because they can break apart is extremely dangerous. There's notes of this happening to cars on Mach-e message boards and Ford is aware because they've issued a TSB: [XXX] (for example). I would strongly encourage the NHTSA to implore a recall before someone gets hurt. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

After getting the HVBJB recall, I have had 1 pedal drive mode turn off unexpectedly about once a day. Since I expect my car to start breaking when I am off the accelerator pedal, when 1 pedal drive disables the car will move faster than expected if I am off the accelerator pedal. This could result in running a stop sign or hitting another car due to this unexpected lack of breaking. I do get a warning (while driving) on the dash screen that 1 pedal drive had a fault and was disabled. I have not taken my car into a dealer.

heavy vibration and pulsation from brakes when stopping from 50 mph. Steering wheel jumped out of my hands on a panic stop one time. I would say this is a safety issue that apparently Ford already knows about with service bulletin #22-2023. Ford refuses to make the repair under warranty because they say it's a wear and tear issue-even thought they admit to faulty brake pads. The problem was confirmed by the dealership and they wanted $1300 to fix their defective brake pads.

The safety of myself and everyone in my car is at risk when the pre-collision assist is not available due to this components failure. Yes, it is a daily occurrence and the camera never stays on for long. A fix was attempted once by a dealer. The component has not been inspected by either of those parties. Only the dealer whom the car was purchased from. Yes, warning messages pop up every time in the car and in the phone app about the issue. I am the owner of a 2021 Ford Mustang Mach-E and have experienced repeated failures of the front-facing camera system, which directly impacts critical safety features such as automatic emergency braking, lane-keeping assist, and adaptive cruise control. When the camera fails, these safety systems become unavailable or behave unpredictably, putting my safety and that of others at risk. The issue began around 40,000 miles. It is typically preceded by dashboard warnings such as “Front Camera Fault” or “Driver Assistance Features Unavailable.” I paid out of pocket for a repair at an authorized Ford dealership, but only four months later, the exact same issue happened again. Now, Ford is requiring me to pay for the repair again, despite this being a repeated safety-related failure of the same component. The dealership confirmed the problem both times, and the front camera module is still available for inspection if needed. Ford has refused to cover the cost under warranty or acknowledge any broader defect. I am also aware that other Mustang Mach-E owners have reported similar failures online. Given the recurring nature of the issue and the impact on critical driver assistance systems, I believe this is a serious safety concern that warrants a formal recall and investigation by NHTSA.

Vehicle was serviced following the ford directive to ensure that the service to the vehicle following the warning was followed correctly. Camera system that allows hands free driving, lane departure warning, collision warning and adaptive cruise control fails and all system offline. Extremely widespread on the 2021 models. Looks to be a cable route to the camera that was installed incorrectly. Went to dealership, they said they see this in almost all of the Mach E vehicles. Usually begins failing when hitting bumps -- system goes in/out. After a while, system drops out completely and all safety systems relying on them fail.

The car gives two messages, "Front Camera Fault Service Required" and "Pre-Collision assist not available." The cruise control does not work, however, the front camera does work. I had the vehicle scan done at the Ford dealer and was told it needs a 7 hour software update, after which it may need cables replaced. I do not understand how software goes bad. I do understand something may have happened to cables but that would not be checked until they had charged me $ 1,400 for a software update. This is a safety issue and, from what I read on the Mach E forum, is not where near unique to my vehicle.

Car gave me a warning to bring car to dealer, who told me I need to replace the High Voltage Battery Junction Box. This part was recalled on other models, but not my specific model, despite the cars using the same part. Dealer said it will cost $3,593 to replace and Ford customer service says there is no recall for it. While getting my recalls repaired at the dealership, I warned them of an issue my car was having with its front camera. I was receiving a “Pre-Collision Assist Not Available” notification each time I started my car. Upon investigation, the issue was described to me by the repair technician as. an internal failure IPM-A module on the car. This results in pre-collision no longer working on the vehicle along with the car no longer being able to utilize cruise control. The car is unable to detect vehicles in front of it to warn of a possible collision.

What component or system failed or malfunctioned, and is it available for inspection upon request? The front IPMA camera is faulty, not working. The dealership was able to diagnose it properly. How was your safety or the safety of others put at risk? The camera fault means that the vehicle safety features for front collision assistance and anything else that uses the front camera will NOT work. Has the problem been reproduced or confirmed by a dealer or independent service center? Bob Tomes Service center in Mckinney, TX has confirmed that the fault. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? In mid-November 2023, the authorized Ford dealership Hutchinson Ford of Forsyth, located in Forsyth, Georgia, sold and installed four Toyo A23 tires (size 225/55R19 99V) on my 2021 Ford Mustang Mach-E. After approximately 23,000 miles, the tires experienced catastrophic premature wear. Upon investigation, I discovered the dealership had installed tires with a load index of 99. The manufacturer's required specification for my vehicle is a minimum load index of 103. This incorrect fitment created an 880-pound deficit in the vehicle's total load-carrying capacity, posing a severe and constant risk of tire failure, especially at highway speeds. This is a critical safety issue for a heavy electric vehicle. The dealership's management has acknowledged the danger. When I raised this issue, the service manager stated they were "glad something bad did not happen." Despite this admission of the risk they created, they have refused to correct the safety issue, claiming the premature failure is normal wear as the tires are "nearing end of life." This constitutes a dangerous service practice by an authorized Ford dealer, which knowingly left a customer's vehicle in an unsafe condition. I am filing this complaint to document this safety issue and the dealership's refusal to rectify it.

The backup camera has failed numerous times. The camera will work and then suddenly stop creating a safety risk of hitting a pedestrian walking behind the vehicle when relying on the camera. The dealer has attempted to fix the problem by installing a new camera, new wiring harness, and a new module. The new module installed, under warranty, in February 2024 now requires a software update that only the dealer can perform at a cost of $1200. The vehicle warranty expired in July and Ford is not covering the cost of the update despite the module being new and requiring the update to work properly. It seems that if a part on a vehicle requires a software update to function properly that the part was defective and should be recalled. The dealer has inspected and advises to update the software on the module to regain proper function of the backup camera.

I am requesting immediate escalation regarding my 2021 Ford Mustang Mach-E (VIN: [XXX] ), which is currently undrivable due to being stuck in “Power Accessory Active” mode. The vehicle will not enter Drive. The dealer has diagnosed DCEM / high-voltage system-related faults and stated that recall repairs cannot be completed unless I first pay approximately $3,000 to repair the “main battery.” My concern is that the symptoms and failure mode exactly match the known High-Voltage Battery Junction Box / DCEM defect described in Ford Safety Recall 23S56, including: •Vehicle unable to drive •High-voltage system refusing to close contactors •Vehicle remaining in accessory power mode •Safety-related loss of motive power While my VIN may not be currently listed as eligible for these recalls, my vehicle is exhibiting identical symptoms and failure modes associated with the following Ford safety recalls: •HV Battery Junction Box / DCEM defect (Recall 23S56) •Powertrain / Software Update (Recall 22S29) •12-Volt Battery / Door Latch Entrapment Risk (Recall 25S65) It is important to note that other Mustang Mach-E owners have reported the same recall-related failures even when their VINs were not listed for these recalls. These failures involve contactor issues, loss of motive power, and vehicles being stuck in accessory mode — identical to the symptoms my vehicle is experiencing. On October 15, 2023, with my EV battery charged at 69% and about 2 minutes after beginning my drive, I suddenly received a warning on the dashboard which stated "Stop Safely Now". Within 5 seconds, the vehicle would not accelerate and eventually came to a stop. The first episode of this occurred on Sunday, October 15. My car had to be towed to Gullo Ford in Conroe, TX, where I purchased the car in March 2022. The vehicle was serviced by Gullo Ford's service department on October 16, 2023. The repair invoice references replacing the electronic parking brake harness connector and reprogramming the battery charge control module per recall on October 16. 2023. I picked the vehicle up from Gullo Ford on the afternoon of October 16. The car worked normally until Monday, October 30, 2023 when the same thing happened. About 2 minutes after starting and driving the car, I received the warning to "Stop Safely Now". The vehicle would not accelerate. My battery was charged over 70% when this occurred. The first time this happened on October 15, 2023, I was able to coast the car into a parking lot, out of traffic. On Monday, October 30, my car did not have enough speed to allow me to coast out of the flow of traffic which left me stranded in my car during rush hour, blocking one lane and the entry to a turn lane. This was a very unsafe circumstance as my car would not accelerate again, leaving me without a way to move out of the flow of traffic. Again, my car had to be towed to Gullo Ford in Conroe, TX where it remains at this time. I do not feel safe driving a car which will stop accelerating with only a 5 second notice to "Stop Safely Now". I have no idea when this will happen leaving me stranded in the flow of traffic with no way to move.

The car is frequently but intermittently reporting a failure with the front camera: "Front camera fault Service required". When this happens, the adaptive cruise control and other ADAS are disabled, which is a safety hazard while driving the car, especially at highway speeds. This issue began very infrequently in March 2024 and now happens on the majority of trips when driving the vehicle. I took the car to a Ford service center for repair. They did a software update on the car and told me that they were sure that the update would fix the issue. The front camera fault re-occurred within a week after having the software update done.
NHTSA #11614635
I am getting a warning in the car that the front camera is failing. The warning includes an error that the front camera failed, and that the forward collision is unavailable. Each time this happens, this causes the cruise control, lane centering and lane departure systems to disengage, which means the vehicles hard brakes at highway speeds when this happens. It disabled forward collision detection when this happens. Ford has TSB for this but when I took it in for repair/replacement they said it wasn't covered by anything and charged for the fix.

Vehicle in Adaptive cruise will unintentionally accelerate when a motorcycle is lane splitting to the right of the vehicle, causing the vehicle to accelerate towards a slowing or stopped vehicle. This puts me and other motorists at risk of a crash.

Car reports Camera Fault; forward-facing camera no longer working and adaptive cruise control, lane keeping, emergency braking and collision avoidance systems are all no longer available.
NHTSA #11623558
When driving two errors occur randomly and repeatedly. 1) Front Camera Fault, and 2) Pre-collision Assist inoperative. If driving with using any of the ADAS features, the car immediately begins to slow rapidly although each time it has occurred there has been no obstacle present. In one instance, it was on a freeway at 75 mph speeds nearly causing a severe accident due to the unexpected rapid deceleration. Luckily this was at 10pm at night. Had it occurred during higher traffic volumes I can almost guarantee there would have been a multiple vehicle collision. Researching online, it appears that the manufacturer has issued a TSB 23-2155 acknowledging the problem exists, but they do not see this as a significant enough safety issue to warrant a recall. When the failure causes complete disconnection of all ADAS features and the vehicle to attempt unneeded, unexpected, and unpredictable evasive maneuvers, I believe this to be a significant safety issue. The warning and attempted braking by the vehicle occur on a daily basis.

I have been having Ford try to resolve the safety systems from failing while driving for over 2 years. In October 2023 I took a settlement (hush money) for my time and effort, thinking that the safety systems were fixed. The dealership currently has had my vehicle for over 7 days and will not give it back until they have found a resolution (good for them). The dealer placed a buyback request with Ford Motor Co last week because of the extensive troubles, safety concerns and inability to resolve, however Ford has denied it because of the settlement. I am not wanting money, I am not wanting anything other than for Ford to take the vehicle back and pay off the loan. I have a teenage driver and I am at a point that the safety of this vehicle is putting myself, my daughter and OTHERS at risk every time it is driven on public roads. The lemon law attorney I used for the settlement refuses too help me any further stating the case is closed. I disagree that a vehicle with safety defects is OK to keep driving and that Ford Motor Co is OK with this. There is no short term or long term resolution to this matter, they do not know what is causing the system to malfunction. In every essence this is a lemon.

Front camera fault. Pre collision systems don’t work without it. Large online community with the same issues but ford refuses to acknowledge it’s an issue. They won’t help even if errors start happening before warranty is over. And once warranty is over they charge large amounts to fix. There is a TSB out for the issue and it seems like a pinched coaxial cable. Our safety is at stake and the number of occurrences can’t not be random.
NHTSA #11636382
Received intermittent warnings that the front collision warning system, cruise control and front camera were not working. The problem I think began after having my roof/windshield removed/reglued under recall. It was intermittent and I could not tell if software updates were resolving the problem, but then the problem became permanent at which time my car was out of warranty. Which does not seem fair since the problem began during the warranty period,. likely as a result of the windshield recall.

Received a front camera fault warning in the car. The fault disables the safety feature of pre-collision-collision assist. Had an independent service center check it out, they let me know there is a TSB from ford. Waiting for the a scheduled appointment.

Front camera stopped working after a long period of working intermittently (started to happen after passing warranty at 30,000 miles). Without that camera many safety features are disabled, like a front collision warning, lane departure, cruise control with adoptive speed, emergency breaking. The cable and processor issue was confirmed by Ford dealership service.

The 2021 Mustang Mach-E has repeatedly shown these "Pre-Collision Assist" and "Front Camera Fault" errors ever since we got it 6 months ago. The errors can come and go multiple times during any drive. For my wife it is just an annoyance since she never uses Adaptive Cruise. But for me or anyone else using Adaptive Cruise, it becomes a safety concern because this error suddenly cancels the cruise and being an EV it begins regenerative breaking and slowing down much faster than other drivers expect while its driver must react as quickly as possible to return to pedal control. It's my understanding that this is starting to happen on many modern Ford vehicles at around this age and dealership service centers have been given guidance on fixing it. Ours has been in for this service and the error is still occurring.
NHTSA #11581896
There’s a known recall. “2021 Mach-E models produced between Feb 24, 2020, and July 18, 2021, are at risk of the glass panels detaching or leaking.” But my car, a 2021 model, isn’t on the list. I just discovered that my headliner is stained and there’s a leak.

I have been trying for over one year to get the safety recall work completed. Ford tells me to contact the dealership to schedule maintenance. I contact the dealership and they refuse to schedule service and tell me they will call me when they are able to handle the request. I tried contacting ford and they told me they cannot interfere with the dealership's scheduling process.

One pedal drive system failure. Had the 22B50 charge port recall done on the 18th of April 24. After leaving the dealer upon approach to a light one mile away, one pedal disengaged during de-acceleration for the light. Warning did come up near the same time. It felt like vehicle was accelerating, but just loss regen. It has since happened daily. sometimes multiple times. Restart the car and it works normal. The recall was to reprogram the BCCM according to the dealer.

On April 19th, I picked up dinner and when I went to leave the parking lot, my vehicle turned on, but did not allow me to move out of park, nor shift into any other gear. I turned the vehicle on and off about ten times to see whether that might allow me to shift into another gear, but to no avail. It was later in the evening, so when I called the Ford customer service line I received an after hours message, and then thereafter I received a call from a Ford customer service center caller saying that they saw I was "stranded" and would send a tow truck to take the vehicle to the nearest Ford mechanic shop. They picked the vehicle up around 11pm in the evening. While the vehicle has had numerous other recalls, (windshield, etc.) I had not received any recall notice related to a battery issue. The dashboard showed a wrench in a car symbol as well as the battery symbol and a note that said "Stop safely now," and "full accessory power active." For the two and a half weeks following, I was not told what was wrong with the vehicle, nor was I provided a courtesy car. When I requested rental coverage, I was informed I would have to call "corporate" in order for them to cover the request, and when I did reach them, I was told that until they diagnosed the vehicle, they could not tell me whether it was my fault or theirs. After weeks of going back and forth, they agreed to cover a Ford rental (that I had to find on my own,) but have still not been given my vehicle as I have been informed that there are program wide software holds on the vehicle that prevent the technician from working on it. My lease ends in another month, and I have not been provided any information as to what my options are, or whether the vehicle will be fixed. I was also informed that I was not eligible for the Ford repurchase program as the vehicle is over the allotted 2 year/24K mile period, but I am extremely frustrated by the lack of care Ford has shown for my safety and ability to get around.

Mach e standard edition shows multiple signs of a failing High Voltage Battery Junction Box (HVBJB). The vehicle was bought certified pre owned with only 12k miles on the vehicle. At 16k miles (2 months of ownership) the vehicle showed a high voltage battery warning and power train malfunction warning. Three warning lights were displayed on the dash which were: a yellow turtle, wrench, and car with exclamation. Vehicle was taken to service department where they ran updates and said the issue was a one pedal drive error and ran a standard osp for a power train update (not related to this power train issue). They made no mention of the battery and power train issue on the service record. 1-2 weeks later and about 1k miles, the same issue repeated. The vehicle was taken to a different dealership where they indicated the car had a over heating charge coupler and unbalanced battery "A". Both of these show a probable issue with the HVBJB. The dealership has been insistent that they can't do anything without a stored code. Prior OSP's updated the car to limit power when contactors in the HVBJB exceeded .5 ohms and would show all the warnings we have seen. The main 12 volt battery was replaced at 1k miles, which Ford did not realize the failure could have been from the HVBJB, as it receives charge from the primary batteries. The dealership and Ford engineers have requested that we drive the vehicle until the safety lights repeat, knowing that this puts us in danger of a larger malfunction that could result in complete loss of power while driving. They say that they cannot work on an issue unless a code is tripped. I suspect a code will not trip as the first two times nothing was stored and the OSP patch that NHSTS requested helps prevent the complete failure. The code will only be stored in a complete failure event, which could lead to an accident. There are current recalls for this HVBJB, however it is only for the higher end models. Out of space :) Thanks

When stopped at a stoplight, in unbridled mode with one pedal driving engaged, the car surged forward on it's own. The surge forward caused my vehicle to rear-end another vehicle. I would assume the failure was the one pedal driving system and it is available for inspection. The problem has not been reproduced or inspected yet, but we have an appointment for a Ford dealer to evaluate the car. The only warning was the pre collision warning which did go off.

There is a software problem which was introduced in a previous OTA which causes drive modes to change. These drive modes are Whisper, Engage, and Unbridle. Each has their own acceleration/throttle response characteristics, as well as steering and suspension feel. You also have the ability to enable one-pedal drive in the drive mode menu. The software bug will randomly throw an error on the main screen that says "Drive Mode Not Available" change your drive mode from Whisper or Unbridle to Engage, affecting the throttle response that can occasionally lead to unintended acceleration. On occasion, it can also lead to disengaging of the one-pedal drive mode at the same time, which can cause the vehicle to not slow down when the foot is lifted off the accelerator. If the driver is unaware that 1PD has become disengaged unintentionally (which cannot be re-engaged without power-cycling the vehicle), it could lead to a crash as the driver is not expecting to use the brake pedal. In a moment's notice situation, it could be deadly. The vehicle has not been inspected, however, this problem has been reported by many other Mustang Mach-E owners. I will link my forum post at the end of this report which has other claims of this fault. The vehicle is available for inspection upon request. It cannot be easily reproduced as it is entirely random. Yesterday (11/8/2023 @ 1:40 EST), I was pulling into a parking spot with concrete posts around it. This time, it happened as I was turning into the spot. I was in Whisper mode this time. I noticed the car SPED UP a bit as I was letting my foot off the accelerator, and I had to slam on the brakes. Sure enough, "Drive Mode Not Available" was up on the screen. I was about 1 inch away from this concrete post. If my reaction time was just a bit slower in hitting the brakes, the vehicle would have crashed itself, relatively outside of my control. https://www.macheforum.com/site/threads/huge-safety-issue-1pd-drive-mode-not-available.31712/

Twice in about a week my 12V battery has died without warning. The car is inoperable and there’s no way to open the car doors or even the trunk and front trunk. We’ve been locked out entirely. If this happened while you’re in the vehicle you’d be stuck in the car since none of the accessory functions (including locks) function when this happens.

Vehicle experienced immediate loss of motive power while in motion at 45 mph after "Stop Safely Now" alert. This is a repeat power failure. The vehicle was in service for 75+ days (Dec-Feb) for this exact defect. The Ford service center said they replaced the “battery controller” yet the issue recurred. The car previously flagged this alert while in motion 2 times prior (one on the highway going ~65mph and on another major street going roughly ~45 mph). As a Ford Reacquired Vehicle (Buyback), the failure to resolve this catastrophic electrical fault creates an extreme risk of crash and entrapment in traffic, putting myself, my family and the drivers in vicinity at significant safety risk. The car is currently at a different Ford Service Center. They have been able to reproduce the issue and are currently running diagnostics to identify a resolution.

After recall-related software updates were performed on my 2021 Ford Mustang Mach-E, the vehicle began experiencing repeated electrical and climate control failures. The heating and defrost system stopped working, leaving the vehicle without heat and causing the windows to fog, which reduces visibility in cold weather and creates unsafe driving conditions. The vehicle has been taken to multiple authorized Ford dealerships for inspection and repair; however, the problem returned immediately after the repair. The issue was reproduced and confirmed by a second dealer. A required part is currently on back order until the end of Feburary, leaving the vehicle unrepaired for an extended period. In addition, the vehicle experienced 12-volt battery failure and slow charging after the last recall update. These issues were not present prior to the recall. There were no warning lights before the failures occurred. During severe winter weather conditions in Pennsylvania, the lack of heat and defrost made it extremely difficult and unsafe to operate the vehicle. Because of reduced visibility and freezing interior temperatures, the vehicle could not be safely driven and, due to frigid temperatures, has remained parked. There is a repeated front camera fault that prevents the "Pre-Collision Assist" from working, which could lead to a car accident and possible injury/fatality. It has been to the dealership many times over more than 2 years, and the issue still has not been resolved. The Ford dealer had been able to confirm and reproduce the issue. There is no consistency for when the alert appears on the car that the "Pre-Collision Assist" is not working, but it happens often.

Some of the driver assistance warning systems may not be operable due to multiple and frequent warning messages. Messages are: Front Camera Fault Service Required; Pre-Collision Assist Not Available; Lane Keeping System Fault Service Required. Ford is unable to correct these making it potentially unsafe. Additionally, they are not covering the attempt to fix this under the extended warranty. The forward collision detection system failed first. Then the car suddenly would not move and gave a pull over immediately warning on the dash and also flashed a battery light. It was 15 degrees outside and my wife and son were stuck in the cold. Not a safe environment. This is inexcusable for a car to be driving fine on minute and to be completely inoperative the next.

The car had an issue with the 12V battery which caused it to be completely disabled and unresponsive to the key fob and phone as a key. My wife had gotten out of the car without knowing this had happened and went to the rear door to let out our infant son, but the door would not open. We called emergency services and roadside assistance. AAA arrived first and tried to open the lock but were unsuccessful. I had to smash the window to open the door and rescue my son. Emergency services arrived after I had already done this. The only reason I know it was an issue with the 12V battery is because the Ford dealership told me after I had the car towed there for inspection. The only way to open the car in this situation is to use a portable jumper to jump two wires hidden in the bumper which open the Frunk. Then you need to jump the actual battery that is located in the frunk. It does not have a mechanical release for the doors. This supposed fix is highly dangerous as it requires one to have a readily available jumper and the 12V battery needs to be completely drained in this scenario. In my case, this didn’t work even when the Ford technician arrived. The only possible way to get in was to break through a laminated glass window which was incredibly difficult to break even with the correct tool to do it.

I bought this car in August of 2024. It started having issues with the door lock in September and in December I decided to sell it back to the dealer but was in a car accident on the way to the dealership. Unfortunately the door jamming issue caused repair problems and the auto repair shop could not fix the door issues. I lost the car to the repair shop in a lien sale due to the cost of repair which was over 20,000. In August of 2025 Ford issued a recall which allowed the auto repair shop was able to fix the car and sell it to a dealership. I have contacted Ford several times in relation to the issues with the car and how they caused me to loose the car, but they have not responded despite the promise to do so in 7-15 business days. Loosing this car has caused deep stresses in my life. I have lost several business clients because I'm no longer able to get around. I have had my life impacted to the point of having to relocate. I bought the car outright paying 45,000. Ford has refused to do anything to compensate for my loss.

My car has repeatedly had to have rotors replaced due to warping. Between 12,000 and 36,000 miles I’ve had 7 pairs of rotors replaced. After exceeding 50 mph, when braking I decided violent steering wheel vibrations between 43-35 MPH when braking normally. I have service records and video to back up my claim. Ford is calling this “normal wear and tear” however no other 2021 mach-es vehicles in my geographic area have had this issue, in fact none have had to have rotors replaced at all. This is a serious safety issue, occurring approximately every 3,000 miles. This points to a defective vehicle, Ford refuses to cover the repairs now that the vehicle has passed 36,000 miles despite having a verified service record of 7 rotor replacements. The date below is for the most recent issue, it has been going on for a year and a half. Ford clearly doesn’t understand how to fix it, as the same fix has been recommended every time to the same result. This feature reset it self after the car was parked overnight. Yesterday while driving over many miles this same warning message appeared to happen again and the one pedal feature quit. This happened while vehicle was traveling in an active school zone very near a crosswalk being used by students. This could have been a very bad situation if not quickly going from one pedal braking to two pedal braking which is needed to stop the car.

Took vehicle to dealership for warranty repair as dealer confirmed parts available. Did not do repair due to Ford issuing a email to stop all repairs under this recall until further notice. Contacted Ford no reason why recall repairs stopped, no information when they would begin again. Ford recall notice blatant information without explanation. NHTSA site information is not accurate, parts are available but for undisclosed reasons they will not perform repair.

The rear camera has malfunctioned. When the vehicle is put into reverse the area usually covered by the rear cameras is blue with white tire tracks indicating the direction the vehicle will travel. Obstructions such as people or other vehicles are covered in blue and not visible on the screen. The dealer has confirmed the problem and ordered replacements in May. In the meantime I am hesitant to drive this vehicle since there have been a couple of near misses in crowded parking lots. This vehicle is available for inspection. I have not received any communication from the dealer. With safety at risk something needs to be done

Having completed the high voltage junction box recall with the Mach-E a week or so before, I was braking in order to complete a left-turn in one-pedal drive mode (therefore lifting my foot off the accelerator causing the vehicle to slow down). I received a notification as I did so that one pedal drive mode had a fault and was deactivated (this appeared on the screen by the steering wheel). This caused the car to lurch forwards, as it shifted from OPD to standard drive mode. Fortunately no cars were ahead of me and I was able to quickly slow the vehicle by braking, but I'm alarmed as to what would have happened had this been in traffic.

On April 10, 2024, my vehicle experienced a warning message to service vehicle and a sudden loss of power while I was driving on the freeway. The issue was caused by a failure in my high voltage battery junction block. Something that Ford had already issued a software update vehicle to my vehicle for a recall. However clearly the software update was not an appropriate fix for the issue and should not be considered fulfillment of the recall. In fact, Ford has an open recall for the extended battery configuration of my vehicle to proactively replace this defective part. I am reporting this issue to the NHTSA because clearly the software ‘fix’ did not resolve the safety issue AND it’s scary that standard range batteries are neglected from the overall recall that applied to the rest of the model. Ford needs to be proactively addressing this issue for all affected model years, not just certain configurations. Frankly it is wild that the NHTSA accepted a software update as an appropriate recall fix for a hardware component overheating and melting while driving. It didn’t work and should not be considered a fix.

The contact owns a 2021 Ford Mustang Mach-E. The contact stated while making a left turn into a parking space, the vehicle collision sensor did not alert the contact that a vehicle was nearby. As a result, the contact crashed into another vehicle while attempting to park. During the impact, the lower passenger light was damaged. No injuries were reported. The vehicle was taken to the dealer where it was diagnosed that the camera feature on the vehicle was malfunctioning due to the changing of the original brand of tires. The dealer did not provide any parts that needed to be repaired or replaced. The vehicle was not repaired. The manufacturer was made aware of the failure and a case was opened. The failure mileage was approximately 24,500.

This vehicle has 3 outstanding recalls: 21V711, 21V712, 23V687. The car was purchased new from Orchid Isle Ford in May 2021. They are the only electric vehicle Ford Dealer on Hawaii Island (they operate another non-electric vehicle dealership on the island). On [XXX] I had the annual maintenance performed. I requested that recalls 21V711 & 21V712 be done as well. the dealership declined and stated they would call me when they could do those recalls. They have never called. I started calling the dealership every 3 months beginning in Sept 2022. They have always refused to perform those recalls w/o explanation. In February 2024 I called to schedule the 36-month service and have all 3 recalls done. I was advised that the dealership could not do any of the recalls. I was called on Feb 23 by a person identifying themselves as the owner's sister (Mary) promising they'd make things right and asking for two weeks to arrange for the glass recalls to be done. I have had no contact since and have again been told by the service department that they can not and will not perform these recalls at any time regardless of where I bought the car. On [XXX], I emailed Ford Motor Company's CEO James Farley advising his office of the problem in getting three safety recalls performed on Hawaii Island. I have received no response from him, or any of the other emails sent to other officers of the Ford Motor Company nor have I even received an acknowledgment of receipt. At this point, I can only presume that Ford does not care if recalls are done. Recall 23V687 has the potential to strand me on rural roads where there is no cell service. The two glass recalls (21V711 & 21V712) can potentially cause death or serious injury in an accident. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I drove the car to work with no issues. I left work a little over 9 hours later. I was able to enter car without issues. Turned on car and didn’t notice any issues. My phone connected as usual via CarPlay and I navigated home. Leaving the work area is low speed, so I never exceeded 20 mph and just went by feeling, but once I was on a more open stretch of road, I noticed that the instrument cluster was frozen and the speed it was displaying felt too slow compared to how I felt I was going. In essence, I couldn’t tell how fast I was going. After closer examination (mind you I was driving while figuring this stuff out), I noticed that the instrument cluster would update once in say 10seconds or so (but it was variable). I tried to set cruise control to initiate blue cruise, but I couldn’t tell what speed cruise control was being set to due to this enormous lag. I also wasn’t sure if the system would also exhibit this lag. Finally, I noticed that even the lights indicator for the blinker would refresh very slowly but the audible “click” associated with the blinker was normal. The drive home was very distracted and painful due to this issue. I documented the behavior in a video if needed.

After transitioning from i35 south, to Highway 183 towards Austin-Bergstrom International Airport, the auto-pilot (Ford Blue Cruse) fails, the speedometer fails, the battery level indicator fails, the instrument cluster fails, cruse control picks a random speed and struggles to keep it, lane keep assist is erratic at best. This occurs every time, and in both directions, I take my vehicle on that highway. I took a video of it this last time, and offered it to Ford. They recommended I file this report with this agency.

This is a follow up to ODI 11510437 that was filed via the phone in 2/2023 related to now NHTSA Recall 23V687. The vehicle in question experienced exactly the issue described in the recall notice. Our incident was part of the NHTSA investigation. The vehicle had its high voltage battery junction box replaced in +/- March 2023, but the new box was the same design as the old box that is poorly engineered. Notices sent out in Fall of 2023 indicating that there was a recall were not sent to us for this vehicle. Similarly, the notice sent in April 2024 informing Mach E owners to schedule appointments at their dealer was not sent to us for this vehicle. Upon contacting Ford, they indicated that this vehicle is not part of this recall. After insisting on speaking to a supervisor, he claimed that it was now a part of the recall but refused to provide me any written acknowledgment of this. When pressed further he then said that our vehicle was not part of the recall because the part was already replaced. When I explained that the part replace was the same defective part he told me to call the dealership or NHTSA because he would do nothing further. He went on to say that there was nothing further he would or could do, nor would it transfer me to anybody at Ford that could address our unique circumstances. Upon calling NHTSA they advised, (amongst other things), that we file a complaint via this portal.

I have experienced multiple powertrain malfunctions since the original recall for this issue was completed back in 2022. Approximately 4-5 of these warnings were temporary and cleared within a few hours or after a visit to the dealer to diagnose. Most dealer visits yielded nothing as the car had cleared the errors and codes it was throwing. In 2021, I experienced a complete vehicle failure with a "Stop Safely Now" and "High-Voltage Battery Warning", right before the original recall was performed. My most recent malfunction has also caused a complete vehicle failure, exactly like what happened in 2021. I am concerned that these issues will continue and eventually happen while driving, creating a massive safety issue.
